目的:通过107例经尿道等离子双极前列腺电切术(plasma kinetic resection of prostate,PKRP),评价该手术对生活在海拔2200米以上前列腺增生(benign prostatic hyperplasia,BPH)患者的治疗效果。方法:患者Ⅰ度增生27例,Ⅱ度增生56例,Ⅲ度及以上增生24例,57.2%的患者并有心脑血管、肺、肝、肾、糖尿病等脏器的并发症。应用PKRP治疗BPH患者107例,手术方法与经尿道前列腺电切术(transurethral resecition of prostate,TURP)操作相同。结果:平均手术时间54分钟,平均切除前列腺组织32克,术中均未输血,术后留置导尿管平均5.2天,随访3-12个月,国际前列腺症状评分(IPSS)由术前平均27.8分降至术后的9.6分(P<0.01),生活质量评分由术前的平均5.4分降至1.9分(P<0.01)。结论:PKRP治疗BPH具有手术时间短、安全性高、并发症少、疗效确切、医疗费用低、可提供高质量的病理标本等优点,适合在西宁地区开展。
