Background & Aims:The clinical value of quantitative hepatitis B surface antigen(qHBsAg) titer in patients taking nucleot(s)ide analogs(NAs) is still *** study aims to investigate the dynamic changes of qHBsAg titers and its significance for predicting virological response(VR) and serological response(SR) to long-term entecavir(ETV) treatment. Methods:Forty-eight ETV-nai've patients were enrolled and followed prospectively for 4 years, and 32 of them were HBeAg-positive at *** alanine aminotransferase(ALT),qualitative hepatitis B virus(HBV) serological markers,and HBV DNA were detected;and qHBsAg titers were measured using Elecsys? HBsAgⅡQuant Assay. Results:The mean baseline HBV-DNA and qHBsAg were 7.51 log copies/mL and 3.78 log IU/mL,*** 48 months of ETV treatment,the rates of VR(<291 copies/mL),ALT normalization and SR(HBeAg/anti-HBe) were 89.6%(43/48),89.6%(43/48) and 34.4%(11/32), *** was a decline in qHBsAg titers from baseline to months 48,ranging from 3.78 to 3.10 log IU/*** greatest decline of HBsAg was observed in the first 3 months of treatment (0.47 log10 IU/mL),which was significantly correlated with corresponding HBV-DNA decreasing (3.89 log copies/mL)(P=0.032).By using ROC analysis,qHBsAg titers at baseline(AUC=0.647) and 3 months after treatment(AUC=0.586) had poor power in predicting of 48-month VR;qHBsAg titers at baseline(AUC=0.779) and 3 months after ETV treatment(AUC=0.658) had poor power in predicting of 48-month SR in HBeAg-positive patients at ***,the declines of qHBsAg in the first 3 months of treatment also had poor power in predicting of either 48-month VR or SR. Conclusions:ETV is efficacious in NAs-nai've patients,and qHBsAg titers declined significantly in the first 3 months of ETV ***,qHBsAg titer is not a good predictor of 4-year virological and HBeAg/anti-HBe serological responses in this cohort.

Background & Aims Increased KCNJ15 expression has been found in patients with hepatitis B virus(HBV)-induced Acute-on-chronic liver failure(HBV-ACLF) in previous gene microarray study. However,the mechanism underlying the enhanced expression of KCNJ15 in patients with HBV- ACLF remains *** this study,we further investigate the up-regulated KCNJ15 and its possible regulatory mechanisms in HBV induced liver injury. Methods The expression of KCNJ15 was studied in patients with HBV-ACLF on both mRNA and protein levels in peripheral blood mononuclear cells(PBMC) and liver tissue,while the function of KCNJ15 was further determined in Jurkat cells in vitro by transfection of a human KCNJ15 expression plasmid. Results KCNJ15 was over-expressed on PBMCs and hepatic infiltrating lymphocytes from HBV- ACLF ***15 had an abundant expression in peripheral and hepatic CD4+T cells in patients with *** the supernatant of KCNJ15 over-expressed Jurkat cells,increased secretion of IL-17A was observed. Conclusions The novel KCNJ15 gene was highly expressed in liver and PBMCs of HBV-ACLF ***15 might contribute to liver injury through IL-17A production in virus induced liver *** more,our data provide a potential susceptible gene for patients with HBV induced liver failure.

Abstract T-bet is a key regulator for the lineage commitment in CD4 T helper（Th） 1 cells by activating the hallmark production of interferon-γ.Previously,two single nucleotide polymorphisms（SNPs） in the TBX21 promoter, T-1993C and T-1514C,have been shown by statistic studies to associate with systemic lupus erythematosus（SLE）.The effect of-1993 SNP on the Yin Yang 1 transcription factor- mediated promoter activity has been already *** study aimed to investigate roles of the T-1514C SNP on TBX21 transcription and its functional effect by luciferase reporter,electrophoretic mobility shift assay（EMSA）,chromatin immunoprecipitation（ChIP） assay,and flow cytometric analysis of intracellular T-bet,IFN-y,and IL-4 expression in activated CD4 T *** TBX21 promoter carrying -1514C possessed significantly lower transcriptional activity than that of-1514T and was markedly downregulated by the overexpression of upstream stimulatory factor 1（USF-1） when compared with the promoter carrying -*** indicated that the transcription factor USF- 1 was bound to the -1514C allele probe with the affinity higher than that to the -1514T allele *** assay suggested that USF-1 bound around -1514 of TBX21 genomic DNA in vivo in the human T cell line Jurkat with -1514C/*** individuals carrying -1514C allele were determined to have significantly diminished expression of T-bet and IFN-y and increased IL-4 production in CD4 T cells compared with those of -1514T *** findings demonstrate that the T-1514C polymorphism affects TBX21 gene expression and Thl cytokine production by binding USF-1 to the SNP site.

Objective To investigate the molecular mechanism of SREBP-lc expression regulated by HCV core protein. Methods construct pcDNA3.1/myc-His(-)-Core(1b,3a) and transfect HepG2 cells,determine the mRNA and protein expression level of SREBP-lc and Sirtl by real time PCR and Western blot 48h *** pGL4.10-Sirtl and co-transfect HepG2 cells with pcDNA3.1/myc- His(-)-Core(1b,3a),measure the Sirtl promoter activity by Luciferase assay 24h post-transfection. construct pmirGLO-SREBP-1c-3' -UTR and co-transfect HepG2 cells with pcDNA3.1/myc-His(- )-Core(3a),measure the relative luciferase activity of SREBP-lc-3' -UTR by Luciferase assay 48h post-transfection. Results HCV core proteins(lb,3a) up-regulate the mRNA level(1.358 times and 1.337 times respectively,n=3,p<0.05) and protein expression(1.608 times and 1.926 times respectively,n=3, p<0.05) of *** HCV core proteins of genotype lb and 3a up-regulate the mRNA level (1.566 times and 1.71 times respectively,n=3,p<0.05) and protein expression(1.436 times and 1.588 times respectively,n=3,p<0.05) of *** Sirtl promoter did not show significant transcriptional activity in the presence of HCV core proteins(1b,3a).The relative luciferase activity of SREBP-1c- 3' -UTR shown significant decrease in the presence of HCV core protein of genotype 3a(RQ=0.667, n=3,p<0.05).Conclusion HCV core proteins up-regulate the mRNA and protein expression level of SREBP-lc and Sirtl,which may be one of the pathogenesis of HCV related *** the regulation of SEBP-1c by HCV core protein,microRNA may play an role of *** there are microRNAs contribute to the regulation of Sirtl by HCV core protein still need to be investigated.

AIMS:The underlying pathogenesis of fulminant viral hepatitis(FVH) has not been fully *** a subset of regulatory T cells,CD3+CD4-CD8- double negative(DN) T cells can suppress T cell *** this study,we present new insights into the immune mediated mechanisms involved in FVH caused by murine hepatitis virus strain 3(MHV-3). METHODS:The phenotype and cytokines of DN T cells were detected by flow cytometric *** levels of mfgl2 were measured by real-time PCR and *** function of mfgl2 was measured by PCA. RESULTS:After MHV-3 infection,the proportions of DN T cells increased significantly in BALB/cJ mice,and splenic DN T cells expressing high levels of CD69 were recruited by MHV- 3 infected hepatocytes to the *** levels of alanine aminotransferase(ALT),aspartate aminotransferase(AST) and total bilirubin(TBil) increased,accompanied by massive hepatocyte *** DN T cells were predominantly consisted of a TCRαβ+ subset expressing high levels of CD44,and did not produce cytokine except *** transfer of this subset of DN T cells to the MHV-3 infected mice resulted in an increase of murine fibrinogen—like protein 2(mfgl2) expression in association with massive fibrin deposition in the *** MHV-3 infection,membrane mfgl2 expression and functional procoagulant activity(PCA) increased remarkably in the DN T cells. Introduction of a recombinant adenovirus which encoded a microRNA specifically targeting mfgl2 gene (Ad-mfgl2-miRNA) in vivo significantly inhibited the hepatic expression of mfgl2,increased mice ***,under this condition,adoptive transfer of the DN T cells accelerated the disease progression and reversed the benefit from mfgl2 gene silence,led to a 100%death. CONCLUTION:Our results demonstrated that DN T cell-derived mfgl2 may serve as an important effector molecule contributing to the pathogenesis of MHV-3-induced FVH.

Background/Aims:Hepatitis B related acute-on-chronic liver failure has a poor prognosis with very high ***,most prognostic predictive models of liver failure are complicated and offer suboptimal *** in entecavir-treated patients with HBV-ACLF is limited. This study was designed to evaluate the efficacy and safety of entecavir in patients with HBV-ACLF and to develop a novel model(Tongji prognostic predictory model,TPPM) for prognostic prediction of HBV-ACLF patients. Methods:In this retrospective study,248 patients with HBV-ACLF were *** were no signifcant differences in baseline clinical and virological characteristics between patients treated with and without entecavir. Results:The 1- and 3-month survival rates of patients in the entecavir-treated group were 72.58%and 61.29%,significantly higher than that in NA-free group which were 53.23%and 45.97%. The entecavir-treated group also achieved a better improvement of MELD score when compared with the *** multivariate logistic regression,high INR for prothrombin time,≥2 Complications and high total bilirubin,but not HBV DNA,were independent predictors of liver-related mortality. By Hosmer and Lemeshow test,TPPM for HBV-ACLF with a very good degree of fit with disease *** on this unique group of patients,the TPPM scoring offered a better prediction value in both specificity and sensitivity for 3-month mortality of patients with HBV-ACLF compared with MELD scoring system with statistically significant *** the patients with HBV-ACLF,using a cutoff of 0.22 for 3-month predicted mortality by TPPM,the positive predictive value was 93.6%, with a negative predictive value of 91.3%. Conclusions:Entecavir treatment prevents disease progression and increased the survival of patients with *** established TPPM scoring system offers superior predictoiy value in both specificity and sensitivity for HBV-ACLF patients when compared with MELD.

Background Initial cases of 2009 pandemic influenza A(H1N1) virus infection having been reported mostly represented a mild *** the number of infected persons increased in our country,more severe infections were reported in the second half of last *** first severely ill patient in our hospital was confirmed in October 26,2009. Methods During October 26,2009 and January 23,2010,totally 71 cases treated in our hospital who developed community acquired pneumonia from confirmed or clinically suspected 2009 pandemic influenza A(H1N1) were observed in this study,including both hospitalized patients and outpatients. Real-time reverse-transcriptase-polymerase-chain-reaction(RT-PCR) testing was used to confirm *** data were collected retrospectively and *** 3-month follow up, high-resolution chest CT and lung function tests were performed. Results Of the 71 cases,median age was 41(IQR 28 to 56) years;63.4%were male,54.9%had at least one underlying medical condition.68 patients received antiviral treatment,and 33 received intravenous corticosteroids.36 of 71 patients developed acute respiratory distress syndrome during the *** 25 patients who were treated by mechanical ventilation,14 survived with noninvasive ventilation,and 10 required invasive ventilation.9 patients died at a median of 16(IQR 10 to 24) days after onset of *** risk factors were identified to be independently associated with death analyzed by multivariate Cox regression:progressing dyspnea after resolution of fever(relative risk,5.667;95%CI,1.312 to 24.475;P=0.020) and higher APACHE II score on presentation(relative risk for each point,1.329;95%CI,1.156 to 1.527;P < 0.001).At 3-month follow up of H1N1 pneumonia survivors,ground-glass opacities still existed in most patients(82%) with smaller extent,and lung function tests revealed that decreased diffusing capacity for carbon monoxide of mild degree is the most common(60%) abnormality.
