目的 Patients with secondary malignant tracheal and main bronchial tumors may suffer severe symptoms as a result of major airway *** surgical resection is usually not suitable because of the presence of metastatic disease and poor performance status.方法 the use of bronchoscopic cryosurgery to reopen the airway is *** clinical records of 37 patients from December 2001 to January 2013 were retrospectively *** temperature cryotherapy(？50℃to？70℃)was delivered to the central part of the tumor by cryoprobe for four to six minutes causing destruction of the tumor mass(Cryo-melt method).Subsequently,the edge of tumor was frozen for 30 seconds to two minutes,followed by piecemeal removal of the frozen tumor tissue(Cryo-resection method).The endpoints of the study were degree of symptomatic improvement and survival.结果 The rates of dramatic and partial symptomatic alleviation were 57.1％and 28.6％,respectively,there were no intraoperative deaths,and median survival was 16.0 *** survival was significantly correlated to age(under 60 years of age 22.2％*** 60100％,P=0.011),tumor location(main bronchi 0％*** 77.8％,P=0.003),and cryorecanalization times(once 33.3％*** or more 80.0％,P=0.037).结论 Bronchoscopic cryorecanalization is a safe,effective,non-invasive choice for improving the symptoms of malignant airway *** addition to achieving local-regional control,the technique may also contribute to improved survival.

目的 Ankylosing spondylitis is a chronic inflammatory condition that usually affects young men. It is common to see series of related pulmonary manifestations,even in early stage. But the diagnosis of AS is often delayed for asymptomatic or latent features. 方法 A 34 years old male patient was admitted to our hospitial with chief complaint of “cough for 2 years, recurrent purulent sputum for 1 year,hemoptysis for 4months ,recurred for 1week”.16 years ago he was diagnosed as lung tuberculosis and received anti-tuberculosis drugs for 8 months. Within the period of treatment, his right hip ached intermittently, with possible relationship with climate or hard work, and symptom alleviated gradually without any treatment. The patient experienced 3 times of thoracic surgery In 2005, 2006, 2009 because of 3 episodes of acute pneumothorax ( including the right chest surgery, left chest thoracoscope surgery, thoracic close drainage),Physical examination showed extremly emaciation. multiple surgical scars on chest *** lung percussion showed hyperresonance, auscultation showed roughbreath sounds without wheezes or *** forward bend, extend, ratation seemed slightly stiff, lateral normal.

目的 In recent years,violence against doctors in China highlights a widespread concern around the world.[1 2] However,violence against medical professionals seems to be common in many countries.[3] Cross-national comparison can shed light on this *** is not unique in China In Australia,58％of general physicians(GPs)encountered verbal abuse and 18％of them encountered property damage,such as *** of the GPs encountered physical violence,stalking,sexual harassment,at a rate of 6％,4％,6％and 0.1％respectively.[4] About half of doctors in the UK have experienced workplace violence,compared that a third of doctors in China.[3] However,Chinese doctors often experience tremendous life-threatening violence,.Furthermore,attacks are particularly occurred in emergency department,outpatient department and psychiatry throughout many countries.[5] In these sectors,patients have to endure a long wait,during which their anxieties,concerns and unrealistic expectations accumulate constantly.[3] In Australia,patients usually have to wait for 6 weeks at home before they get medical service.[4] This seems to be more serious in China,where patients often sleep in the corridor of hospital before making appointments with doctors.

目的 To describe a case of human infection with avian influenza A(H7N9)virus who was complicated with severe acuterespiratory distress syndrome(ARDS),pulmonary bullae and refractory pneumothorax.方法 Case report结果 A 28-year-old man was admitted to our respiratory intensive care unit(RICU)with a 3-day history of hyperpyrexia,hemosputum and shortness of *** acute respiratory distress syndrome progressively developed and the patient wasoxygenated via veno-venous(VV)ECMO due to the failure of invasive mechanical ***,a right refractorypneumothorax *** the patient was treated with bilateral pleural drainage and select bronchial occlusion(SBO),hestill failed to ***,successful combination of ECMO with VATS of pulmonary bullae resection was performed andpneumothorax was *** was removed 7 days after the *** 3 days after the removal of ECMO,the patient'scondition deteriorated progressively again with a rapid decline in lung *** developed refractory hypoxemia andmulti-organ failure due to progressive lung fibrosis and died at last.

目的 To investigate the effect of Ang(1-7) on smoking-related lung fibrosis by the regulation of autophagy and ROS. 方法 In vivo, Ang-(1-7) was constantly infused into passive smoking rats for 8 *** vitro, primary lung fibroblasts were pretreated with antioxidant or NOX4 siRNA, LC3B siRNA before exposure to ***-mRFP-LC3 advenovirus was introduced to evaluate the autophagic flux in cells. 结果 In vivo,Ang-(1-7) improved lung fibrosis,reduced H2O2 concentration,protein levels of NOX4 and the impaired autophagy induced by smoking *** vitro, CSE treatment elevated NOX4 protein expression and ROS production, resulting in the accumulation of impaired autophagosomes in ***3B depletion enhanced CSE-induced collagen *** with antioxidants orNOX4 siRNA inhibited CSE induced insufficient autophagic flux and collagen *** contrast, the action of Ang-(1-7) opposed the effects of CSE. 结论 Ang (1-7) improves smoking-induced pulmonary fibrosis via attenuating the impaired autophagy caused by NOX4-dependent ROS in vivo and in vitro.

目的 To observe the effects of propofol on the inflammatory response in microglia during intermittent hypoxia (IH), as a result of obstructive sleep apnoea (OSA).方法 The levels of nuclear factor- кB (NF-кB) inhibitor (IкB) and activated p38 mitogen-activated protein kinase (MAPK) exposed to IH with or without propofol treatment were detected by Western *** viability of cells exposed to various concentrations (0, 25, 50, 100, 250 μM) of propofol was monitored with MTT *** protein and mRNA levels of TNF-α and IL-6 were measured by RT-qPCR and ELISA, respectively.结果 IH exposure promoted the activation of NF-кB/p38 MAPK signalling and the secretion of TNF-α and *** was not toxic to *** with the control group, propofol attenuated the IH-induced activation of NF-кB and p38 MAPK, which accompanied with reduction of pro-inflammatory cytokine secretion.结论 Propofol down-regulated the IH-induced secretion of pro-inflammatory cytokine, and inhibit inflammatory responses in *** effect might be based on attenuation of the p38 MAPK and NF-кB siglnalling *** could contribute to alleviating IH-induced inflammatory response in OSA by inhibiting p38 MAPK and NF-кB mediated inflammation in microglia.
