Diabetic foot ulcers (DFU) take a long time to heal,seriously affect the quality of life of patients and has a heavy burden on social *** 1998,a multicenter epidemiological study on inpatients with chronic wounds indicated that the leading causes of chron ic cutaneous wounds were trauma (67%),with low frequency of DFU (<5%).Ten years later,the leading causes of chronic cutaneous wounds were DFU (32.6%),trauma/burns (23.8%),iatrogenic factors (10.6%) and pressure ulcer (10.5%).DFU is the leading cause of amputation in patients with diabetes,also big burden for patients' family and society.

随着新一代测序技术的建立,人类对肿瘤的治疗进入一个新的时代。2015年年初美国总统奥巴马在今年的国情咨文中启动了"精准医学计划",在随后的讲话中他进一步阐述了精准医学计划的内容。早在2012年新英格兰杂志曾发表一篇文章标题为"Preparing for Precision Medicine",文中对精准医学进行了定义,即用最先进的分子特征谱,结合临床病理资料量体裁衣,设计针对每个病人的诊断、预后、治疗方案。精准医学主要由两项重要技术驱动—DNA测序和基因组技术。力争把根据基因匹配肿瘤用药变得像输血配血型那样标准化,把找出正确的用药剂量变得像测体温那样简单。而实现精准医学必须具备5个要素:1,完善的生物信息学分析,包括临床病理资料和基因组数据等;2,先进的检测技术及方法学;3,疾病特异性分子标记物的发现及功能研究;4,高特异性靶向药物的开发;5,政策和法规的支持。我国的医疗水平与美国比较尚存在一定的差距,实现精准医学的道路更加漫长。但是国内学者及医疗机构已经开始关注并进行了有益尝试。复旦大学附属中山医院成立了精准医学中心,从心脏疾病,糖尿病和恶性肿瘤的个体化治疗和预防以及基因体检等方面,进行了精准医学的探索和实践,取得一些进展并积累了经验,为进一步推广及实现精准医学打下了基础。
