目的评价远程家庭监护对植入型永久起搏器及心律转复除颤器(ICD)患者药物治疗方面的影响。方法 2009年7月-2012年4月于我院安置永久型起搏器者及ICD者83例,通过百多力远程家庭监护系统接受患者的日常信息和触发警报的报警事件,以发现起搏器功能障碍、患者状态的变化和房性、室性的心律失常、心力衰竭的进展。起搏器触发事件包括系统警报和房性及室性心律失常的诊断信息,ICD触发事件包括心律失常的发生事件和治疗事件。对于CRT-ICDs的触发事件包括CRTT起搏百分比和心力衰竭监护。临床医生通过远程家庭监护系统得到患者信息并与其进行电话联系,指导患者临床药物的使用。结果患者平均观察时间326.02±258.79d(范围7-1039d),接受传输信息3795条,包括日常信息2903条,报警事件信息892条。有71名患者208次信息传输暂时中断(≥持续7d)通过家庭监护诊断为房颤的19人(22.89%),非持续性室速或室颤14人(16.87%),与3 1名患者进行沟通(电话95例次,药物治疗更改36例次,入院12例次,程序重置24例次)。在19名患者中发现有房颤事件发生,其中13名患者在植入前未发现有房颤。通过电话干预60例次,药物治疗更改23例次,住院治疗4例次。ICD患者中3例患者中更改了药物治疗方案,1例患者入院治疗。2名患者监护过程中发现监护系统由于T波过感知而将窦性心律误判为室颤,将患者召回医院经参数重置后ICD工作回复正常。心力衰竭患者中有2例患者室性早搏>110次/h,CRT比例降低<95%,经入院治疗后控制室性早搏数后CRT比例恢复>95%。结论家庭监护系统是适于起搏器和ICD患者的有效、安全、可靠的监护系统,它能在早期发现患者房性及室性心律失常的出现、设备的故障和心力衰竭的恶化,经过临床医生的处置使患者得到适当的治疗方案。

目的探讨心脏再同步治疗(CRT)患者术后QRS时限和各临床因素的相关性。方法收集CRT患者共计20名,记录术后QRS时限及各项临床指标。利用SIMCA-P软件,运用偏最小二乘回归方法(partial least-squares regression,PLS)分析不同临床指标和QRS时限的相关性。结果 PLS显示中性粒细胞比值、左心室舒张末期内径、手术时间、谷草转氨酶、出院前右心房导线测试P波值、出院前右心房导线测试阻抗、既往心血管病史、心房脑钠肽、出院前右心房导线测试电压值、谷丙转氨酶、血钠、最长RR间期、术前QRS时限、左心室导线靠近心尖部与术后QRS时限正相关,左心室射血分数、年龄、血小板计数、右心室导线测试阻抗、体重指数、血钾、总心搏数、最慢心室率、出院前右心室导线测试R波值、出院前右心室导线测试阻抗值、左心室导线起搏电压、左心室导线测试阻抗与QRS时限负相关。对危险因素重要性的VIP分析,与CRT术后QRS时限关系显著的临床因素依次是中性粒细胞比值、左心室舒张末期内径、左心室射血分数、手术时间、谷草转氨酶、年龄、出院前右心房导线测试P波值、出院前右性房导线测试阻抗、既往心血管病史、血小板计数、心房脑钠肽、出院前右心房导线测试电压值、谷丙转氨酶、右心室导线测试阻抗、体重指数、血钾、总心搏数、血钠、最慢心室率、最长RR间期等。建立PLS回归方程。预测建模分析显示,模型的拟合效果是令人满意的。结论影响CRT的主要因素是心脏疾病和全身状态的指标如中性粒细胞比例以及左心室舒张内径及左心室射血分数,手术时间越长提示导线到达合适位置的时间延长。导线的位置同样影响CRT的效果。

Objective Observe the relationship between the mean amplitude of f（fibrillatory ）-wave in V lead and the AF（atrial fibrillation） recurrence rate in the AF patients after catheter ablation. Methods 102 AF patients with the same baseline were chosen in this research,prior to ablation, they were divided into 2 groups,the f-wave amplitude≥0.05mV（56 patients） and the f-wave amplitude Results One year later 15.18%of patients with mean f-wave amplitude≥0.05(P≥0.05（P<0.05）. Conclusion Fine measurement of the f-wave amplitude in lead V1 could predict the recurrence rate of AF patients after catheter ablation.

目的预测低危药物或弱致心律失常因子的致心律失常作用是重要而且困难的,目前世界上共有8种基础或临床模型来预测与复极异常相关的多形性室性心动过速(室速),包括尖端扭转性室速,但结果均不令人满意。莫西沙星、胺碘酮、决奈达隆和ziprasidone可抑制复极钾电流,在用药患者中的千分之五到百万分之一可发生尖端扭转性室速(TdP)和猝死。本研究以尖端扭转性室速作为致心律失常的金标准,证明雌性家兔离体心脏经低剂量延迟钠电流增强剂处理后的心脏可能是目前所知的最敏感的预测恶性心律失常的动物模型。方法采用雌性新西兰大耳白兔Langendorf灌流心脏,房室结热消融造成三度房室传导阻滞后,在心室起搏1Hz的情况下,1到3纳摩尔海葵毒素(sea anemone toxin,ATX-Ⅱ)处理心脏20min,而后评估低危致心律失常药物,如莫西沙星、胺碘酮、决奈达隆和齐拉西酮的致尖端扭转性室速作用。结果在正常离体家兔心脏,治疗浓度下的莫西沙星(10μmol)、胺碘酮(0.3μmol)、决奈达隆(0.3μmol)和ziprasidone(10μmol),仅轻度延长心室肌动作电位时程,而不会引起尖端扭转性室速(TdP)。ATX-Ⅱ(1-3 nM)延长心室肌单相动作电位时程,而不诱发TdP。在ATX-Ⅱ(1-3 nM)处理过的心脏,同样浓度的莫西沙星、胺碘酮、决奈达隆和齐拉西酮显著延长心室肌单相动作电位时程并分别在80%,100%,100%和78%的心脏诱发TdP,洗脱这些药物后心律失常消失。结论结合文献资料复习,作者认为经延迟钠电流增强剂ATX-Ⅱ处理过的离体雌性家兔心脏是目前已知的最敏感的预测恶性室性心律失常的动物模型,可用于探测低危药物和其他可能的引起心脏复极储备降低的因子,如器质性心脏病患者引起的恶性心律失常。

Objective To investigat the effect of artemisinin on post-infarct myocardial fibrosis. Method Adult male Sprague Dawley rats were divided into a sham group and MI groups that were treated either with oral gavage of artemisinin(75 mg/kg/day) or vehicle(0.5%carboxymethyl cellulose) three times a day for 4 *** treatment was started at 24 hours after ligation of the left anterior descending coronary artery. Results Four weeks after MI,the survival rate was significantly improved in the artemisinin-treated group compared with that of the vehicle-treated group(65%vs.40%,P<0.05). Although infarct size was similar in both groups,cardiac function and left ventricular dimensions was significant improments in in the artemisinin-treated ***,the degree of myocardial fibrosis and elevated levels of fibrosis-related factors(transforming growth factor- β1,collagen typeⅠ) in the non-infarcted myocardium were remarkably ameliorated by artemisinin(all P<0.05). Conclusions artemisinin improve cardiac function and attenuate post-infarct myocardial fibrosis by down-regulating TGF- β1.

Objective Little is known about the independent role of the intrinsic cardiac nerve activity in the initiation of atrial fibrillation(AF).This exvivo study expels the innervation of extrinsic cardiac nerve system and investigated the independent role of the intrinsic cardiac nerve activity in the induction of AF. Methods In 10 langendorff-perfused dogs,the atrial effective refractory period,the pulmonary vein effective refractory period,and the percentage of AF induced were measured at baseline and during the right pulmonary artery ganglionated plexi stimulation. Results Right pulmonary artery ganglionated plexi stimulation shortened the atrial effective refractory period from 129±11 ms at baseline to 115±17 ms(P<0.05),shortened the pulmonary vein effective refractory period from 136±12 ms to 122±14 ms(P<0.05),and significantly increased the AF episodes from10%at baseline to 90%(P<0.05). Conclusions The intrinsic cardiac nerve activity provokes AF independently without the innervation or the cooperation of the extrinsic cardiac nerve *** local neuronal loop consisting of ganglionated plexi may be the most important trigger in the rise of AF.

Objective To investigate the clinical significance of AT termination in patients with persistent AF undergoing ablation. Methods A total of 200 consecutive patients with persistent AF were enrolled in this study,and underwent radiofrequency catheter ablation for the first time in individualized ablation strategy navigated by Carto(Biosense Webster,Diamond Bar,CA,USA).If AF organized into AT,we tried to define the mechanism of every AT,then optimal ablation was performed to terminate these *** we failed to define or terminate the ATs,cardioversion was used to restore sinus rhythm(SR).A 3-month blanking period was resumed. Results AF organized into AT during procedure in 93 patients(mean age 57.1±11.3 years,73 male),AT was terminated to SR successfully by further ablation in 62 patients and by cardioversion in 31 *** a mean follow-up of 38.5±15.9 months,SR was maintained in 51 patients(54.8%).There was a lower recurrent incidence(32.3%vs.71.0%,P = 0.001) in patients who had AT termination by ablation alone compared with those who did not. Conclusions AT termination by ablation alone is associated with an improved clinical outcome in patients with persistent AF.

Objective Test the association between the variation rs 12143842 in NOS1AP and idiopathic ventric ular tachycardia(TVT). Methods A case control association study of NOS1AP were performed with 277 idiopathic ventric ular tachyccardia patients and 726 controls from a Chinese GenelD *** was performe d using High-Resolution Melt analysis. Results significant association was obtained between rs12143842 and idiopathic ventricular tachyc cardia in a Chinese Han population(allelic P = 0.003 with OR = 0.72;genotypic P =0.029 with a dominan tmodel).When the idiopathic ventricular tachyccardia cases were divided into idiopathic left ventricular ta chyccardia(42.6%) and idiopathic right ventricular tachyccardia(50.5%),significantly strong association was found with idiopathic right ventricular tachyccardia(OR = 0.70,P = 0.01) and idiopathic left ventricu lar tachyccardia(OR = 0.73,P = 0.04). Conclusions the minor allele T of SNP rs 12143842 confers a significant protective risk of IVT in a Chinese Han *** results establish the association between a common variant in NOS1AP and I *** functional study of the variation needs to be demonstrated.
