目的:脂肪组织在机体血糖稳态的调节中发挥重要作用,其慢性炎症是胰岛素抵抗和2型糖尿病发生发展的的主要因素之一。降钙素基因相关肽超家族:如降钙素基因相关肽,肾上腺髓质素,能改善机体的炎症反应。新近发现的降钙素基因相关肽超家族成员垂体中叶素(Intermedin),是一种重要的心血管保护肽。我们的研究是探讨IMD在高脂饮食诱导的脂肪组织炎症和机体胰岛素抵抗中的作用。材料和方法:16周的C57小鼠高脂饮食喂养8周,在喂养的第5周,皮下植入含IMD的微量渗透泵(300ng/kg/hour),对照组为生理盐水组。高脂饮食喂养8周后,我们检测了胰岛素抵抗相关指标及炎症指标。离体实验中,我们主要使用大鼠原代脂肪细胞和分化的3T3-L1脂肪细胞。结果:在高脂饮食喂养小鼠及db/db小鼠中,脂肪组织IMD和受体激活调节蛋白1、2、3(RAMP1,RAMP2,RAMP3)表达均下降。在高脂饮食喂养8周后,通过检测空腹血糖、血胰岛素、糖耐量试验(GTT)、胰岛素耐量试验(ITT)及正糖高胰岛素钳夹试验(EHC)等发现,高脂饮食第四周皮下持续微量给与IMD组小鼠胰岛素敏感性相比对照组明显改善。同时IMD处理组小鼠脂肪组织胰岛素信号水平明显增强,而血浆炎症因子(TNFα、IL-6、MCP-1等)和游离脂肪酸水平显著下降。脂肪组织H&E及免疫学染色显示IMD处理组小鼠,脂肪细胞脂滴更小,更少的"花环状结构"(crown like structures)及炎症细胞浸润,实时定量PCR和免疫组化显示IMD组炎症因子表达水平下降。为了探讨IMD改善高脂饮食小鼠胰岛素抵抗和脂肪组织炎症的机制,我们培养了大鼠原代脂肪细胞和分化的3T3-L1脂肪细胞,IMD预处理脂肪细胞,能改善由TNFα引起的炎症因子mRNA表达和蛋白分泌。抑制TNFα引起的P38和JNK磷酸化,以及IκBα的降解和NF-κb转录活性。同时我们发现IMD能时间和剂量依赖性的激活AMPK,ACC,使用.AMPK抑制剂Compound C能抑制IMD对NF-κb的作用。结论:上述结果显示IMD处理能改善高脂饮食引起的小鼠胰岛素抵抗,其可能的机制是IMD通过AMPK-NF-κB通路抑制脂肪组织的慢性炎症。
