The 15th annual PhysioNet/CinC Challenge aims to encourage the exploration of robust methods for locating heart beats in continuous long-term data from bedside monitors and similar devices that record not only ECG but usually other physiologic signals as well, including pulsatile signals that directly reflect cardiac activity, and other signals that may have few or no observable markers of heart beats. Our goal is to accelerate development of open-source research tools that can reliably, efficiently, and automatically analyze data such as that contained in the MIMIC 11 Waveform Database, making use of all relevant information. Data for this Challenge are 10-minute (or occasionally shorter) excerpts ("records") of longer multi-parameter recordings of human adults, including patients with a wide range of problems as well as healthy volunteers. Each record contains four to eight signals;the first is an ECG signal in each case, but the others are a variety of simultaneously recorded physiologic signals that may be useful for robust beat detection. We prepared and posted 100 training records, and retained 300 hidden test records for evaluation of Challenge entries. A total of 1,332 entries from 60 teams were processed during the challenge period.

Information extracted from heart sound signals are associated with valvular heart diseases and other cardiovascular disorders. This study aims to develop a computational framework for the classification of a given heart sound recording. Different techniques have their respective superiority in classifying heart sound recordings with various patterns, and it is difficult to find one technique that outperforms all the others. We hence propose a multi-modal classifier by fusing the classification results from various techniques based on various features. Using the data obtained from the 2016 PhysioNet/CinC Challenge, we generate two different feature sets: one set was calculated from segmented results by peak-finding method, and the other set was extracted by audio signal analysis. We then assess the performance of two classification techniques -support vector machines (SVMs) and extreme learning machines (ELMs) -by feeding them with the best subset of features selected from these two feature sets. The final heart sound classification result (normal / abnormal) is determined by ensembling the two classifiers with voting. The best performance out of five online entries achieved an overall score of 0.83 with sensitivity=0.70 and specificity=0.96.

Aims: This study proposes a cardiac diagnostic model using convolution neural network (CNN). This model can predict whether a heart sound recording is normal or not by classifying phonocardiograms (PCGs) from both clinical and nonclinical environments - in accordance with the "2016 Physionet/CinCChallenge". Methods: Heart sound recordings in the training data set are filtered by using Windowed-sinc Hamming filter algorithm to remove signals regarded as noise. The filtered recordings are then scaled and segmented. Using the filtered and segmented recordings, CNN is trained to extract features and construct a classification function. The CNN is trained by back propagation algorithm with stochastic gradient descent and mini-batch learning. To classify one sound recording, the signal should be filtered and segmented. Each segment of the signal is then classified by the trained CNN model. The model assigns each segment signal a relative probability between normal and abnormal labels. By accumulating these relative probability values for all the segmented signals, one can reliably and robustly determine whether the target signal is normal or abnormal. Results: The proposed model achieved an overall score of 79.5 with a sensitivity of 70.8 and a specificity of 88.2.

The rotor ablation has gained favor in recent years to treat chronic atrial fibrillation (CAF). However, it may cause complications in patients and the results remain suboptimal. Besides, antiarrhythmic drugs for the management of AF are not sufficiently effective and can cause significant cardiac and extracardiac side effects. To reduce the risk and increase the effectiveness, a rotor modulation via the localized release of dofetilide is proposed through an in silico study. A computational model of two-dimensional atrial tissue was implemented using human cell model under CAF conditions. Dofetilide was modeled blocking the ionic currents I-Kr and I-KACh using Hill's equation. A CAF episode, sustained by a rotor, was simulated. Different concentrations of dofetilide were applied in specific distributions based on the rotor tip location, detected by phase map analysis. Non-localized application of dofetilide resulted in the benefit of CAF progression. In contrast, localized dofetilide liberation terminated the CAF episode. These results could have therapeutic implications in novel treatments of CAF.

In the paper, we present a fully automated real-time multi-lead ST-segment monitoring algorithm. For a representative normal beat in each ST measurement interval, the ECG leads with low signal quality are excluded and the remaining leads are used in a multi-lead waveform-length transformation to form a length signal for Q-onset (Q) and J-point (J) determination. From Q, the isoelectric point is determined and used with the J to measure the ST-segment at J or J plus an offset for all available leads. A development set of 158 records and a test set of 60 records with cardiologists' beat-by-beat Q and J annotations were used to develop and evaluate the Q and J detection. The ESC ST-T Database and a 60-patient annotated 12-lead PTCA dataset were used to evaluate the algorithm's ST performance. Detailed statistical results are given in the paper. The test results demonstrate that the described ST-segment monitoring algorithm is effective and reliable.

This study examines the effect of missing body surface potentials on inverse solutions, and determines if linear interpolation can regain information lost. Using simulated and experimental data, electrograms (EGMs) were reconstructed after 1) removing and 2) interpolating 'broken' signals. Results showed that torso potentials missing over the chest can reduce inverse reconstruction accuracy. Linear interpolation did not improve, and could further deteriorate reconstruction accuracy.

Sudden cardiac death (SCD) from ventricular arrhythmias is a leading cause of mortality. Accurate arrhythmic risk stratification is vital for preventative clinical interventions. Ejection fraction (EF) is the primary metric used, but its accuracy is under debate, as many SCD cases exhibit preserved EF. Thus, identifying clear links between EF and arrhythmic risk is critical. Here, as a step forward, we investigate the ionic processes determining cellular pro-arrhythmic mechanisms and their relationship with active tension. A population of 2500 human ventricular electromechanical cellular models was created, and stimulated to produce pro-arrhythmic behaviour. We quantified their susceptibility to develop early afterdepolarizations (EADs) and action potential duration (APD) shortening, as key arrhythmic markers. The relationship between both arrhythmic markers and tension amplitude was found to be highly dependent on ionic mechanism. Variability in L-type calcium current was the primary determinant of active tension and arrhythmia susceptibility, alongside SERCA and hERG expression. Models with low tension could exhibit both high and low EAD susceptibility. APD shortening, however, displayed a weak positive correlation with active tension amplitude.

Heart sounds can be obtained in various ways. A recent modality is airborne ulfrasound echo Doppler, which has been used either in a pulsed wave mode or a continuous wave mode. In the present work we analyse the continuous wave ulfrasound echo from the chest. In 10 subjects a piezo ultrasound transceiver was used to transmit a 40 kHz acoustic sine wave directed to the chest wall. The reflected signal (echo) was recorded with a microphone and sampled at 1 MHz. The FM modulated echo was demodulated in MatLab using a quadrature signal. The signal thus obtained represented the chest displacement, which was subdivided into three frequency bands: respiration (below 0.5 Hz), movement of the myocardium (>1 Hz) and valve sounds (30-100 Hz). Above 100 Hz the signal decayed by 20 dB/decade with a noise floor above 1-2 kHz. Interestingly, signals obtained directly from the skin and signals obtained through a thin layer of clothes were similar. Non-contact continuous-wave echo Doppler thus is a promising new modality for the recording of heart sounds, although its true potential has to be further investigated.

Many approaches have been proposed for automatic classification of different pathological events in ECG signals. Present study is focused on analysis and classification of non-ischemic, moderate and severe ischemic, and VPB segments in data obtained in rabbit isolated hearts. It is shown, that use of low number of morphological parameters calculated from electrograms combined with even simple classification method allows achieving of accurate results (with overall accuracy up to 0.99) for four types of the segments.

The aim of this study was to assess the recent publication of new Peguero/Lo Presti (P-LP) ECG criteria for left ventricular hypertrophy (LVH) which purported to have a higher sensitivity than commonly used existing criteria but with no significant loss of specificity (>= 90%) compared to the other LVH criteria.. ECGs from 1496 apparently healthy adults from the Glasgow normals ECG database were used to assess specificity. The new P-LP criteria (maximum S amplitude in any lead plus S amplitude in V4) were assessed together with the long established Sokolow-Lyon and Cornell criteria (the latter being sex based) for LVH. The Glasgow criteria were also assessed. The specificity of the new P-LP criteria were found to be exceptionally poor in younger persons, being 50% and 76% in males and females < 30 years respectively increasing to 83% and 89% respectively in the group >= 50 years. The Cornell criterion specificity exceeded 90% at all ages in both sexes. New diagnostic criteria for LVH must be age and sex dependent and ideally should undergo a thorough evaluation of specificity on a large database of apparently healthy individuals from both sexes aged 18 years and over before publication.
