Background The aim of this study was to investigate the early response of repetitive transcranial magnetic stimulation(rTMS) combined with paroxetine and its effect on executive function in Chinese subjects with first-episode depression. Methods A total of 48 patients(aged 18-45) meeting DSM-Ⅳcriteria for first-episode depression randomly received rTMS(n= 25) or sham rTMS(n= 23) to the left dorsolateral prefrontal cortex for 4 *** patients concomitantly took paroxetine(20-40 mg/day).The rTMS group received 20 sessions(5/week) at 10-Hz(80%motor threshold and 800 pulses per session).Sham stimulation followed the same *** efficacy variables were the Hamilton Depression Rating Scale(HAMD;24 item) and Wisconsin Card Sorting Test(WCST). Results The HAMD scores reduced significantly from the 1st to the 4th week after treatment in both ***,the rTMS group showed a faster reduction than the sham group(P < 0.05). The response and remission rates were significantly greater in the rTMS group after the stimulation period(95.5%and 68.2%vs.71.4%and 38.1%,respectively;P = 0.041 and P = 0.009,respectively),but not at the *** function was improved to the same extent in both groups. Limitations:The number of subjects was relatively small. Conclusions rTMS at 10- Hz accelerated the onset of action and augmented the response to paroxetine in patients with first-episode depression with no effect on executive function.

Objectives Korean Medication Algorithm Project for Depressive Disorder (KMAP-DD) was developed in 2002 and revised in *** the last revision six years ago, this revision reflected the new research result and the latest trends in the areas of pharmacological treatment. Methods 123 psychiatrists who have vast clinical experiences in depressive disorder are primary selected then survey was sent to them via mail,67 surveys were *** survey is constructed with 44 questionnaires in which contained from overall treatment strategies to treatment strategies under the specific *** treatment strategy or treatment option is evaluated with the overall score of nine and the following 95%confidence interval result treatment option were divided into three phases of recommendation;primary,secondary,tertiary. Results ECT was recommended as an initial strategy for major depressive disorder,severe without psychotic features with urgent suicidal risk,and as a second strategy for non-responders on antidepressant combination therapy and combined with physical *** the patient of major depressive disorder,severe with psychotic features,ECT was preferred as an initial strategy for urgent suicidal risk patients,but as a second strategy for non-responders on antipsychotics and antidepressants combination therapy and comorbid with physical *** was not recommended as a initial treatment strategy for major depressive disorder,but could be a second strategy for non-responder on antidepressants combination therapy in severe episodes without psychotic features and non-responders on pharmacotherapy in moderate episodes. Conclusions ECT was an initial strategy in severe episode with/without psychotic features who has an urgent suicidal risks,and secondarily preferred in case of non-responders,comorbid with physical illness and pregnants.

Object Recently,it has been elucidated that the alterations of neural circuit including synapse/neuro-genesis are the key mechanism of various clinical conditions of psychiatric diseases such as schizophrenia,depression,and alcohol related disorders,and the neurogenic and neurotrophic effects of psychotropics are considered to contribute to the prevention of morphological and functional brain change induced by those psychiatric diseases. In relation to these evidences,protective role of brain-derived neurotrophic factor(BDNF) for the brain volume deficits in these diseases have been also ***,we have investigated the effect of psychotropic agent such as antidepressant and BDNF on the neurogenetic change induced by alcohol and endoplasmic reticulum(ER) *** also have shown that the alterations of trophic factor signaling and transcription factors,CREB and NRSF/REST play an important role in the effects of these psychotropic *** addition,there are some reports demonstrated that serum BDNF levels in patients of these disorders are lower than control subjects,and antidepressant and antipsychotic treatment increases its serum levels in ***,its mechanism and biological meaning in the pathophysiology of schizophrenia,depression,and alcohol related problems have not yet understood. Methods In this study,we investigated that antidepressants increased the platelet BDNF release through TrkB signaling and other cellular signaling. Results We also demonstrated that serum BDNF was also increased by the intravenous treatment of ***,ethanol suppressed the antidepressant-induced platelet BDNF release and the antidepressant-induced release decreased remarkably in corticotrophin-induced depression model rat. Objective We explored the microstructural change of whole-brain white matter by diffusion tensor imaging in first-episode adults with major depressive disorder,using novel tract-based spatial statistics. Methods Forty-one depressive patients and forty-one controls underwent diffusion tensor *** automated tract-based spatial method was used to analyze fractional anisotropy. Clinical correlations of the fractional anisotropy in white matter tracts showing group difference were examined using Spearman's correlation analysis. Results The patients exhibited significantly greater mean fractional anisotropy than healthy subjects in the white matter of the left superior longitudinal fasciculus(MNI x,y,z coordinates, -27,-22,21;t = 4.88;p < 0.001).There were no significant linear correlations between FA values and depression severity,duration of illness and age of onset(p>0.05). Conclusions The microstructure of the whole-brain white matter in patients with first-episode depression was altered,and there may have been compensatory regeneration of nerve fibers in depression early in the course of the disorder.

Methods Psychometric properties were evaluated in a small sample of 26 health control of Chinese people who speak *** we compared the data with the data in another study in English version of *** the Chinese sample,ten men and sixteen women,ages between 18 to 55 years age were recruited in four *** each group,four transformed scores were computed on the SCIP total score,percentiles,z-score,T-score. Results No statistically significant differences between the two versions of *** time for SCIP administration was 13.2 minutes. Conclusion It is comparable between the two versions of *** studies should aim to use the Chinese version in clinic population with a larger sample of health control.

Background Despite clinical evidence of advantages,long-acting injectable antipsychotics(LAIA) are not widely used due to the negative attitude of patients and caregivers toward the *** survey was undertaken to investigate the difference of attitude between patients and their caregivers with first-episode and multiple-episode psychosis toward LAIA and the reasons for reluctance toward LAIA-treatment. Method The survey included the questionnaire about participants' demographic characteristics, prior knowledge about LAIA,attitude toward medication or psychotherapy and preference for LAIA- *** were the questions for reasons of reluctance toward LAIA-treatment, Drug Attitude Inventory(DAI),Questionnaire to assess Attitudes towards Psychotherapeutic Treatment(QAPT),Medication Adherence Rating Scale(MARS),and Cognitive Failure Questionnaire(CFQ). The preference for LAIA was re-evaluated after providing short-information about current LAIA or to the hypothetical questions like if there are a 3-months lasting LAIA or the cost of LAIA is cheap. Result Questionnaires were completed by 276 Participants(51 first-episode patients(FEP),40 caregivers of first-episode patients,73 caregivers of multiple-episode patients(MEP),112 multiple-episode patients).The baseline preferences of LAIA were increased from 33.3%to 39.2% for the patients with FEP,from 55.0%to 70.0%for the caregivers of FEP and from 61.6%to 69.0%for the caregivers of MEP after reading the short-information about ***,the preference of MEP remained the same(30.6%).Interestingly,the preferences were significantly increased to the questions such as "Would you prefer LAIA if it is cheaper than that of the current oral medication or its effect lasts 3 months".The reasons for reluctance toward LAIA-treatment most frequently chosen were psychological resistance(62.8%and 46.4%for the patients with FEP and MEP),worry about adverse effects(35.0%for the caregivers of FEP) and satisfaction for c

Objectives Mammalian target of rapamycin(mTOR) is a protein kinase involved in the regulation of translation initiation and protein synthesis required for synaptic plasticity. Recent studies suggest that mTOR activation is required for antidepressant ***,the mTOR signaling underlying antidepressant drug action has not been *** aim of the present study was to determine whether alterations in mTOR signaling were observed following treatment with different classes of antidepressant drugs. Methods Using Western blotting we measured changes in the phosphorylation of mTOR,its well-known downstream regulators(4E-BP-1 and p70S6K),and its upstream regulators(Akt and ERK) under toxic conditions induced by B27 deprivation in rat hippocampal neuronal cultures. Dendritic outgrowth of hippocampal neurons was determined by neurite assay and the synaptic proteins,PSD-95 and SYP,were also examined by Western blotting. Results We findings demonstrated that escitalopram,paroxetine,and tranylcypromine significantly increased levels of phospho-mTOR,phospho-4E-BP-1,and phospho-p70S6K in a concentration-dependent manner(p <0.05 or p <0.01);fluoxetine,sertraline,and imipramine did not affect ***,all antidepressant drugs tested increased the phosphorylation of Akt and ERK(p <0.05 or p <0.01).Additionally,increased mTOR phosphorylation induced by escitalopram,paroxetine,or tranylcypromine was significantly blocked in the presence of specific PI3K,MEK,or mTOR inhibitors,respectively(all p<0.01). All antidepressant drugs tested promoted hippocampal dendritic outgrowth(all p <0.01) and simultaneously increased levels of the synaptic proteins,PSD-95 and SYP(all p <0.01).The effects of escitalopram,paroxetine,and tranylcypromine were significantly blocked by the mTOR inhibitor,rapamycin(all p <0.01),whereas fluoxetine,sertraline,and imipramine effects were not affected by rapamycin. Results Lithium,valproic acid,and carbamazepine,significantly prevented B27 deprivation-induced decrease in levels of PSD-95,BDNF,NLG1,P-neurexin and SYP(p< 0.05 or p< 0.01),whereas lamotrigine had no effect in this *** the drugs significantly increased the total outgrowth of hippocampal dendrites(p< 0.01).Moreover,this effect of lithium and valproic acid is blocked by specific inhibitors of calcium/ calmodulin kinaseⅡ(CaMKⅡ),KN-93 (1μM),protein kinase A(PKA),H-89(1μM),phosphatidylinositol 3-kinase(PI3K),LY294002 (10μM),or MAPK/ERK kinase(MEK),PD98059(1μM) signaling(p< 0.05 or p< 0.01), whereas other mood stabilizers do not affect these signaling. Conclusions Taken together,these results suggest that certain mood stabilizers may regulate synaptic plasticity by enhancing synaptic protein levels and dendritic outgrowth in hippocampal *** addition,these effects on dendritic outgrowth likely require CaMKⅡ,PKA,PI3K or MEK signaling pathways.

Objectives Recent studies have demonstrated that antidepressant effect of N-Methyl-D-aspartate(NMDA) antagonist ketamine activates rapidly the mammalian target of rapamycin(mTOR) pathway and increase synaptic proteins the prefrontal *** is a protein kinase involved in the regulation of translation initiation and protein synthesis required for synaptic *** studies suggest that mTOR activation may be related to the antidepressant ***,the mTOR signaling underlying antidepressant drugs action has not been well *** aim of the present study was to find out whether alterations in mTOR signaling could be observed following treatment with ***,we investigate whether this drug affect the synaptic proteins and neurite outgrowth via mTOR signaling. Methods For purposes of western blotting and neurite assay,primary rat hippocampal neuronal cultures were cultured for 4 days and 5 days,respectively,with tianeptine(50 and 100μM). Control cells were cultured without tianeptine under the B27-deprived condition(for western blotting) or normal condition(for dendrite outgrowth aasay).Using Western blotting,we measured changes in the phosphorylation of mTOR,its well-known downstream regulators [eukaryotic initiation factor 4E(elF4E)-binding protein-1(4E-BP-1) and p70S6 kinase (p70S6K)],and its upstream regulators[Akt and extracellular signal-regulated kinase(ERK)] under toxic conditions induced by B27 deprivation in rat hippocampal neuronal cultures. Dendritic outgrowth of hippocampal neurons was determined by dendrite outgrowth aasay. Dendrites were visualized by immunostaining with microtubule-associated protein 2(MAP2) known as a dendritic ***,the synaptic proteins,postsynaptic density protein-95 (PSD-95) and synaptophyin(SYP),were also examined by Western blotting.
