Surgical tool segmentation is becoming imperative to provide detailed information during intra-operative execution. These tools can obscure surgeons' dexterity control due to narrow working space and visual field-of-view, which increases the risk of complications resulting from tissue injuries (e.g. tissue scars and tears). This paper demonstrates a novel application of segmenting and removing surgical instruments from laparoscopic/endoscopic video using digital inpainting algorithms. To segment the surgical instruments, we use a modified U-Net architecture (U-NetPlus) composed of a pre-trained VGG11 or VGG16 encoder and redesigned decoder. The decoder is modified by replacing the transposed convolution operation with an up-sampling operation based on nearest-neighbor (NN) interpolation. This modification removes the artifacts generated by the transposed convolution, and, furthermore, these new interpolation weights require no learning for the upsampling operation. The tool removal algorithms use the tool segmentation mask and either the instrument-free reference frames or previous instrument-containing frames to fill-in (i.e., inpaint) the instrument segmentation mask with the background tissue underneath. We have demonstrated the performance of the proposed surgical tool segmentation/removal algorithms on a robotic instrument dataset from the MICCAI 2015 EndoVis Challenge. We also showed successful performance of the tool removal algorithm from synthetically generated surgical instruments-containing videos obtained by embedding a moving surgical tool into surgical tool-free videos. Our application successfully segments and removes the surgical tool to unveil the background tissue view otherwise obstructed by the tool, producing visually comparable results to the ground truth.

Nuclear imaging is a commonly used tool in today's diagnostics and therapy planning. For interventional use however it suffers from drawbacks which limit its application. Freehand SPECT was developed to overcome these limitations and to provide 3D functional imaging during an intervention. It combines a nuclear probe with an optical tracking system to obtain its position and orientation in space synchronized with its reading. This information can be used to compute a 3D tomographic reconstruction of an activity distribution. However, as there is no fixed geometry the system matrix has to be computed on-the-fly, using ad-hoc models of the detection process. One solution for such a model is a reference look up table of previously acquired measurements of a single source at different angles and distances. In this work two look up tables with a one and four millimeter step size between the entries were acquired. Twelve datasets of a phantom with two hollow spheres filled with a solution of Tc99wm were acquired with the Freehand SPECT system. Reconstructions with the look up tables and two analytical models currently in use were performed with these datasets and compared with each other. The finely sampled look up table achieved the qualitatively best reconstructions, while one of the analytical models showed the best positional accuracy.

Percutaneous treatment of scaphoid fractures has found increasing interest in recent years as it promises to minimize soft-tissue damage, and minimizes the risk of infections and the loss of the joint stability. However, as this procedure is mostly performed on 2D fluoroscopic images, the accurate localization of the scaphoid bone for fracture fixation renders extremely challenging. In this work, we thus propose the integration of a statistical wrist model with 3D intraoperative ultrasound for accurate localization of the scaphoid bone. We utilize a previously developed statistical wrist model and register it to bone surfaces in ultrasound images using a probabilistic approach that involves expectation-maximization. We utilize local phase symmetry to detect features in noisy ultrasound images;in addition, we use shadow information in ultrasound images to enhance and set apart bone from other features. Feasibility experiments are performed by registering the wrist model to 3D ultrasound volumes of two different wrists at two different wrist positions. And the result indicates a potential of the proposed technique for localization of the scaphoid bone in ultrasound images.

Physical models of patient anatomy have been used increasingly as 3D printing technologies have become mainstream. Such models can be used for both the validation of new minimally invasive surgical techniques, as well as surgical rehearsal and training. However, current workflows for creating flexible models with complex anatomy rely on the use of expensive 3D printing techniques. We present a mold design with which we create patient-specific physical models using low-cost techniques and materials. This generic mold makes it possible to create physical models with multiple components and complex internal structures including tumours, vasculature and other anatomic components with accuracy. To demonstrate this, we have created kidney models derived from the CT of excised porcine kidneys, including vasculature and an artificial tumor. We created the models in two parts, first using a rigid positive model to create a negative mold, and then creating a silicone model with the 3D printed vasculature inside, and removing it to leave wall-less vessels. The vasculature models include at least six separate bifurcations with minimum lumen diameters of approximately 1mm. The mean Euclidean offset distance between the model and original vessels was 0.42 mm, with a standard deviation of 0.50 mm. Both generic and patient specific models can be built with this workflow.

Intraoperative brain deformation can significantly degrade the accuracy of image guidance using preoperative MR images (pMR). To compensate for brain deformation, biomechanical models have been used to assimilate intraoperative displacement data, compute whole-brain deformation field, and to produce updated MR images (uMR). Stereovision (SV) is an important technique to capture both geometry and texture information of exposed cortical surface at the craniotomy, from which surface displacement data (known as sparse data) can be extracted by registering with pMR to drive the computational model. Approaches that solely utilize geometrical information (e.g., closest point distance (CPD) and iterative closest point (ICP) method) do not seem to capture surface deformation accurately especially when significant lateral shift occurs. In this study, we have developed a texture intensity-based method to register cortical surface reconstructed from stereovision after dural opening with pMR to extract 3D sparse data. First, a texture map is created from pMR using surface geometry before dural opening. Second, a mutual information (MI)-based registration was performed between the texture map and the corresponding stereo image after dural opening to capture the global lateral shift. A block-matching algorithm was then executed to differentiate local displacements in smaller patches. The global and local displacements were finally combined and transformed in 3D following stereopsis. We demonstrate the application of the proposed method with a clinical patient case, and show that the accuracy of the technique is 1-2 mm in terms of model-data misfit with a computation time <10 min.

The quality of brain tumor resection surgery is dependent on the spatial agreement between preoperative image and intraoperative anatomy. However, brain shift compromises the aforementioned alignment. Currently, the clinical standard to monitor brain shift is intraoperative magnetic resonance (iMR). While iMR provides better understanding of brain shift, its cost and encumbrance is a consideration for medical centers. Hence, we are developing a model-based method that can be a complementary technology to address brain shift in standard resections, with resource-intensive cases as referrals for iMR facilities. Our strategy constructs a deformation 'atlas' containing potential deformation solutions derived from a biomechanical model that account for variables such as cerebrospinal fluid drainage and mannitol effects. Volumetric deformation is estimated with an inverse approach that determines the optimal combinatory 'atlas' solution fit to best match measured surface deformation. Accordingly, preoperative image is updated based on the computed deformation field. This study is the latest development to validate our methodology with iMR. Briefly, preoperative and intraoperative MR images of 2 patients were acquired. Homologous surface points were selected on preoperative and intraoperative scans as measurement of surface deformation and used to drive the inverse problem. To assess the model accuracy, subsurface shift of targets between preoperative and intraoperative states was measured and compared to model prediction. Considering subsurface shift above 3 mm, the proposed strategy provides an average shift correction of 59% across 2 cases. While further improvements in both the model and ability to validate with iMR are desired, the results reported are encouraging.

Laparoscopic surgery is a minimally invasive surgical approach, in which abdominal surgical procedures are performed through trocars via small incisions. Patients benefit by reduced post-operative pain, shortened hospital stays, improved cosmetic results, and faster recovery times. Optimal port placement can improve surgeon dexterity and avoid the need to move the trocars, which would cause unnecessary trauma to the patient. We are building an intuitive open source visualization system to help surgeons identify ports. Our methodology is based on an intuitive port placement visualization module and atlas-based registration algorithm to transfer port locations to individual patients. The methodology follows three steps: 1) Use a port placement visualization module to manually place ports in an abdominal organ atlas. This step generates port-augmented abdominal atlas. This is done only once for a given patient population. 2) Register the atlas data with the patient CT data, to transfer the prescribed ports to the individual patient 3) Review and adjust the transferred port locations using the port placement visualization module. Tool maneuverability and target reachability can be tested using the visualization system. Our methodology would decrease the amount of physician input necessary to optimize port placement for each patient case. In a follow up work, we plan to use the transferred ports as starting point for further optimization of the port locations by formulating a cost function that will take into account factors such as tool dexterity and likelihood of collision between instruments.

This paper presents a methodology for the digital formatting of a printed atlas of the brainstem and the delineation of cranial nerves from this digital atlas. It also describes on-going work on the 3D resampling and refinement of the 2D functional regions and nerve contours. In MRI-based anatomical modeling for neurosurgery planning and simulation, the complexity of the functional anatomy entails a digital atlas approach, rather than less descriptive voxel or surface-based approaches. However, there is an insufficiency of descriptive digital atlases, in particular of the brainstem. Our approach proceeds from a series of numbered, contour-based sketches coinciding with slices of the brainstem featuring both closed and open contours. The closed contours coincide with functionally relevant regions, whereby our objective is to fill in each corresponding label, which is analogous to painting numbered regions in a paint-by-numbers kit. Any open contour typically coincides with a cranial nerve. This 2D phase is needed in order to produce densely labeled regions that can be stacked to produce 3D regions, as well as identifying the embedded paths and outer attachment points of cranial nerves. Cranial nerves are modeled using an explicit contour based technique called 1-Simplex. The relevance of cranial nerves modeling of this project is two-fold: i) this atlas will fill a void left by the brain segmentation communities, as no suitable digital atlas of the brainstem exists, and ii) this atlas is necessary to make explicit the attachment points of major nerves (except I and II) having a cranial origin.
