目的检测活动性类风湿关节炎(RA)患者血清的DKK-1和RANKL水平,同时了解它们和蛋白聚糖代谢片段及关节破坏的相关性。方法本研究入选87例活动期的RA患者,用DEXA检测患者的BMD,Sharp's法评估患者的双手X线结果,ELISA法检测RA患者的血清DKK-1、RANKL及蛋白聚糖的KS代谢片段,同时检测90例正常人血清的蛋白聚糖代谢片段水平。并收集患者的人口流行病学资料、类风湿因子(RF)、抗CCP抗体、血沉及C-反应蛋白,计算RA患者的疾病活动指数(DAS28)。结果 RA患者的血清蛋白聚糖代谢片段水平明显高于正常对照组(5.3±1.9rig/μ1 VS 2.3±1.5ng/μl,F=48.324,P=0.0001)。血清蛋白聚糖代谢片段水平与DKK-1,RANKI,Sharp评分及抗CCP抗体呈正相关(相关系数分别为:r=0.335,P<0.0001,r=0.226,P=0.001,r=0.301,P=0.0001,r=0.358,P<0.0001)。多元回归分析显示血清蛋白聚糖代谢片段、DKK-1水平和股骨颈BMD是关节侵蚀的危险因素。结论活动性RA患者的血清蛋白聚糖代谢异常升高;同时检测蛋白聚糖代谢片段、血清DKK-1水平及股骨颈BMD可望能预测RA患者的关节破坏
