In the past ten years, numerous new surgical and interventional techniques have been developed for treating heart valve disease without the need for cardiopulmonary bypass. Heart valve repair is now being performed in a blood-filled environment, reinforcing the need for accurate and intuitive imaging techniques. Previous work has demonstrated how augmenting ultrasound with virtual representations of specific anatomical landmarks can greatly simplify interventional navigation challenges and increase patient safety. These techniques often complicate interventions by requiring additional steps taken to manually define and initialize virtual models. Furthermore, overlaying virtual elements into real-time image data can also obstruct the view of salient image information. To address these limitations, a system was developed that uses real-time volumetric ultrasound alongside magnetically tracked tools presented in an augmented virtuality environment to provide a streamlined navigation guidance platform. In phantom studies simulating a beating-heart navigation task, procedure duration and tool path metrics have achieved comparable performance to previous work in augmented virtuality techniques, and considerable improvement over standard of care ultrasound guidance.

image registration of preprocedural contrast-enhanced CTs to intraprocedual cone-beam computed tomography (CBCT) can provide additional information for interventional liver oncology procedures such as transcatheter arterial chemoembolisation (TACE). In this paper, a novel similarity metric for gradient-based image registration is proposed. The metric relies on the patch-based computation of histograms of oriented gradients (HOG) building the basis for a feature descriptor. The metric was implemented in a framework for rigid 3D-3D-registration of pre-interventional CT with intra-interventional CBCT data obtained during the workflow of a TACE. To evaluate the performance of the new metric, the capture range was estimated based on the calculation of the mean target registration error and compared to the results obtained with a normalized cross correlation metric. The results show that 3D HOG feature descriptors are suitable as image-similarity metric and that the novel metric can compete with established methods in terms of registration accuracy.

3D reconstruction of vessels from 2D X-ray angiography is highly relevant to improve the visualization and the assessment of vascular structures such as pulmonary arteries by interventional cardiologists. However, to ensure a robust and accurate reconstruction, C-arm gantry parameters must be properly calibrated to provide clinically acceptable results. Calibration procedures often rely on calibration objects and complex protocol which is not adapted to an intervention context. In this study, a novel calibration algorithm for C-arm gantry is presented using the instrumentation such as catheters and guide wire. This ensures the availability of a minimum set of correspondences and implies minimal changes to the clinical workflow. The method was evaluated on simulated data and on retrospective patient datasets. Experimental results on simulated datasets demonstrate a calibration that allows a 3D reconstruction of the guide wire up to a geometric transformation. Experiments with patients datasets show a significant decrease of the retro projection error to 0.17 mm 2D RMS. Consequently, such procedure might contribute to identify any calibration drift during the intervention.

Dartmouth and Medtronic Navigation have established an academic-industrial partnership to develop, validate, and evaluate a multi-modality neurosurgical image-guidance platform for brain tumor resection surgery that is capable of updating the spatial relationships between preoperative images and the current surgical field. A stereovision system has been developed and optimized for intraoperative use through integration with a surgical microscope and an image-guided surgery system. The microscope optics and stereovision CCD sensors are localized relative to the surgical field using optical tracking and can efficiently acquire stereo image pairs from which a localized 3D profile of the exposed surface is reconstructed. This paper reports the first demonstration of intraoperative acquisition, reconstruction and visualization of 3D stereovision surface data in the context of an industry-standard image-guided surgery system. The integrated system is capable of computing and presenting a stereovision-based update of the exposed cortical surface in less than one minute. Alternative methods for visualization of high-resolution, texture-mapped stereovision surface data are also investigated with the objective of determining the technical feasibility of direct incorporation of intraoperative stereo imaging into future iterations of Medtronic's navigation platform.

Surgical simulators are powerful tools that assist in providing advanced training for complex craniofacial surgical procedures and objective skills assessment such as the ones needed to perform Bilateral Sagittal Split Osteotomy (BSSO). One of the crucial steps in simulating BSSO is accurately cutting the mandible in a specific area of the jaw, where surgeons rely on high fidelity visual and haptic cues. In this paper, we present methods to simulate drilling and cutting of the bone using the burr and the motorized oscillating saw respectively. Our method allows low computational cost bone drilling or cutting while providing high fidelity haptic feedback that is suitable for real-time virtual surgery simulation.

Automatic segmentation of the prostate in magnetic resonance imaging (MRI) has many applications in prostate cancer diagnosis and therapy. We propose a deep fully convolutional neural network (CNN) to segment the prostate automatically. Our deep CNN model is trained end-to-end in a single learning stage based on prostate MR images and the corresponding ground truths, and learns to make inference for pixel-wise segmentation. Experiments were performed on our in-house data set, which contains prostate MR images of 20 patients. The proposed CNN model obtained a mean Dice similarity coefficient of 85.3%+/- 3.2% as compared to the manual segmentation. Experimental results show that our deep CNN model could yield satisfactory segmentation of the prostate.

Electromagnetic (EM) tracking has been recognized as a valuable tool for locating the interventional devices in procedures such as lung and liver biopsy or ablation. The advantage of this technology is its real-time connection to the 3D volumetric roadmap, i.e. CT, of a patient's anatomy while the intervention is performed. EM-based guidance requires tracking of the tip of the interventional device, transforming the location of the device onto pre-operative CT images, and superimposing the device in the 3D images to assist physicians to complete the procedure more effectively. A key requirement of this data integration is to find automatically the mapping between EM and CT coordinate systems. Thus, skin fiducial sensors are attached to patients before acquiring the pre-operative CTs. Then, those sensors can be recognized in both CT and EM coordinate systems and used calculate the transformation matrix. In this paper, to enable the EM-based navigation workflow and reduce procedural preparation time, an automatic fiducial detection method is proposed to obtain the centroids of the sensors from the pre-operative CT. The approach has been applied to 13 rabbit datasets derived from an animal study and eight human images from an observation study. The numerical results show that it is a reliable and efficient method for use in EM-guided application.

Angiographic evaluation is important in embolization procedures to ensure a proper amount of embolic material is delivered to target vessels. Simulation of image-guided interventions with phantoms enables scientists and residents to better understand techniques to avoid procedure-related complications. In this study, a phantom with an exchangeable, single-use tumour model was designed and fabricated using 3D-printing in polylactic acid plastic. A 1.5 cm sphere made of gyroids with varying pore sizes was used in place of tubular structures to simulate tumour microvasculature and thereby provide restrictive passages that trap embolic material. Hemodynamic blood properties were replicated using a pulsatile flow pump containing a glycerol-water mixture and flow resistance was mimicked by adding tubing to each outlet vessel. Tissue-mimicking material was introduced around the phantom to provide radiographic path equalization. The modular phantom was used to mimic fluoroscopically guided embolization procedures in three tumour models, culminating in reflux observed in the feeding artery. Angiography revealed a tumour blush created by the gyroid patterns. Embolization of the tumour model proceeded gradually with contrast agent accumulating in the tumour over time where reflux developed after injecting 5.4 +/- 2.4 mL of a microsphere/contrast mixture. The pulsatility of the blood-mimicking fluid was clearly seen when contrast agent was injected. Digital subtraction angiography after embolization revealed complete stasis of antegrade flow through the tumour model. The 3D printed modular phantom design facilitates the reusability of the liver model following embolization, allowing for a more efficient and versatile tool for embolization training and research applications.

Ultrasound calibration allows for ultrasound images to be incorporated into a variety of interventional applications. Traditional Z-bar calibration procedures rely on wired phantoms with an a priori known geometry. The line fiducials produce small, localized echoes which are then segmented from an array of ultrasound images from different tracked probe positions. In conventional B-mode ultrasound, the wires at greater depths appear blurred and are difficult to segment accurately, limiting the accuracy of ultrasound calibration. This paper presents a novel ultrasound calibration procedure that takes advantage of synthetic aperture imaging to reconstruct high resolution ultrasound images at arbitrary depths. In these images, line fiducials are much more readily and accurately segmented, leading to decreased calibration error. The proposed calibration technique is compared to one based on B-mode ultrasound. The fiducial localization error was improved from 0.21mm in conventional B-mode images to 0.15mm in synthetic aperture images corresponding to an improvement of 29%. This resulted in an overall reduction of calibration error from a target registration error of 2.00mm to 1.78mm, an improvement of 11%. Synthetic aperture images display greatly improved segmentation capabilities due to their improved resolution and interpretability resulting in improved calibration.

Based on the Iterative Closest Point (ICP) framework, we present a generalized solution for the registration between homologous points and lines. The transformation we seek comprises an anisotropic scaling, followed by rotation and translation. This algorithm is demonstrated using the Perspective-n-Point (PnP) problem where lines form a bundle, and the Non-Perspective-n-Point (NPnP) problem where each line potentially has its own origin. We also prove that one existing NPnP solution is, in fact, equivalent to ICP, and that a second PnP solution differs from ICP only in the iteratively estimated translation. Applications for these types of registration include ultrasound calibration, kinematics tracking under fluoroscopic video, and camera pose estimation. Simulation results suggest this ICP algorithm compares favorably to existing PnP and NPnP algorithms, and has an extremely compact formulation.
