The global demand for tele-medicine services has increased rapidly, prompting clinics to adopt virtual care solutions. This shift towards remote medical consultations has become especially crucial for minimizing direct patient contact, particularly in urgent medical situations where receiving professional healthcare in a timely manner is essential, such as during episodes of stroke. Importantly, access to specialist care in regional centres (spokes) is often limited and relies on communication with major centres (hubs) for consultation and treatment support. The technical needs for tele-medicine are well defined, however the tools are often inadequate, requiring multiple parallel applications to be used at a time. For consultations, remote clinicians need to access to patient assessment notes, medical records and often imaging in order to provide treatment recommendations, oversee patient care and make transfer recommendations when the patient requires a higher level of care. The implementation of the hub and spoke tele-medicine model has been highly successful in supporting regional healthcare throughout the world, greatly increasing access to specialist care. In a typical regional centre (spokes), there is often no specialist staff, which is strongly associated with poorer clinical outcomes among patients living in regional areas. We have developed a single, scalable platform to support tele-medicine needs patients in the Australian context. The platform enables healthcare providers to rapidly connect, collaborate, and plan effective treatment pathways for patients. It supports real-time clinical assessment notes sharing, live patient tracking with integrated video conferencing and chat capabilities by facilitating real-time communication and coordination among medical professionals. In this paper, we propose a new ultra-wideband (UWB)-based simultaneous localization and wall-mapping (SLAM) system, which adopts two-way ranging optimization on UWB anchor and tag nodes to track the target's real-time movement in an unknown area. The proposed UWB-based SLAM system captures time difference of arrival (TDoA) of the anchor nodes' signals over a line-of-sight propagation path and reflected paths. The real-time location of the UWB tag is estimated according to the real-time TDoA measurements. To minimize the estimation error resulting from background noise in the two-way ranging, a Least Squares Method is implemented to minimize the estimation error for the localization of a static target, while Kalman Filter is applied for the localization of a mobile target. An experimental testbed is built based on off-the-shelf UWB hardware. Experiments validate that a reflector, e.g., a wall, and the UWB tag can be located according to the two-way ranging measurement. The localization accuracy of the proposed SLAM system is also evaluated, where the difference between the estimated location and the ground truth trajectory is less than 1 meter.

Context caching plays an increasingly important role in delivering near real-time responses for context-aware distributed Internet of Things (IoT) applications, services and systems. A context management platform (CMP), a middle-ware which acts as an aggregator and redirector of contextual information to support smart IoT applications, requires adaptive context caching to process and manage enormous amounts of context stemming from IoT. In this work, we propose a novel approach to estimating the context information's demand probability, which helps improve the context retrieval performance of a CMP under near real-time constraints. The proposed approach uses context query logs and applies machine learning algorithms to estimate the context caching probability for context caching. We further use an evolutionary technique for optimising the context caching probability to improve the context retrieval performance of the CMP. We conduct an experimental evaluation using a research prototype CMP, Context-as-a-Service (CoaaS) and show that the proposed technique can significantly improve the context retrieval performance. Analysis of the experimental results showed with context caching probability optimized by evolutionary technique there is an average percentage decrease of 43.68% in the response time of CoaaS.
