Objective Uric acid(UA) is a cardiovascular risk marker associated with oxidative stress and ***,atrial fibrillation(AF) has been associated with inflammation and oxidative stress. The objective of this observational study was to investigate the association between UA levels and paroxysmal AF. Methods Consecutive patients with paroxysmal AF who were hospitalized in the Second Hospital of Tianjin Medical University from September 2011 to May 2012,were screened in this study.,We excluded subjects with acute coronary syndrome,congestive heart failure,valvular heart disease, congenital heart disease,cardiomyopathy,acute inflammatory conditions,thyroid *** final study population consisted of 106 patients,65 patients with paroxysmal atrial fibrillation were finally enrolled,and 41 age,sex matched-subjects without AF were served as the control *** baseline clinical data and laboratory examinations results were detected,left atrium diameter(LAD),left ventricular end-diastolic dimension(LVEDD) and left ventricular ejection fraction(LVEF) were determined by 2D-echocardiography. Results In univariate analysis,serum UA,LAD,LVEF,urea nitrogen and estimated glomerular filtration(eGFR) level were significantly increased in patients with paroxysmal AF compared with *** multivariate logistic regression analysis,the serum uric acid level(OR:5.935,P<0.05) and LAD(OR:1.144,P<0.05) were independent risk factors for the occurrence of paroxysmal AF. Conclusion We demonstrated that hyperuricacidemia is an independent risk factor for the development of paroxysmal *** larger studies are needed to evaluate this potential association as well as the underlying mechanisms.

目的观察窦性心率震荡(HRT)在急性脑梗死患者中的变化,探讨HRT对急性脑梗死患者的病情评估和预后的临床价值。方法选择急性脑梗死患者100例及同期年龄、性别相匹配的有室性早搏而无器质性心脏病健康者100例分别作为脑梗死组和对照组。所有入选者采用美国Mortara Instrument公司生产12导联动态心电图记录仪监测,进行心率震荡分析及心率变异性时域分析。结果①梗死组较对照组HRT指标明显减弱或消失,TO值明显增大(P<0.001),TS值明显减小(P<0.001)。与健康对照组相比较,脑梗死组的HRV各时域指标SDNN、SDANN、RMSSD均降低,且差异有统计学意义(SDNN、SDANN:<0.05,RMSSD:P<0.01)。②高危组中的TO值大于低危组(P<0.01),高危组中的TS明显低于低危组(P<0.001),而HRV各时域指标SDNN、SDANN、RMSSD高危组较低危组有着更明显的减低,并有统计学意义(SDNN、RMSSD:<0.05:SDANN:P<0.01)。高危组TO≥0者占79.87%,明显高于低危组的11.34%(P<0.05);而高危组中TS≤2.5 ms/RRI占54.24%,低危组20.56%,高危组的阳性率明显高于低危组(P<0.05)。③右侧岛叶组HRT和HRV的各项指标与对照组做比较均有显著意义(P<0.01),左侧岛叶组TO明显增大、SDNN减小与对照组相比较有统计学意义(P<0.05),延髓组的HRT和HRV的各项指标与对照组比较均有显著差异(TO、TS、SDANN:P<0.01;SDNN、RMSSD:P<0.05),脑桥组中TS值与RMSSD与对照组比较,差异有统计学意义(P<0.05);右侧岛叶组与左侧岛叶组比较TO、TS、SDANN和RMSSD指标均有统计学意义(P<0.05)。④脑梗死组中TO、TS两个指标之间无明显相关性,TO与SDNN、SDANN、RMSSD存在负相关性(TO与SDNN:=-0.258,P<0.05;TO与SDANN:=-0.523,P<0.01;TO与RMSSD:=-0.235,P<0.05);TS与SDNN、SDANN、RMSSD存在正相关性(TS与SDNN:=0.328,<0.05;TS与SDANN:=0.458,;TS与RMSSD:=0.593,P<0.01)。结论 HRT可反映和评估急性脑梗死患者心脏自主神经功能受损的程度,TO、TS可作为急性脑梗死病情的危险分层和预后评估指标;TO、TS对脑梗死引起的心脏自主神经功能受损导致的恶性室性心律失常有预测价值;不同的梗死部位对心血管自主神经功能的影响不同,尤以右侧岛叶和脑干中的延髓区域梗死的心脏自主神经功能显著异常;TO、TS可作为互相独立的预测指标,且联合应用TO、TS可能会有更大的预测价值。TO、TS与心率变异性时域分析指标SDNN、SDANN、RMSSD均有相关性,均可反映心脏自主神经功能状态。

Methods and results Relative young(3 weeks) transgenetic cardiac-specific PPAR-γ(MHC-PPAR-γ) overexpression mice with low(PPAR-γL) and high(PPAR-γH) levels,and control mice were *** weight(HW),body weight(BW),HW/BW ratio,and serum brain natriuretic peptide (BNP) were ***43 expressions were measured by RT-PCR and western blot and rniRs abundance were assayed using real-time quantitative RT-PCR(qRT-PCR).Cx43 contents,both in mRNA and protem levels,in heart with PPAR-γH were significantly lower than those with PPAR-γL and control ***,Cx43 mRNA levels tended to be negatively correlated to MHC-PPAR-γ(r = -0.38;PmiR-1 level was higher in PPAR-γH ***-206 also tended to be increased in PPAR-γH mice but did not reach statistical ***-1 level was positively correlated to MHC-PPAR-γ(r = 0.38;P = 0.02) in PPAR-γH ***-1 level was negatively correlated to Cx43 mRNA abundance(r = -0.36;P<0.01). Conclusions Cardiac-specific overexpression of PPAR-γleads to a reduction of Cx43 in the heart, which may be mediated by the increased miR-1 contents.

Objective Chronic atrial fibrillation（AF） leads to heterogeneous autonomic nerve innervation termed meural *** quantitative changes in neuronal density as a function of autonomic remodeling and it's association with sustained AF has not been previously determined. Method and Results Seven dogs（paced group） were chronically paced with electrodes sutured to the epicardium of left atria appendages（LAA）.Seven dogs（control animals） were not *** paced dogs developed sustained AF after 5 weeks *** fat pads on the atrial epicardium containing ganglionated plexi（GP） were separated along with underlying myocardial *** techniques were used to identify the neurons immunoreactive to anti-tyrosine hydroxylase（TH） and anti-acetylcholine（ACh） *** chronic AF,sympathetic and parasympathetic neurons in the atria intrinsic cardiac ganglia increased *** paced dogs,the density of sympathetic neurons was 3021.59±507.13μm/mm inthe right atrial GP（vs control P<0.01）,8570.72±476.05μm/mm inthe ventral left atrial GP（vs control P<0.0001）,6422.09±463.91μm/mm in the dorsal atrial GP（vs control P<0.0001） and 5391.70±595.45μm/*** inferior vena cava-inferior atrial GP（vs control P<0.0001） *** density of parasympathetic neurons was 4395.51±877.30μm/mm inthe right atrial GP,7769.47±464.93μm/mm inthe ventral left atrial GP,7016.47±888.08μm/mm in the dorsal atrial GP and 5485.03±553.60μm/mm inthe inferior vena cava-inferior atrial GP respectively,which was more than control cohorts in corresponding GP（P<0.05）. Conclusions This study provides evidience for the remodeling of intrinsic cardiac nervous system in AF *** increase of sympathetic and parasympathetic neurons were present in atrial intrinsic cardiac ganglia.

Backgroud Short QT syndrome（SQTs） is a genetically determined disorder,characterized by short QT interval,increased susceptibility to malignant tachyarrhythmias and sudden cardiac *** date,the pharmacotherapy of SQTs is mainly related to the SQT1 caused by N588K mutation in KCNH2 gene. Objective The purpose of this study was to evaluate the efficacy and remediation role of various antiarrhythmic drugs to SQTs caused by E428G mutation in SCN5A gene in Chinese patients which was discovered by our research group recently. Methods Whole-cell patch clamp measurements of hNavl.5 current(I) mediated by E428G mutated channels and the antiarrhythmic drugs - mexiletine（50 uM）,amiodarone（50μM） and propafenone（30μM） were performed at room temperature in human embryo kidney（HEK） 293 cells. Results In the presence of amiodarone,mexiletine and propafenone in vitro,only propafenone could significantly increase the peak I of E428G *** can be observed that the amiodarone, mexiletine and propafenone induced a hyperpolarization shift of theⅠ-Ⅴrelationship curve,voltage dependence of steady-state activation（SSA） and steady-state inactivation（SSI）.The increment of I caused by hyperpolarization shift of SSA was partly balanced by the decrement of I caused by hyperpolarization shift of *** amiodarone and mexiletine had no effect on the peak 7Na of E428G channel,while propafenone increased about ten percent of E428Gchannel current. Conclusion Our study,for the first time,to shows that propafenone may be a quite potential drug to treat SQTs caused by E428G mutation in SCN5A gene.

Objective Probucol is a diphenolic compound with anti-oxidant and anti-inflammatory properties that reduces atherosclerosis and *** benefit of probucol on atrial fibrillation(AF) prevention remains *** sought to investigate the effects on atrial heat shock protein 70(HSP 70) expression and in AF promotion in alloxan-induced diabetic rabbits. Methods Twenty alloxan-induced diabetic rabbits were *** of these were treated with probucol at a dose of 1000 mg/d for 8 weeks and the remaining were left untreated while 10 additional healthy rabbits served as *** Langendorff perfused rabbit hearts were used to evaluate interatrial conduction time(IACT) and vulnerability to *** blotting was performed with left atrial tissues for heat shock protein(HSP70) *** also performed histology examination to measure atrial fibrosis and measured plasma oxidative stress biomarkers. Results Atrial HSP70 expression and AF inducibility were significantly increased in the diabetic rabbits than controls(8/10 s vs 1/10,P<0.05).Probucol significantly downregulates atrial HSP 70 expression,reduced AF vulnerablilty in the treated rabbits(3/10 vs 8/10,P<0.05).Moreover,Probucol attenuated atrial structural remodeling reducing the IACT and the atrial interstitial fibrosis as well. Probucol increased plasma superoxide dismutase(SOD) activity and on the other hand decreased malondialdehyde(MDA),Medullary oxide enzyme(MPO) and catalase(CAT) levels. Conclusions Probucol downregulates atrial HSP 70 expression and attenuates AF promotion in alloxan-induced diabetic *** seems to relate to its powerful antioxidant activity in this experimental model.

Objective To evaluate the ventricular lead performance and safety of right ventricular outflow tract septal pacing in patients with brady arrhythmia in South China during 24 months follow-up. Methods 192 patients(108 males,84 females,63±21 yrs) with bradyarrhythmia were randomly divided into two *** right ventricular outflow tract(RVOTs) had lead placement near the septum(n=97) while the right ventricular apex(RVA) group had a traditional apical placement(n=95). RV septal lead positioning was achieved with a specialized stylet and confirmed using fluoroscopic *** patients were followed for 24 months,including stimulation threshold,R-wave sensing, lead impedance and lead complications. Results The time of electrode implantation in ROVTs group and RVA group were significantly different(7.51±1.12 min vs.4.55±0.75 min,min P<0.01).No differences were identified in threshold, impedance or R-wave sensing between the two groups at 1 day,3 months,12 months and 24 months follow-up(P>0.05).No implant complications were reported,no electrode displacement,increased pacing threshold and inadequate sensing were found. Conclusions The traditional right ventricular apical pacing is not *** the long-term active fixation ventricular electrode performance in RVOTs pacing is similar to the ventricular lead in traditional RVA pacing;RVOTs pacing near the serum using active fixation electrodes may provide stability during long-term follow-up.

Objective The present paper studies the instruction of pacemaker Home Monitoring(HM) function in the clinical. Methods The study analyzed 14 cases,implanting pacemakers with HM function.7 patients implanted common pacemakers,4 ICD,2 CRTP,and 1 *** the real time monitor of arrhythmia and the changes of statistical data,the medicine used was decided and the curative effect after the adjustment of drug dosage anticipated. Results①Four patients did not follow the doctors' instructions,having never turned on the Cardio *** had a sudden cardiac death,providing no record.②Mode switched frequently in one *** fibrillation was observed when the patient was program *** switch deceased significantly when treated by amiodarone.③Two cases implanted CRTP and CRTD *** prompted ventricular rates above 80 *** the resting *** treated with increased dosage of diuretic and βblockers,their cardiac function became better,not warning any more.④1 case implanted *** warned that ventricular pacing percentage was more than 30%in 24 *** adjusting the dosage of βblockers,ventricular arrhythmias and ventricular pacing reduced.⑤One case implanted pace maker for mixed types of inhibition vasovagal *** prompted that the mode switched above 20 times within 24 *** atrial arrhythmias was not found. Conclusions Some patients do not have clinical *** can lower their frequencies to see the doctors,and save time and *** can recognize the problems timely and judge the drug *** is significantly important to guide the clinical medicining and adjust the dosage,especially for those suffering from complications and heart *** patients implanting ICD,adjusting the drug dosage to treat ventricular arrhythmias,can avoid discharge and prolong their services *** amounts of prospective researches are needed to testify its validity and reliability.

Objective Recent Large-Scale Candidate Gene Analysis identified a variant,rs4845625 in interleukin-6 receptor(IL-6R) gene associated with atrial fibrillation(AF).Levels of circulating interleukin-6,along with levels of other pro-inflammatory molecules such as C-reactive protein and fibrinogen,have consistently been associated with risk of AF and recurrence after catheter *** study tested the hypothesis that the variant rs4845625 associate with AF recurrence after catheter ablation. Methods A total of 278 consecutive patients(mean age 59.4±11.5 years,43%female) with paroxysmal(36.0%),persistent(59.7%),permanent(4.3%) AF who underwent AF catheter ablation from 2007-2011 were included in this *** was genotyped using High Resolution Melt analysis(HRM). Electrocardiographic recordings were acquired to detect AF recurrences. Results In our study cohort,early recurrence of atrial fibrillation(ERAF) which recurrence within the first 4 weeks was observed in 42.8%,whereas late recurrence of atrial fibrillation(LRAF)(between 3 and 12 months) occurred in 25.9%of the *** sigmficant difference of clinical or echocardiographic baseline characteristics was observed between patients with ERAF and *** contrast,the T allele of rs4845625 increased the risk for both ERAF(odds ratio[OR]:1.84,95% confidence interval[CI]:1.31-2.59,P =4.10×10-4) and LRAF(OR:1.92,95%CI:1.30-2.81,P =0.001). The association was also significant after adjusted by age,sex,hypertension,diabetes and other risk factors. Conclusions In our study,we first found that variant in IL6R gene confers risk to AF recurrence after catheter *** rinding provides evidence for a potential causal role of the interleukin-6 pathway or the inflammation in AF recurrence after catheter ablation.

Objective Whether renal sympathetic denervation has effect on the function of ganglionated plexi and inducilibity of AF is unknown. Methods Thirteen dogs were randomly divided into control group(7 dogs) and renal artery ablation(RAA) group(6 dogs).In control group,after right thoracotomy,atrial effective refractory period (AERP) was measured at three sites in right *** was induced by anterior right ganglionated plexi (ARGP) *** of heart rate and AF were *** RAA group,each renal artery was ablated via femoral artery and dogs were allowed to recover for 6 *** that,the procedure of stimulation was nearly same as in the control *** of AngⅡand aldosterone in blood were studied. Results There was no significant difference in AERP and dAERP in right atrium between the two *** voltage in the sinus rate slowing response and the lowest voltage that induce AF by different levels of ARGP stimulation were higher in ablation group than that in control group(3.4±1.6V vs. 1.6±0.3V,P=0.03;5.1±1.0V vs.2.1±0.7V,P<0.01).Compared with before artery ablation,the plasma AngⅡand aldosterone concentration decreased significantly after 6 weeks in RAA group(AngⅡ: 181.2±23.4 pg/ml vs.102.1±31.4 pg/ml,P<0.01;Aldosterone:272.2±37.4 pg/ml vs.156.3±17.6 pg/ml, P<0.01). Conclusions Renal sympathetic denervation decreased the inducilibity of AF by ARGP stimulation. Inhibition activity of renin-angiotensin-aldosterone system may be one of the mechanisms underlying these results.
