For patients presenting with ST Elevation Myocardial Infarction (STEMI), both thrombolytic therapy and primaryangioplasty are now established treatment for restoration of blood supply to the affected myocardium in suitable *** thrombolytic therapy may be administered by all hospitals with trained physicians, primary angioplasty is limited toselected tertiary hospitals with highly trained interventionist, and supportive services and staff. Pereutaneous coronaryrevascularization during acute myocardial infarction involves balloon angioplasty, with use of coronary stenting where fea-sible, and adjunctive pharmacotherapy (heparin ±platelet glycoprotein Ⅱb/Ⅲa receptor inhibitor). Although primaryangioplasty is proven to be an effective treatment for STEMI, recently concluded trials showed that there is still unresolvedissues especially for those who presented with occluded vessel prior to primary angioplasty.

Deletion of the angiotensin (Ang) AT1a gene produces animals that are more sensitive to osmotic stimulation. Toexplore this finding, we evaluated the effect of consumption of a high salt diet (8% NaCl) on the circadian rhythm ofmean arterial pressure (MAP), heart rate (HR) and drinking activity in AT1a -/- and control (AT1a + / + ) malemice. A radiotelemetric transmitter was implanted into the carotid artery (Data Sciences Intl). After a 4-5 d recoveryperiod, MAP, HR and water consumption (total volume and licking activity) were measured for 24 h on day 0 (baseline)and days 2, 5 and 8 of salt consumption. Under basal conditions, AT1a -/- mice had lower MAP than controls duringboth the light and dark phases (86.2±6.9 vs 108.1±3.7 mmHg, light, P<0.05;93.6±3.2 vs 118.5±4.5

Background: Ischemic preconditioning （IPC） is a phenomenon in which a brief peried of ischemia confers moretolerance to prolonged ischemia. Even though IPC has been studied for more than sixteen years since Murry et al first re-ported in 1986, the mechanisms of IPC that mediate the protection have not been clearly elucidated. IPC reduces theseverity of acidosis during prolonged ischemia, which is associated with reduced intracellular sodium and calcium contentsvia stimulation of NHE activities. However, these reports are consistent with numerous studies showing benefits of NHEinhibitors. Enhanced NHE1 activity may accelerate ischemia/reperfusion injury which is reduced by IPC. Although nu-merous studies have reported cardiac protective effect by IPC or using NHE inhibitors, lack of specific inhibitors of eachisoform and drug toxicity make it difficult to assess their effect on the cardiac function. To overcome these difficulties,genetic approach of ablation of NHE1 may provide definite clues on its function when compared to the effect of pharmaco-logical inhibitors. Recently we demonstrated that Nhel mice are more resistant to ischemia, however, it is not knownwhether we can add additional protection in Nhel mouse against lethal ischemia.

Background: An occluded vessel with heavy thrombus load in the infarct related artery (IRA) is a common findingin acute ST-elevation myoardial infarction (STEMI). Percutaneous coronary intervention (PCI) is not uncommonly as-sociated with slow-or no-flow. Current recommendation is to administer Glycoprotein 2b/3a receptor inhibitor(GP2b3aI) during angioplasty. Despite their use, PAMI - stent and CADILLAC trials showed that PCI outcome withGP2b3aI is still sub - optimal. In striated muscle including skeletal and cardiac muscle, the extramyofibrillar cytoskeleton consists of 3 compo-nents: microfilaments, microtubules, and intermediate filaments. Unlike the former two that are always formed mainly bythe same proteins in different cell types, the intermediate filaments are composed of tissue - specific intermediate filamentproteins. The classical muscle - specific intermediate filament protein is desmin. Changes in the expression and/or dis-tribution of desmin protein have been observed in many cardiac diseases. The change can be primarily due to genetic de-fects including mutations in the desmin gene itself and mutations in other proteins that may be essential to the formationand maintenance of desmin filaments or to desmin function. In some cases, the alterations of desmin filaments in cardiacmyocytes are a stress response to various insults imposed upon the heart. Recently, the functional consequences of bothup-regulation and altered distribution of desmin protein have been investigated using mouse transgensis. These studiesclearly demonstrated that altered distribution and organization of desmin filaments can compromise cardiac function andremodel the heart while overexpression of wild type desmin is not, by itself, detrimental to the heart.

Background and objectives: It is known that fibrobast growth factor-2 (FGF-2) is produced from a single copygene as five different isoforms with molecular masses of 18, 22, 22.5 24 and 34 kD through alternative translation initia-tion at AUG and CUG start codons and through internal ribosomal entries. The cellular localization and functions of 18 kD(LMW FGF-2,'lo' FGF-2) and other higher molecular weight forms of FGF-2 (HMW FGF-2,'hi' FGF-2) aredifferent. The 'lo' FGF-2 is mostly cytoplasmic, while the majority of the 'hi' FGF-2 is directly translocated into thenucleus. Apparent difference in function between 'lo' FGF-2 and 'hi' FGF-2 forms have been reported. Contrast to'hi' FGF-2, 'lo' FGF-2 enhances cell motility, alters integrin and tissue plasminogen activator (tPA) expression. Onthe other hand, overexpression of 'hi' FGF-2 reduces the serum requirement for cell growth, decreases expression oflaminin B1, tPA and plasminogen activator inhibitor-1 (PAI-1), inhibits interleukin-6 promoter activity. Use of in-hibitor of FGF-2 binding to its recepors or overexpression of dominant negative FGF receptor inhibit the biological effectsinduced by 'lo' FGF-2, but not by 'hi' FGF-2, Which suggest different pathways of autocrine FGF receptor-depen-dent manner and intracrine FCF receptor-independent manner between 'lo' and 'hi' FGF-2s.
