目的检测临床分离新生隐球菌对抗真菌药物的敏感性,分析抗真菌感染治疗效果。方法采用法国生物梅里埃公司Bact/ALERT 3D 120培养仪、VITEK-32微生物分析系统、真菌鉴定YBC卡进行隐球菌种的鉴定,ATB FUNGUS 3微量肉汤稀释法检测新生隐球菌对抗真菌药物的敏感性,结合临床抗真菌感染治疗。结果2004年10月~2007年8月收治艾滋病合并机会性感染的541例患者中,有17例合并隐球菌性脑膜炎。培养鉴定检出新生隐球菌15株(2例未做微生物培养)。真菌药物敏感试验5-氟胞嘧啶、两性霉素B、依曲康唑、伏立康唑均敏感。有2株新生隐球菌对氟康唑中度敏感(13.3%,2/15),未检出耐药菌株。临床抗隐球菌感染的治疗为两性霉素B+氟康唑联合用药,3例死亡,14例好转。结论艾滋病合并隐球菌脑膜炎需要长期抗真菌治疗。及时进行真菌培养及真菌药敏试验,可提供有效的病原学诊断信息和抗真菌药物敏感试验结果,帮助临床诊断和治疗。
