目前西医对CAG及胃癌前病变尚无良好对策,而祖国医学将其归纳为"胃痞"、"痞满"、"胃痛"、"嘈杂"等范畴,在治疗上有良效.饮食失宜是CAG发病的重要因素,故本文从中医内伤之饮食失宜角度探讨防治慢性萎缩性胃炎、肠上皮化生(Intestinal Metaplasia,IM)及不典型增生(atypical hyperplasia,又称异型增生).具体从食量失宜、偏嗜"辛辣"、恣食生冷及热烫、嗜食肥甘油腻及酒炙之物等多个方面讨论饮食失宜与胃疾的关系,正如《医学正传·胃脘痛》言:"初致病之由,多由纵恣口腹,喜好辛酸,恣饮热酒煎煿,复餐寒凉生冷,朝伤暮损,日积月深……故胃脘疼痛."腌制品属于肥甘厚腻食品,可阻碍脾胃运化,长期食用会导致脾胃受损.CAG患者脾胃素虚,若饮食不节而食用含亚硝酸盐的腌制品,易加重脾胃受损,脾失健运,胃失和降,中焦枢机不利,产生气滞、痰浊、湿热、血癖等各种病理产物,加速病情发展,最终发为"胃岩".世界卫生组织下属国际癌症研究中心(International Agency for Research on Cancer,IARC)发布报告,将食用加工肉类食物分为"对人类致癌"(第1组),代表性食物有腌制品等.腌制食物保存时间长,抗氧化物水平含量低,亚硝酸盐类物质(主要是亚硝酸盐、硝酸盐和胺类、酰胺类等含氮化合物)含量高,亚硝酸盐进入胃内在某些条件下可与食糜中的二级胺发生亚硝化反应,形成N-亚硝基化合物(N-nitroso compound,NOC).NOC在慢性萎缩性胃炎(Chronic atrophic gastritis,CAG)向胃癌发展的过程中发挥着重要作用.NOC致癌的相关报道可追溯至1956年,由Magee和Barnes首次发现并发表.其后国内外越来越多的流行病学调查、回顾性研究以及动物实验证明了亚硝酸盐及NOC与胃癌前病变乃至胃癌有密不可分的关联性.亚硝酸盐在人体内代谢产物NOC促使萎缩性胃炎病情发展并向胃癌转化的机制应从分子生物水平探讨,学界主流看法认为NOC经过羟化酶作用转化为重氮烷,其与DNA形成DNA加合物导致突变.结合当前一些文献分析,DNA受到损伤后通常会进行自我修复,当DNA持续受NOC损害,而修复不足以应对NOC的破坏作用时,组织将遵循Correa级联反应由慢性萎缩性胃炎向肠上皮化生、异型增生、胃癌转变.因此,从饮食失宜角度探讨CAG发生及发展对临床上CAG患者的预防及治疗有深远意义.
