目的研究健康人群和患病人群的腰椎骨密度(bone mineral density,BMD)与腰椎间盘退变(lumbar disc degeneration,LDD)的相关关系。方法选取2009年1月至2014年7月,来自体检中心的140例健康人群和骨科病房的127例患病人群。两组人群均具有完整的骨密度和腰椎磁共振资料。通过对每个人的腰椎磁共振影像观察并且根据Pfirrmann标准对每个椎间盘退变程度进行评分,结合骨密度检查得出的各节段的测量结果,采用多因素回归统计分析骨密度与年龄、体重指数(body mass index,BMI)以及腰椎间盘退变的相关性。结果腰椎骨密度与年龄呈负性相关(Coef.=-0.0044,P<0.01),与体重指数呈正性相关(Coef.=0.0097,P<0.01)。健康人群中各节段腰椎(L1-L5)的骨密度与相应节段的椎间盘退变均无相关(P>0.05)。患病人群中第一,第二腰椎(L1,L2)与相邻椎间盘(L1/2,L 2/3退变程度均无相关关系(P>0.05);而第三,第四腰椎(L3,L4)与相邻椎间盘(L3/4,L 4/5)退变呈正性相关(P<0.05)。结论健康人群腰椎骨密度不影响相邻节段椎间盘退变,患病人群中腰椎上段骨密度与相邻椎间盘退变无相关关系,而腰椎下端骨密度与相邻椎间盘退变呈正性相关,骨密度越大,相邻腰椎间盘退变越严重,骨密度增加可能是相应腰椎间盘发生退变的危险因素。
