clinical decisions for improving motor function in patients both with disability as well as improving an athlete's performance are made through clinical and movement analysis. Currently, this analysis facilitates identifying abnormalities in a patient's motor function for a large amount of neuro-musculoskeletal pathologies. However definitively identifying the underlying cause or long-term consequences of a specific abnormality in the patient's movement pattern is difficult since this requires information from multiple sources and formats across different times and currently relies on the experience and intuition of the expert clinician. In addition, this data must be persistent for longitudinal outcomes studies. Therefore a multimedia ePR system integrating imaging informatics data could have a significant impact on decision support within this clinical workflow. We present the design and architecture of such an ePR system as well as the data types that need integration in order to develop relevant decision support tools. Specifically, we will present two data model examples: 1) A performance improvement project involving volleyball athletes and 2) Wheelchair propulsion evaluation of patients with disabilities. The end result is a new frontier area of imaging informatics research within rehabilitation engineering and biomechanics.

There are varieties of health-care evaluation either for the quality of design or service, as well as the impact of those factors to the patients or management system. Usability is one of the evaluations, which evaluate the quality in-use. Usability approach is focusing on the method of collecting data. It is based on the exploration of the user experience by knowing their perspective. It is measured by three parameters;the effectiveness and efficiency of the design or facilities offered and the user's satisfaction. The purpose of this study is to explore an understanding of the complexity of patients who use the facilities provided. In addition it also to observe the phenomenon of patient feedback during journey experience in the hospital spatial design and it relationship. The methods of collecting data in this study are through walkthrough observation and interviewing patients from two public hospitals. As a result, we identified that a variety of usability criteria contributes to the quality of architecture in-use. Most of the respondents expressed the usability factors are caused by accessibility, reach ability and way finding criteria. This paper describes the design and implantation issues for a Picture Archiving and Communications System (pacs) in a 60-bed provincial hospital. Considerations such as the state of the imaging equipment, preventive maintenance, use and misuse of the equipment and the difficulties regarding migration to an all-digital imaging unit are discussed, as well as the criteria for the selection of the imaging modalities. Finally a design is proposed with four imaging modalities that use open-source database and display workstations. The system is currently under evaluation for possible reproduction in as many as 60 small clinics and hospitals throughout the country.

Application of predictive biosimulation within pharmaceutical clinical development: examples of significance for translational medicine and clinical trial design

The challenge of accurately predicting human clinical outcome based on preclinical data has led to a high failure rate of compounds in human clinical trials. A series of methods are described by which biosimulation can address these challenges and guide the design and evaluation of experimental and clinical protocols. Early compound development often proceeds on the basis of preclinical data from animal models. The systematic evaluation possible in a simulation can assist in the critical step of translating the preclinical outcomes to human physiology. Later in the process, clinical trials definitively establish a therapy's beneficial effects, as well as any adverse side effects. Biosimulation allows for the optimal design of clinical trials to ensure that key issues are addressed effectively and efficiently, and in doing so, improves the success rate of the trials.

The International conference on Harmonization (ICH) has recently developed the ICH guidelines, which are the common guidelines for regulatory requirements in the ICH regions, i.e., European Union, Japan, and the United States of America. The guidelines have been successively implemented in the ICH regions, and the people involved in drug evaluation in the pharmaceutical industry, such as statisticians and pharmaceutical scientists, have referred to the guidelines during their daily work. Probably one of the most influential ICH guidelines on the drug development process in the ICH regions, especially in Japan, is the ICH-E5 guideline, which is encouraging the pharmaceutical industry to use the mountain of foreign clinical data for the registration of a new drug. In this article, we review the issues concerning the ICH-E5 guideline discussed so far, and discuss some practical issues in applying the guideline, especially the design of the study conducted in a new region. This is called a "bridging study." In addition, we introduce some statistical approaches for evaluating the acceptability of foreign clinical data.

A single point of hardware failure in pacs is at the pacs controller, or the main archive server. When it occurs, it renders the entire pacs inoperable and crippled until the problem is diagnosed and resolved. Many current pacs do not have a fault-tolerant design or adequate back-up system for the main archive server due to several issues including cost. Several large scale pacs utilize the Tandem or cluster design but are very costly and have not been critically tested for their degree of fault tolerance. This paper describes a novel, portable, and scalable fault-tolerant pacs controller design that is affordable for most pacs implementations. Currently, most pacs controllers are based on UNIX servers. The UNIX server can be replaced by a specially designed Continuous Availability Server (CAS) consisting of three identical machines as the UNIX server that run the same operating system and application software simultaneously and independently. Any operation entering the CAS is transmitted to all three machines. A "voter" scheme is used to determine the condition of each machine. The operation is then executed step-by-step in the CAS with the majority vote. In this way, the CAS guarantees no interruption of data flow or data loss in the event of a hardware failure, and hence the term continuous availability. Hardware failure of any one or multiple components in a machine in the CAS including the power supply, processors, SCSI ports, network ports, RAID controller, and disk controllers was simulated. A series of clinical scenarios were performed while executing a simulated failure of the key hardware components within the CAS. No interruption of pacs data flow passing through the CAS was observed. It is possible to replace the UNIX server in the pacs controller by the CAS. The result is a continuous availability server immune to a single point of hardware failure. Although the current hardware cost of the CAS in the laboratory prototype is almost three times the cost
