Purpose To explore the value of whole-brain CT perfusion(CTP)for detecting crossed cerebellar diaschisis(CCD)in patients with basal ganglia hemorrhage,in order to provide basis for clinical *** and Methods The whole-brain CT data and clinic information of 33 patients with basal ganglia hemorrhage were retrospectively *** to the normal cerebellar hemisphere based on CT scan and accompanying hypoperfusion of the contralateral cerebellar hemisphere,all patients were divided into CCDpositive group and CCD-negative *** different regions of interest(ROI)were manually outlined on CTP maps for peripheral lesions including normal-appearing brain tissue surrounding the perilesional low-density area(NA)and perihematomal low-density area(PA).Cerebral blood flow(CBF),cerebral blood volume(CBV),mean transit time(MTT)and time to peak(TTP)levels were measured in the above ROI in all patients and cerebellar(including ipsilateral and contralateral)in CCD-positive *** perfusion parameters were compared between the contralateral and ipsilateral cerebellum in CCD-positive group,and the values of continuous variables(including age,hematoma volume,NIHSS score,the degree of NA and PA perfusion abnormality)were also compared between the CCD-positive group and *** relationships between the degree of CCD severity and the variables(including NIHSS score,hematoma volume,the degree of NA and PA perfusion abnormality)were further *** A total of 11 CCD-positive cases had significant differences in the perfusion parameters(values of CBF,CBV,MTT and TTP)between the contralateral and ipsilateral cerebellum(P<0.05).The degree of cerebral perfusion abnormality and the degree of CCD severity showed significant linear correlation(r=-0.65,0.65,P<0.05).Conclusion Whole-brain CTP can effectively evaluate the relationship between CCD after basal ganglia hemorrhage and the degree of cerebral perfusion abnormality.
