目的 研究用聚乳酸-聚乙二醇共聚物(PLA-PEG)包载粉尘螨主要过敏原Der f 1蛋白的纳米疫苗特异性免疫治疗小鼠过敏性哮喘的机制及疗效。方法 选用聚乳酸-聚乙二醇共聚物(PLA-PEG)为佐剂,通过超声乳化法包载粉尘螨过敏原Der f 1蛋白制备成过敏原纳米疫苗,以小鼠过敏哮喘模型为研究对象进行特异性免疫治疗。通过检测小鼠治疗前后气道高反应性、血清中特异性抗体(IgE、IgG1和IgG2a)、细胞因子(IL-4、IL-10和INF-γ)生成水平以及肺组织病理学切片等指标对治疗模型的构建进行评价。结果 PLA-PEG/Der f 1纳米疫苗能有效抑制尘螨特异性抗体IgE、IgG1的产生和Th2型细胞因子IL-4的分泌,促进封闭性抗体IgG2a产生和Th1型细胞因子IFN-γ及免疫耐受型细胞因子IL-10的分泌,使Th2反应向Th1反应转变,有效减轻小鼠过敏症状和肺组织炎症。结论 纳米聚合物PLA-PEG具有很好的控缓释和保护药物作用,用其包载的过敏原疫苗比传统单一过敏原疫苗进行特异性免疫治疗的效果好、疗效长、副作用小,更适合做过敏原疫苗的佐剂。

目的 To compare the predictive value of fractional exhaled nitric oxide (FeNO) of sputum eosinophilia in chronic cough (CC) and classic asthma (CA). 方法 Patients with CC and CA who referred to the Guangzhou Institute of Respiratory Disease were enrolled from 2010 to 2015. The causes of CC was confirmed according to the results of relevant tests and a successful response to the therapy for the initial diagnosis. Sputum induction and FeNO measurement were performed in all patients. Correlations between FeNO and sputum eosinophils in CC and CA were determined by Spearman rank correlation. Receiver operating characteristics (ROC) curve was constructed by using the values of FeNO.

目的 Miliary tuberculosis is a form of tuberculosis that is characterizedby a wide dissemination into lung fields. So far, no report has shown a miliary tuberculosis case where diffuse miliary nodules can be observed in airways *** we reported a case of military tuberculosis that can easily be misdiagnosed as sarcoidosis. 方法 Report of one case. 结果 Bronchoscopy showed the presence of diffuse miliary nodules in themucosa of bilateral bronchi. Pathological examination revealed non-caseatingnecrotic granuloma. Mycobacterium tuberculosis was not detected. Our case was extremely difficult to differentiate sarcoidosis from tuberculosis. With patient'ssafety in mind, we finally administered anti-tuberculosis therapy and obtainedsatisfactory results.

The effects of ACE2 in ALI have been shown to not only result from its antagonizing hydrolyzing angiotensin Ⅱ (AngⅡ), which is responsible for reduction in the vascular tension and pulmonary accumulation of inflammatory cells, but also result from a role of ACE2 in suppressing the ALI-induced apoptosis of pulmonary endothelial cells (PECs). Nevertheless, the underlying mechanisms of the role of ACE2 on PEC apoptosis are not completely understood.
