Objective:The aim of the present study was to investigate the influence of off-pump coronary artery bypass grafting(OPCAB)on early and long-term mortality and morbidity in a consecutive series of elderly patients(over 65 years old)compared with conventional coronary artery bypass grafting(CCABG). Methods:From January 1999 to December 2003,data were extracted for 1191 patients over 65 years old: CCABG group(n=744);OPCAB group(n=447).Age-related early outcomes of interest were in-hospital mortality and postoperative ***-term outcomes including total mortality,repeated revascularization,stroke,readmission and the combination of death,Q-wave myocardial infarction,stroke,and repeated revascularization were evaluated with Cox regression analysis. Results:Univariate analysis for early outcomes showed significant benefits from *** adjusted for baseline characteristics,there was still a benefit of ***-Meier event-free survival analysis for long-term outcomes showed that there was no significant difference in total mortality(p=0.654).However,stroke,MACCEs and readmission occurred more frequently in OPCAB group(p<0.001).Cox regression for long-term outcomes showed that total mortality and repeated revascularization were similar in the two groups and that patients of OPCAB group had a higher incidence of stroke[hazard ratio(HR)2.611,95%CI 2.152 to 3.070],readmission(HR 2.000,95%CI 1.747 to 2.253)and MACCEs(HR 1.764,95%CI 1.456 to 2.072).
