目的:探讨女性2型糖尿病肾病(diabetic nephropathy,DN)患者骨密度(bone mineral density,BMD)及骨代谢指标的变化。方法:将123例2型糖尿患者按尿白蛋白排泄率(AER)分为两组,2型糖尿病(type 2 diabetics mellitus,T2DM)组65例,DN组58例。比较两组患者各部位BMD及血生化和骨代谢指标的变化。并用二元logistic逐步回归方法分析其他相关因素对AER的影响。结果:DN组各部位BMD均数均低于无肾病T2DM组,在校正体重指数(body mass index,BMI)后,差异更加明显,其中L2、L3、L4、L1-4、NECK、WARDS差别有统计学意义(p<0.05)。DN患者血磷低于T2DM组,骨代谢指标DN组有下降趋势,但两组间差异无统计学意义。应用二元logistic回归方法进行分析,血肌酐、血压与ARE有关。结论:DN患者BMD低于无肾病T2DM患者,且骨代谢呈低转换型。
