支气管肺发育不良是由严重呼吸窘迫综合征导致的慢性肺部疾病。2000年6月由美国国家儿童卫生与人类发育研究机构(NICHD),国家心脏、肺和血液研究院(NHLBL)及少见疾病委员会(ORD)联合举办的BPD研讨会上制定了BPD新定义和疾病分度:BPD是... 详细信息

支气管肺发育不良是由严重呼吸窘迫综合征导致的慢性肺部疾病。2000年6月由美国国家儿童卫生与人类发育研究机构(NICHD),国家心脏、肺和血液研究院(NHLBL)及少见疾病委员会(ORD)联合举办的BPD研讨会上制定了BPD新定义和疾病分度:BPD是指任何氧依赖(>21%)超过28天的新生儿。如胎龄<32周,根据纠正胎龄36周或出院时需氧分为:(1)轻度:未用氧(2)中度:Fi O2<30%(3)重度:Fi O2≥30%或需机械通气。BPD常继发于其他肺部疾患,发展到慢性期时预后差,早期诊断及治疗非常重要此对本病的早期治疗至关重要,其诊断有赖于影像学检查。影像学检查方法及影像表现胸部X线Northway1最早根据BPD的病理过程将胸部X线表现分为4期:I期(1～3天):双肺毛玻璃影,呈HMD样改变;II期(4-10天):肺实变明显;III期(11-30天):两肺多发小囊状低密度区,伴肺不张;IV期(1个月以后):小囊状低密度区扩大呈囊泡状,伴两肺结构紊乱,有散在条片或斑片影,以及充气过度和肺不张。X线表现中仅有肺过度充气和肺纹理模糊,偶见小泡状影。单纯靠胸部X线征象来诊断BPD往往要到病程的1至2个月甚至更晚,此时支气管及肺已发生不可逆转的损害。虽然早产儿支气管肺发育不良的诊断主要依据临床病史,对于临床怀疑而X线胸片无明显改变的患儿,应行胸部CT检查以期早期确诊。胸部多排螺旋CT2.1常规CT平扫CT与X线相比,发现结构异常的敏感性高,主要包括线性和三角形胸膜下密度增高影,周围肺组织异常:"马赛克"衰减和肺气肿、空气储留。病变多发生在两下肺,常呈对称性。胸部CT上多发囊状影是诊断BPD的重要征象,连续影像随访有助于该病的早期诊断。鉴于病理改变上以肺泡和肺微血管发育不良为主要特征,可见囊泡影对早期诊断有重要意义。How ling等也认为,CT上的多发囊泡影是断BPD的重要依据。但囊泡影对于支气管肺发育不良并不是特征性的。多排螺旋CT分辨率高,结合薄层重建,既可控制较小的辐射剂量,图像分辨率亦高。2.2多层螺旋CT容积漫游技术(Vo lume rendering Technique,VRT)CT轴位像提高了疾病的检出率,但对病变的形态、大小及范围并不直观、立体。利用VRT可充分暴露气管树,通过旋转、多方位、多角度观察,真实显示容积内解剖结构与病变间相互关系,可发现气管、支气管树有无发育异常。通过VRT发现支发育不良肺组织体积缩小或无充气肺组织结构,相应气管支气管不规则狭窄或不发育。VRT需薄层厚、小螺距扫描,以提高图像空间分辨力,结合切割技术,除去床板、胸壁软组织及含气食管,必要时切除对侧肺组织,以充分暴露患侧气管树。VRT安全无创,重复性好,易被接受;与表面遮盖技术(SSD)相比,VRT为透明图像,层次多,丧失信息少,不受阈值影响;与多平面重建(MPR)相比,VRT直观、立体显示支气管树,优于MPR及CT横断面图像,二维影像是三维影像的基础,VRT不能取代二维影像,诊断时应密切结合CT二维影像以提高诊断的准确性。VRT不足是对叶及以下肺气肿此与脏层胸膜及支气管树重叠影响病变观察、CT扫描参数选择、后处理软件配置、VRT操作技巧等因素密切相关,如何提高VRT图像质量及解剖细节显示仍是目前有待进一步探索。高分辨CT(High Resolution Computerized Tomography,HRCT)胸部高分辨力CT能够较好地显示肺的细微结构,可早期显示肺部病变特征、次级肺小叶和肺内间质的解剖结构。传统HRCT是在常规CT扫描基础上进行的扫描,增加了工作时间和辐射量,多层螺旋CT容积数据进行的薄层高分辨率算法重组图像(thin-reconstruction of volumetric data with a high-resolution algorithm。VHRCT)以超薄的层厚(0.625～1.25 mm),通过高空间分辨率算法以及靶重建等技术,可显示肺间质病变的分布及特征,能早期发现肺部的细微病变。常见征象包括气体滞留、马赛克征象,网状阴影和结构破坏。HRCT还可显示治疗后肺发育状况的信息,有助于疾病的随访。Howling SJ等发现支气管肺发育不良患儿肺部后遗症的HRCT的主要表现:多发灶性肺密度减低区和灌注减少区、支气管壁增厚、支气管与伴随肺动脉直径比率减小、线性不透光区和肺大泡,胸膜下致密影、支气管扩张以及或气肿。Mgnetic Rsonance Iaging(MRI)尽管磁共振检查没有辐射,由于肺部组织结构和功能的特殊性,很短的T2值(0.9～22ms)及呼吸和心脏运动、肺部血流、分子弥散等因素,都限制了MRI在肺部的临床应用。近年来MR新技术和新序列的开发应用使MR能够用于观察肺实质,也为MR肺灌注成像提供了可

Purpose:To evaluate the value of 3D CT whole brain perfused volume(3D PBV)based on CT angiography(CTA)row data in hyperacute cerebral ***:35 patients with stroke in 3 hours performed no enhancement CT(NECT)and CTA scan.3D software Neuro PBV was utilizes to process the CTA row data and PBV of brain was *** examination was performed in 6 hours after CT *** ischemic lesion volume and quantity on 3D PBV and NECT were compared with ***:In 35 patients,the number of infarct lesion detected by MRI,PBV,NECT was 40,38,16 *** results of Kappa analysis between NECT and PBV with MR were-0.0128,*** result of Paired t-test for measurement of volume between PBV and MRI was t=7.249,P>*** lesions’which did not been detected by PBV volume were less than 4.5cm3Conclusion:3D PBV has the potential to assess the full extent of an ischemic stroke at an early stage,whereas PBV is limited to a relatively small ***3D PBV technique requires no additional x-ray exposure and contrast medium injection,and can be performed in a short period of time.

Purpose To evaluate the clinic application value of 4D computed tomography(CT)performed with a prototype 128-slice CT scanner in acute infarction caused by cerebral artery severe stenosis or *** Thirty-four patients with intracranial artery severe stenosis were scanned by 128-slice CT *** volume CT perfusion(CTP)parameter maps of cerebral blood flow(CBF),cerebral blood volume(CBV),mean transit time(MTT),and time to peak(TTP)ware *** cerebral CTA images were *** CTP was showed abnormal in Thirty-four *** decreased than contralateral side in the region of ischemic penumbra around the cerebral infarct(P>0.05).There were significant differences of CBV between affected side and contralateral side(P<0.05).TTP and MTT of affected side were significantly prolonged compared to contralateral side(P<0.05).The agreement between CTA and DSA was greater in the detection of cerebral artery severe stenosis or occlusion(kappa=0.888,P=0.039<0.05).Conclusion Comprehensive information of intracranial artery severe stenosis and hemodynamic changes in acute infarction could be achieved with the 4D-CTA protocol by 128-slice CT *** has great value for clinical application.
