环境内分泌干扰物(EEDs)是指环境中一类能够干扰生物体内源性激素的合成、释放、转运、结合或清除,从而影响机体的内环境、生殖的外源性物质.随着塑料餐具、不粘锅、化妆品、洗发剂、洗涤剂、家具中的各种黏合剂、阻燃剂、电子产品等广泛应用,EEDs在我们日常生活中无处不在,有报告显示中国人血清中某些EEDs浓度近年来增加非常显著.生殖系统是EEDs的高敏感靶器官之一,人群流行病学资料己证实大剂量EEDs暴露的生殖毒性,包括对对妇女妊娠能力及结局、子代产生不良影响的重要危险因素的研究.然而,以往研大多集中于2004年《斯德哥尔摩公约》定义的十二种传统的EEDs.近年来,诸如双酚A(BPA)和三氯生(Triclosan,TCS)一类的新型EEDs在日常生活中使用量不断增多,其在人体中的蓄积也逐渐上升.但有关此类EEDs对人类生殖的影响仍亟待人群调查和相应分子机制研究.旨在了解BPA和TCS对女性生殖道影响,我们收集并检测了304名单纯管性不孕的IVF-ET患者提供的219例尿液和281例卵泡液中BPA和TCS的浓度,分析(新鲜)胚胎移植患者的174份尿液和206份卵泡液中BPA和TCS浓度与其胚胎种植率的相关性;使用小鼠模型,研究BPA和TCS暴露与胚胎种植,子宫蜕膜化的相关性,分析了相关的分子机制.结果显示:患者尿液高BPA水平组胚胎种植率显著性低于尿液低BPA水平组(P=0.008);但是卵泡液中BPA水平与胚胎种植率无现显著性相关(P>0.05);尿液和卵泡液中TCS浓度与种植率均无显著相关性(P>0.05).小鼠模型显示:种植前(embryo day 0.5-3.5)灌胃环境相关剂量的BPA或TCS均可导致种植点数目剂量依赖性下降;环境相关剂量BPA或TCS灌胃小鼠蜕膜化的子宫重量增加低于对照组(P<0.05),并显著影响母鼠子宫血管生成.机制研究表明BPA可通过下调子宫上皮细胞SGK1和ENaC α的影响小鼠子宫内膜蜕膜化,其可能通过雌激素途径发挥作用.结论:BPA和TCS暴露可以抑制母鼠子宫内膜蜕膜化及其血管生成.其中BPA可能通过雌激素受体通路,影响SGK1/ENaCα表达,进而影响IVF胚胎种植.
