目的 Chronic obstructive pulmonary disease(COPD)is characterized by chronic lung inflammation,emphysema and persistentairflow *** stress is a major contributing factor in the pathogenesis of *** assessed the effect of a single3-hour and of a 12 3-hour exposures over 6 weeks to ozone using micro-CT,pulmonary function test and histological analysis.方法 C57/BL6 mice were exposed to ozone or filtered air for single time or over 6 weeks.C57/BL6 mice were exposed to ozone(2.5ppm,3 hours)or filtered air for single time or over 6 ***-Computed Tomography(Micro-CT),lung function test,bronchoalveolar lavage,blood collection,histology and immunohistochemical analysis of lung tissue were performed.

目的 To reveal the relationship between the cytokines and OSAS developed asthma, and evaluate their clinical effect.方法 Data acquired from Polysomnography (PSG) were used to diagnosis Obstructive sleep apnea syndrome (OSAS) according their Pittsburgh Sleep Quality Index (PSQI).The serum samples of OSAS children with and without asthma were collected by double blind *** quantities of cytokine were analyzed by cytokine *** clinical data of the patients were analyzed, and the confounding factors were *** means with statistically significance, which differences was great than 10 times, were *** to significant sequence, the preceding 5 factors were *** in pediatric patients without asthma were followed up to observe the prevalence of asthma in high-level expression with the 5 factors at least 2 years.结果 These study included 716 cases between May, 2013 and December, ***2, VEGF, IL-11, TNF-γ,MCP2 were selected as an early serological marker for Asthma in children with *** compliance rates were greater than 73.1％.结论 There is a certain correlation between OSAS and asthma, which can provide evidence for early intervention through the detection of related cytokines.

目的 Targeted therapy has become a valuable approach in adenocarcinoma of the lung. The number of actionable mutations has been continuously increasing with significant acceleration from discovery to clinical application. 方法 We herein describe a case of innovative treatment using targeted therapy of a 63-year-old metastatic adenocarcinoma female with dual mutation. 结果 It was an interesting case of rapid response to crizotinib in de novo c-met-amplified adenocarcinoma detected by the method of Q-PCR in circulating blood platelets in a patient carrying initially an activating mutation in exon 19 of the epidermal growth factor receptor (EGFR) detected by the method of CAST-PCR in peripheral blood. The initial treatment was gefitinib which responded favorably for three months. Off-label use of crizotinib, a potent inhibitor of c-met, was additionally prescribed when the second blood test showed a c-met (P.R982G, 5784/17[0.003] amplification. Within 4 weeks of treatment initiation, the tumor, the dependent lymphadenopathy, the malignant pleural effusion and the cerebral metastasis responded with rapid shrinkage.

目的 Asthma-Chronic Obstructive Pulmonary Disease(COPD)overlap syndrome(ACOS)is a new determinate syndrome whoseexact characteristics remain *** objective of this study was to find more difference between ACOS and COPD方法 Data from 65 patients with ACOS and 65 patients with COPD were retrospectively collected and *** basis of thisstudy was to compare the two groups while ruling out differences in age,sex and smoking history结果 Patients with ACOS tended to have earlier ages of onset,longer durations of symptoms,better nutritional status,highersingle-breath diffusing capacity of carbon monoxide(DLCO)％predicted and airway resistance％***％predicted,airway resistance％predicted,and length of hospitalization were the variables most significantly associated with the presence ofACOS in patients with *** correlating airway resistance％predicted value and current ACOS showed an optimal cutoff ofairway resistance％predicted of over *** follow-up,16 patient deaths were recorded(3 patients with ACOS).The riskremained significantly higher in patients with COPD alone than in patients with ACOS.

目的 To confirm the role of PI3K/AKT/mTOR protein synthesis pathway in skeletal muscle atrophy induced by chronic obstructivepulmonary disease(COPD),and to explore the possible targets in the signaling pathway inhibiting muscle atrophy.方法 Rats exposed to chronic cigarette smoke(CS)were selected for our study,divided into two exposure groups sham-exposedand 12 weeks *** RNA and protein were extracted from extensor digitorum longus muscle(EDLM)for Real-time PCR andWestern blot analysis to assess the MHC *** proteins expression associated with PI3K/AKT/mTOR signaling pathway(PI3K,Akt,mTOR,GSK-3β,p70S6K1,and 4EBP1)were assessed by Western blot analysis.2％FBS induces L6 cells to ***-1(PI3K agonist)and 10％CSE exposed L6 myoblasts for 24 hours,then detect the expression of MHC,and the proteinsassociated with PI3K/AKT/mTOR signaling pathway.

背景：1933年在1例多发性骨髓瘤患者血清中首次发现一种在低温发生沉淀,37℃左右又溶解的蛋白质,这种蛋白质在1947年被命名为冷球蛋白.冷球蛋白是一组存在于人血清中的特殊免疫球蛋白,冷球蛋白血症是指在血清中存在冷球蛋白. 病历资料：患者75岁已婚男性,既往有慢性阻塞性肺疾病病史10余年,此次因再发咳嗽、咳痰加重10天于入院.患者除慢性咳嗽、咳痰并反复住院的病史外,有气温变冷后全身皮肤青紫、皮下血管显现、关节疼痛病史7年,6年前住院期间发现多浆膜腔积液,并发现静脉采至采血管的血液很快凝结形成团块;之后多次住院均发现多浆膜腔积液,血液遇冷形成团块、沉淀.查体：锁骨上及颌下分别可及黄豆大小的淋巴结,质韧,移动度大,胸部听诊呼吸音低,双肺野可闻及少量细湿罗音,腹部移动性浊音阳性,双下肢轻度水肿,双下肢皮肤可见皮下瘀斑.主要检验结果：补体C4C3明显下降,抗人球蛋白试验(Coomb's试验)阳性,冷凝集;冷球蛋白定性实验阳性,未定量.骨髓流式细胞学检查：P7区细胞占14.4％,表型考虑为克隆性B细胞;P3区细胞占11.4％,表型考虑为克隆性B细胞可能性大;尿本周蛋白阳性;血清免疫固相电泳：ELP上有一条M蛋白带,与抗IgM和抗L形成特异性反应沉淀带,与IgD、IgE均未形成特异性反应沉淀带;尿本周氏蛋白电泳：ELP上有一条M蛋白带,与抗IgM和抗L形成特异性反应沉淀带.影像学资料：胸腹部CT平扫/增强报告：1.双侧颈部、腋窝、纵隔及腹膜后多发淋巴结：性质待定;2.大网膜改变：意义？3.脾大;4.腹水,肝内多发小囊肿;5.双上肺纤维化病变;6.双侧胸腔积液,肺气肿.胸水和腹水常规、生化均提示为漏出液.
