【目的】通过药物诱导睡眠内镜检查(Drug induced sleep endoscopy,DISE)观察中重度阻塞性睡眠呼吸暂停低通气综合征(Obstructive sleep apnea hypopnea syndrome,OSAHS)患者上气道各部位阻塞程度和形态的变化,了解改善上部气道阻塞之... 详细信息

【目的】通过药物诱导睡眠内镜检查(Drug induced sleep endoscopy,DISE)观察中重度阻塞性睡眠呼吸暂停低通气综合征(Obstructive sleep apnea hypopnea syndrome,OSAHS)患者上气道各部位阻塞程度和形态的变化,了解改善上部气道阻塞之后下部阻塞情况,探讨上气道上部和下部阻塞之间的联系,为中重度OSHAS患者多平面手术提供理论依据。【方法】给予55例经多导睡眠监测(Polysomnography,PSG)诊断为中重度OSAHS患者DISE检查,分别观察患者入睡后腭咽、口咽、舌根、会厌、喉部的阻塞程度和形态。对经DISE检查发现上气道上部和下部同时阻塞的患者,给予一侧鼻腔置入鼻咽通气管(通气管下缘越过软腭游离缘到达口咽部)解除上部阻塞后,再次行DISE检查,观察患者此时下部阻塞的程度和形态的变化。其中,上部阻塞指腭咽或口咽阻塞,下部阻塞指舌根、会厌、喉部任意部位阻塞;气道阻塞程度分为无阻塞(塌陷度<50%)、不完全阻塞(50%≤塌陷度<75%)、完全阻塞(塌陷度≥75%);阻塞形态分为X轴(左右方向)、Y轴(前后方向)和环形阻塞。【结果】55例患者轻度0例,中度5例,重度50例,呼吸暂停低通气指数(Apnea hypopnea index,AHI)为(16.0～105.4)次/小时,平均(60.13±23.54)次/小时。55例患者均完成DISE检查,其中53例同时存在上部及下部阻塞。1.放置鼻咽通气管前上气道具体阻塞情况:(1)上部阻塞:55例患者均有腭咽部气道阻塞,其中完全阻塞52例,以环形阻塞为主(46/52);口咽部气道阻塞53例,其中完全阻塞47例,以X轴阻塞为主(35/47)。(2)下部阻塞:舌根部阻塞47例,其中完全阻塞36例;47例中X轴阻塞17例,表现为咽侧壁塌陷向中线挤压;Y轴阻塞18例,为舌根后坠阻塞气道;环形阻塞12例,多由舌根后坠与侧壁塌陷共同所致。会厌部气道阻塞49例,其中完全阻塞36例;49例中X轴阻塞19例,为咽侧壁向中线塌陷挤压会厌呈"V"型;Y轴阻塞15例,表现为会厌软化及后倾,如活瓣样随呼吸运动与咽后壁紧贴,堵塞咽腔;环形阻塞15例,表现为会厌后倾和咽侧壁塌陷共存。喉部气道阻塞28例,其中完全阻塞11例,主要表现为杓区粘膜和(或)杓状会厌襞向声门内卷曲堵塞声门。2.放置鼻咽通气管后:53例上部和下部气道同时阻塞的患者一侧鼻腔置入鼻咽通气管,有1例不能耐受,放弃检查,其余52例顺利完成余下检查。上气道具体阻塞情况:(1)舌根部阻塞原有46例,改善20例,其中17例从完全阻塞变为不完全阻塞,1例从不完全阻塞变为无阻塞,另有2例从完全阻塞变为无阻塞。(2)会厌部阻塞原有48例,改善6例,其中2例从完全阻塞变为不完全阻塞,4例从不完全阻塞变为无阻塞。(3)喉部阻塞原有28例,改善4例,其中有1例从完全阻塞变为不完全阻塞,有3例从不完全阻塞变为无阻塞。3.对比放置鼻咽通气管前、后的下部阻塞情况,发现舌根部气道阻塞改善有统计学意义(P<0.05),会厌及喉部气道阻塞改善无统计学意义(P>0.05)。在舌根部阻塞改善的20例患者中,11例为X轴即咽侧壁塌陷改善,5例为Y轴阻塞改善,4例为环形改善,分别占原X轴、Y轴和环形阻塞人数的的68.75%、27.78%、33.33%。放置鼻咽通气管后,最低血氧饱和度高于放置之前,由0.520～0.900,平均0.750±0.095,提高到0.690～0.970,平均0.895±0.107。差异有统计学意义(P<0.05)。【结论】1.中重度OSAHS患者上气道下部阻塞的形态及程度具有多样性,放置鼻咽通气管解除上部阻塞后,下部阻塞改善程度具有个体差异,其中舌根部阻塞与上部阻塞关系较为密切,而会厌部及喉部阻塞改善较差。***联合鼻咽通气管对上气道的观察对于评估悬雍垂腭咽成形术(Uvulopalatopharyngoplasty,UPPP)手术疗效及多平面手术方案的制定有一定价值。

Objectives:This study evaluated the association between sleep patterns and the risk of being overweight/obese in Chinese ***:A total of 3086 children(1608 boys and 1478 girls)aged between 7 and 14 years in primary schools were recruited as eligible study participants in the present *** collected the children's information about sleep patterns,body height and weight,insomnia,healthy status,time allocation of daily activities,and demographic characteristics using a parental-reported ***:Overweight/obese children were younger,predominantly male,and more prone to have suffered from illness in the past 12 months compared to normal-weight *** were also less prone to compensate for sleep deficit during weekends(47.6%vs.39.1%;x2=11.637,P<0.001)and holidays(52.0%vs.42.0%;x 2=16.057,P<0.001).Sleep duration on weekdays did not affect the risk of being overweight/*** adjusted ORs for overweight/obesity using the compensated group as a reference were 1.197(95%confidence interval[CI]:1.004-1.493)during weekends and 1.309(95%CI:1.052-1.630)during holidays for the non-compensated ***:Sleep deficit compensation on non-weekdays may ameliorate the risk of being overweight/obese in Chinese ***,no significant association between the risk of being overweight/obese and sleep duration on weekdays was demonstrated in the current study,which may be due to pervasive sleep insufficiency on weekdays in Chinese children.

In insomniac individuals,memory complaint is frequent but not unequivocally corroborated by objective *** it is involved in dysfunction of neuroendocrine remains *** this study,21 patients with chronic insomnia disorder(CID)and 25 patients with insomnia comorbid with depression disorder(CDD)were enrolled from the clinical setting,and 20 good sleepers were serviced as *** memories,including spatial working and reference memories,object working and reference memories,and object recognition memory,were evaluated using Nine Box Maze Test,and the serum hormones were *** to the controls,the CID patients had more errors in the spatial working and object recognition memories,whereas the CDD patients had poorer performance in all assessed *** CID patients had higher levels of corticotrophin-releasing hormone,Cortisol and total triiodothyronine and thyroxine,and lower levels of thyrotropin-releasing hormone,gonadotropin-releasing hormone and adrenocorticotropic hormone than the CDD ***,the levels of all hormones were higher in the CID and CDD patients compared to the *** the associations between memories and hormones,after controlling confounding factors(age,sex,education,sleep and mood),only the CID patients,but not the CDD patients,showed few correlations,i.e.,Cortisol level correlated with the errors positively in the object working memory and negatively in the object recognition *** results suggested that the CID patientshad selectively impaired memory,andincreased Cortisol might direct.

Background:Parkinson's disease(PD)is a common progressive neurological disorder involving1.7%of the geriatric population in China and 2.0%of the elderly population globally,*** are no specific neuro-imaging modalities and reliable biomarkers for the diagnosis of PD until *** is usually diagnosed clinically based on the UK Brain Bank criteria,which are composed of 4 aspects mainly referring to motor symptoms,including bradikinesia,rigidity,rest tremor and postural ***,dementia,drug-induced psychosis,impulsivity and sleep disturbances may complicate the course and management of PD and lead to a variety of short-and long-term negative *** studies have focused on non-motor symptoms besides motor ***-motor symptoms such as sleep disturbances and mood disorders are common in patients with *** disturbances have been reported to occur in 90%of patients with ***,the relationship between non-motor symptoms and motor symptoms was not clear in Chinese patients with PD without ***:In this study,we sought to investigate changes in sleep in Chinese non-demented PD patients and further study the relationship between sleep quality and ***:A total of 487 patients with PD were recruited from the neurology out-patient clinics at the authors'affiliated institutions from March 2010 to *** patients were diagnosed with idiopathic PD by movement disorders neurologists according to the UK Brain Bank *** with severe cognitive impairment or their Mini Mental State Examination(MMSE)score<24 were *** with cerebrovascular disease,encephalitis,trauma and drug-induced Parkinson's syndrome were excluded by referring to CT or MRI scan ***'s plus syndrome,conditions suffering from a serious physical illness,previous history and family history of mental illness,and patients treated by antidepressant or anxiolytics were also excluded from this *** study protocol

【Objective】Chronic sleep restriction(CSR)is a big public health issue in modern *** could affect human health in many aspects,especially the association with Alzheimer's *** using CSR with wild type mice,we tried to study the cognitive impairments,the phenomenon and mechanism of beta-Amyloid(Abeta)deposition in brain cortex.【Methods】9-month-old C57 mice divided into 2 groups:C57 CSR group and C57 control *** wheelswere used to build the model ofchronic sleep restriction for 2 *** Water Maze(MWM)test was used to assess the behavioral ***,Amyloid Precursor Protein(APP),betasite APP Cleaving Enzyme 1(BACE1)and C99 protein level were measured by ELISA,Western Blot,and ***-antisense(BACEl-AS)transcriptand BACElmRNA expression were measured byRTPCR.【Results】Compared with control group,CSR group manifestedpoorer performance in Morris Water Maze test,accompanied by Abeta deposition in frontal ***1,C99 protein concentration were increased in CSR group than that of controlgroup,together with the increased level of BACE 1-AS level and BACElmRNA *** difference of APP was found between the two groups.【Conclusions】Our study showed the clear evidentthat sleep restriction could cause cognitive impairments and Abeta deposition in the brain cortex in adult wild type C57 ***-1 pathway played important role in modulate generation and deposition of Abeta,andBACEl-AScould be the key factor that enhance this pathological *** study also links the chronic sleep deficiency with sporadic Alzheimer's Disease.

The basal forebrain(BF)cholinergic neurons have long been thought to be involved in behavioral wakefulness and cortical ***,owing to the heterogeneity of BF neurons and the poor selectivity of traditional methods,the precise role of BF cholinergic neurons in regulating the sleep-wake cycle remains *** investigated the effects of cell-selective manipulation of BF cholinergic neurons on sleep-wake behavior and electroencephalogram(EEG)power spectrum using pharmacogenetic technique,the"designer receptors exclusively activated by designer drugs(DREADD)"approach and ChAT-IRES-Cre *** results showed that activation of BF cholinergic neurons expressing hM3Dq receptors significantly and lastingly decreased the EEG delta power spectrum,produced low-delta non-rapid eye movement sleep,and slightly increased wakefulness in both light and dark phases;whereas inhibition of BF cholinergic neurons expressing hM4Di receptors significantly increased EEG delta power spectrum and slightly decreased ***,the projections of BF cholinergic neurons were traced by humanized Renilla green fluorescent protein(hrGFP).Abundant and highly dense hrGFP-positive fibers were observed in the secondary motor cortex and cingulate cortex;and sparse hrGFP-positive fibers in the ventrolateral preoptic nucleus,a known sleep-related ***,we found that activation of BF cholinergic neurons significantly increased c-Fos expression in secondary motor cortex and cingulate cortex,but decreased c-Fos expression in ventrolateral preoptic *** together,these findings reveal that the primary function of BF cholinergic neurons is to inhibit EEG delta activity through activation of the cerebral cortex,rather than to induce behavioral wakefulness.

【目的】睡眠与其他健康相关行为一样,是人们健康生活的重要组成部分。过短或过长的睡眠都可能导致死亡率上升及心血管疾病、糖尿病、高血压的发病率升高。中国老年人一直认为午休是一种健康的习惯,但事实上午休的利弊迄今仍不明确。血管功能紊乱的发病率在中老年人群中日益增多,因此,人们自然地推测午休或夜间睡眠与发生大小血管疾病的风险或许具有关联性,午休和夜间睡眠两大因素可能共同而非单独地对大小血管疾病的发病风险产生影响。目前,尚无相关研究对此进行评估,因而我们开展了相关横断面研究。【方法】研究选取来自中国东南部福建省的8996名受试者(年龄≥35岁;56.5%为女性),通过对患者进行血管无创性检测(直接眼底检查/波普自动分析),评估午休或夜间睡眠时间(通过受试者自我报告)两大因素单独或共同对大小血管疾病发病风险所产生的影响。本研究经福建省立医院学术委员会审查批准,且每一名受试者都签署了知情同意书。所有参与研究的人员对本研究的目的和受试者个体情况均不了解。【结果】本研究的最重要亮点在于发现午休时间和小血管疾病间呈正线性关系。如图1所示,午休时间每增加6分钟(0.1h),小血管疾病的发病风险就会增加39%。未见其他潜在因素对此产生显著影响(校正后OR估计值=1.18～1.25,最大P值=0.024)。与无午休习惯者相比,午休者患小血管疾病的风险显著升高。午休时间0.1～1h组,多元校正后OR值=1.35;午休时间>1h组,多元校正后OR值=1.40(P线性趋势=0.022)。研究还发现,睡眠时间短与小血管疾病患病率的增加相关(睡眠时间参考值范围=7.1～8h;睡眠时间≥5h,多元校正后OR值=1.73;睡眠时间=5.1-7h,多元校正后OR值=1.21;睡眠时间≤8.1h,多元校正后OR值=0.92(P线性趋势=0.006)。这似乎表明午休导致小血管疾病风险升高可能是由夜间睡眠不足而补眠引起的,然而交互分析显示午休和夜间睡眠时间与小血管疾病无交互作用(P交互作用=0.960)。大部分先前的研究也显示午休与夜间睡眠时长并无显著联系。【结论】Masa等人提出午休可被视为睡眠呼吸暂停综合征的预测标志。Nazzaro等人则提出重度阻塞性睡眠呼吸暂停综合征可造成小血管的损伤,从中似可推测阻塞性睡眠呼吸综合征是促成午休和小血管疾病相关联的主要原因。然而现阶段研究表明,在回归模型中加入睡眠呼吸综合征的重要标志——打鼾并没有改变午休的OR估计值。同样地,调整睡眠的质量也并没有改变OR值。这一新发现的机制和意义仍待进一步的探究。据我们所知,通过大型横断面研究发现人类睡眠时间减少与小血管疾病发病风险增加之间存在关联性,尚属首例。睡眠时间短和小血管疾病发病风险间的分级联系值得进一步探讨。在这组横断面数据之中虽无法建立因果关系,但睡眠时间短可能是导致白天乏力或与睡眠生物学过程相关的精力恢复不良的诱因。睡眠不足可对炎性信号通路、代谢途径和神经通路造成严重影响而导致C-反应肽水平升高、胰岛素抵抗及交感神经活动改变,从而增加小血管疾病的风险。可能还存在其他潜在因素对睡眠及小血管疾病发病风险产生影响。虽然我们已将睡眠时间和小血管疾病之间的联系纳入研究范畴,但其他不可测量因素也可能对之产生作用。睡眠时间短和小血管疾病高发已成为当今社会的普遍问题,其诱发机制仍需进一步研究。本文关于睡眠时间越短(≤5h)则小血管疾病风险越高的发现仍处于初步阶段,有待进一步研究证实。我们的研究表明,午休和大血管疾病间不存在相关性。然而,Stang等人选取4797名45～74周岁德国受试者进行研究发现,每日午休时间与冠状动脉钙化积分及踝臂指数呈正相关。两项研究结论的差异可能源于受试者社会地位、身体状况、研究设计、大血管疾病的定义和测量方法的不同,也可能由偶然因素引起。目前相关研究较少,建议在将来有关血管疾病的研究中地加入午休的作用及不同种族/民族人群等因素进行系统性探讨。如今,越来越多研究结论显示睡眠时间越长,大血管疾病风险越大。与夜间睡眠时间为7.1～8h的受试者相比,夜间睡眠≤5h者患大血管疾病的多元校正后OR值为1.06;夜间睡眠5.1～7h者患大血管疾病的多元校正后OR值为0.82;夜间睡眠≥8.1h者患大血管疾病的多元校正后OR值为1.30(P线性趋势=0.002)。Abe等人发现长时间睡眠(≥7 h)与颈动脉粥样硬化密切相关,6小时睡眠时间次之,而更短的睡眠时间则与之无关。目前尚无研究表明更长的睡眠时间是大血管疾病的诱因。Shankar等人提出,除长时间睡眠与总死亡率相关外,还存在其他潜在诱发因素诸如抑郁症、甲状腺功能低下、社会经济地位低下或阻塞性睡眠呼吸暂停综合征。然而,本研究中受试者的社会背景和身体状况与西方国家完全不同。从长时间睡眠与危害健康的相关性可看出,长时间睡眠是导致各种慢性疾病

Abstract:Background:Insomnia disorder is a persistent sleep difficulty with some form of daytime impairments,but it is little understand for the mechanisms underlying these functional impairments.S100B isa calcium-binding protein secreted mainly in brain astrocytes,andits serum concentration has been considered a suitable surrogate marker for the diagnostic or prognostic assessment of neurodegenerative diseases,brain damage and ***:Toexplore the alteration of serum S100B level in thepatients with chronic insomnia disorder(CID)in the clinical ***:Fifty-three insomniac outpatients were categorized into CID(n=29)and CID comorbidmajor depressive disorder(CDD,positive control,n=24)*** demographically similar healthy subjects who accompanied the patients or had a medical examination were selected as negative *** subjects were assessed with the Pittsburgh Sleep Quality Index(PSQI),the 17-item Hamilton Depression Rating Scale(HAMD-17),and the Chinese-Beijing Version of Montreal Cognitive Assessment(MoCA-C).The special memory was evaluated with the Nine Box Maze *** S100B level was quantified with a quantitative sandwich ***:①Compared to the controls,the average PSQI,AIS and HAMD-17 scores were significantly higher in the insomniacs(Ps<0.001).The differences in PSQI and AIS scores between the two insomnia groups were insignificant(Ps>0.05),but the average HAMD-17scores in the CID group were significantly lower than those in the CDD group(Ps<0.05).②There were no significant differences of all tested cognition items,including the MoCA-C score and the number of errors in five forms of memories(spatial working and reference,object working and reference,and object recognition,Ps>0.05).③S100B concentrations in the sera of the CID,CDD and control groups were 17.12±4.73pg/ml,23.01+7.51 pg/ml and 14.67±3.74 pg/ml *** CID group and the CDD group had higher serum S100B than the control group(p=0.020,0.00
