据WHO统计,在育龄夫妇中,大约有15%受不育症的困扰,且男女因素各占一半。不育症的诊治已成为当代医学的重要内容,相关研究是生物医学的热点、难点。该项目历时15年,在国家和省级课题的支持下,围绕着男性不育的基础与临床问题,对精子能量代谢的关键酶-精子特异性乳酸脱氢酶(即乳酸脱氢酶C4,LDHC4)进行了酶学、免疫学和分子遗传学等系列研究,取得了以下创新成果:
1、针对国内外尚未解析人LDHC4晶体结构的现状,利用通用的生物信息学分析软件,对人LDHC4的结构和功能进行了计算生物学分析,预测了人LDHC4的立体结构,为理解人LDHC4的酶学特征、抗原性以及基因变异性提供结构生物学依据。
2、利用LDHC4可特异催化含4~6个碳原子酮酸或羟基酸的特点,在优化以2-羟基戊酸为底物、检测精子LDHC4活性的基础上,开发了一种以廉价、易得的2-羟基丁酸钠为底物的活性显色方法,解决了精子LDHC4活性显色的难题,建立了该酶活性积分的正常参考范围。以此为基础,在精子LDHC4活性降低的不明原因男性不育患者人群中发现:患者精子LDHC4活性水平与其精子LDHC4蛋白质表达水平呈正相关。
3、在一组精子LDHC4活性降低的不明原因男性不育患者人群中,发现了第一个LDHC基因突变(即L178X突变),证明:LDHC基因突变可能是部分男性不育症患者发病的原因。
4、以重组人LDHC4蛋白(rhLDH-C4)为包被抗原,建立了以单一精子抗原为靶抗原、检测抗精子抗体的ELISA方法,证明:LDHC4是不育症患者体内抗精子抗体所针对的主要抗原之一。在此基础上,提出“避孕基因疫苗”的概念,构建了两个能表达全长人和小鼠LDHC4的避孕基因疫苗(pVAX1-hLDHC 和pVAX1-mLDHC);在动物实验中成功诱导了针对精子LDHC4的免疫反应,使实验动物的产仔数明显下降;形成了避孕基因疫苗制备和进行基因免疫的实用技术方案。
该项目已发表论文13篇,其中3篇被SCI收录,3篇被PubMed收录,总共被50篇次论文引用。发表综述文章2篇。培养硕士、博士研究生(博士后)11名。项目组成员受邀在各类会议(学习班、培训班)作专题报告(讲座、讲课)5次,包括海峡两岸第九次检验医学学术会议暨福建省第十四次检验医学学术会议等。以大会发言和论文报告形式参加省级以上会议交流达14次,包括第20届国际遗传学大会((XX International Congress of Genetics,2008,Berlin,Germany)分会发言、中国遗传学会第八次代表大会暨学术研讨会、中华医学会第十七次全国医学遗传学学术年会等。该项目的开展,直接推动该院完成福建省第一个人类精子库的筹建,有力推动了该省男科遗传学技术水平的提高,取得了显著社会效益。
