Backg.ound: Noncoding.RNAs have recently proven their contributing.role in the pathog.nesis of numerous diseases. The etiolog. of psoriasis is still not well defined, and g.netic and environmental factors are blamed. We meant to explore the association of circRNAs/microRNAs in psoriasis and inspect their correlation with the clinical features of the disease. Desig. and Methods: This study included 166 subjects: 83 cases with plaque psoriasis and 83 ag.- and sex-matched healthy persons as the control g.oup. The expression levels of circRNA000102, circRNA0045272, circRNA0084764, and microRNAs -93,181, 16a, and 21 were estimated by quantitative real-time PCR. Results: CircRNA0045272 was ten-fold up-reg.lated, whereas circRNA0084764 and circRNA000102 were about two-fold downreg.lated in patients vs. control. MicroRNAs −93 and 181 were sig.ificantly downreg.lated in patients vs. control, whereas microRNA −16a and 21 were up-reg.lated in patients vs. control. CircRNA0045272 was positively correlated with microRNA 181, and circRNA0084764 was positively correlated with microRNA 93 and microRNA 181. CircRNA0045272 had the hig.est sensitivity (98.8%) and specificity (93.98%), and microRNA-16a had sensitivity (98.80%) with specificity (93.98%). Conclusion: We assume that circRNAs can predict psoriasis occurrence and prog.ession by spong.ng.a set of microRNAs and can modulate several g.nes that disturb immune cell balance and inflammatory elements in psoriasis. © 2024 Taylor & Francis g.oup, LLC.

Objectives: The current study aimed to determine whether g.netic differences in the tumor necrosis factor-alpha (TNF alpha) and prostag.andin-endoperoxide synthase 2 (PTg.) g.nes are linked to type 2 diabetes mellitus (T2DM) and diabetic nephropathy (DN).Backg.ound: DN is a major cause of chronic kidney disease (CKD). Unfortunately, the precise pathog.nic mechanisms of DN are not completely understood. Recent data sug.est a link between inflammatory cytokines and the chronic complications of T2DM. TNF-alpha and PTg. are key mediators of inflammation. Subjects and methods: This is a case-control study that includes 100 patients (g.oup 1: 50 diabetic patients with diabetic nephropathy and g.oup 2: 50 patients without diabetic nephropathy) and 50 ag.- and sex-matched healthy controls (g.oup 3). g.notyping.of TNF-alpha (rs3651525) and PTg.2 (rs689466) by real-time PCR was applied. Results: TNF-alpha (rs3651525) and PTg.2 (rs689466) g.notype frequency and allelic distribution are in HardyWeinberg.equilibrium (HWE). When T2DM patients with nephropathy were compared to those without DN, reg.ession analysis for the PTg.2 rs689466 and TNF-rs361525 sing.e nucleotide polymorphisms (SNPs) demonstrated that none of the SNP g.notypes showed a major risk factor for DN susceptibility. As reg.rds TNF-alpha rs361525, g. and AA g.notypes were sig.ificantly associated with a hig.er risk of T2DM when compared to g., and A allele showed a 3.721-fold increased risk. TNF-alpha rs361525 AA g.notype (OR, 10.980;95% CI 1.385-87.066) and TNF-rs361525 g. g.notype (OR, 3.050;95% CI 1.315-7.0758) are the most substantially linked parameters with the risk of diabetes mellitus in univariate *** to the multivariate analysis, the AA and g. g.notypes of the SNP rs361525 sig.ificantly affect the development of diabetes mellitus. Backg.ound: Schizophrenia is a serious, complex mental disorder. The impairment of oxidative phosphorylation has a detrimental consequence on CNS function. Different ATP synthase subunits have been involved in the patholog.cal process of various neurodeg.nerative disorders. Our g.al was to evaluate the mRNA expression level of the ATP synthase membrane subunit c locus 1 (ATP5g., also named ATP5MC1) g.ne in patients with schizophrenia. Methods: Determination of the expression levels of ATP5g. in plasma and peripheral blood mononuclear cells (PBMCs) were performed by real-time PCR in 90 controls and 90 patients with schizophrenia. Results: Patients had sig.ificantly decreased ATP5g. mRNA expression levels in both plasma and PBMCs compared to controls. The receiver operating.characteristic curve was applied to detect a cut-off value of ATP5g. expression in plasma and PBMCs. The ATP5g. relative expression in PBMCs had better performance with a cutoff value <= 21 (AUC = 0.892, P < 0.001), sensitivity of 94.44%, and specificity of 72.22% in discriminating.between schizophrenic patients. ATP5g. expression in PBMCs was an independent predictor in schizophrenia. Conclusion: This study revealed a down-reg.lation of ATP5g. expression in schizophrenia, precisely expression in PBMCs. That mig.t g.ve insig.t into the role of ATP5g. g.ne in the pathog.nesis of schizophrenia.

Backg.ound: Identification of patients at hig. risk of surg.cal-site infections may allow surg.ons to minimize associated morbidity. However, there are sig.ificant concerns reg.rding.the methodolog.cal quality and transportability of models previously developed. The aim of this study was to develop a novel score to predict 30-day surg.cal-site infection risk after g.strointestinal surg.ry across a g.obal context and externally validate ag.inst existing.models. Methods: This was a secondary analysis of two prospective international cohort studies: g.obalSurg.1 (July-November 2014) and g.obalSurg.2 (January-July 2016). Consecutive adults underg.ing.g.strointestinal surg.ry were elig.ble. Model development was performed using.g.obalSurg.2 data, with novel and previous scores externally validated using.g.obalSurg.1 data. The primary outcome was 30-day surg.cal-site infections, with two predictive techniques explored: penalized reg.ession (least absolute shrinkag. and selection operator ('LASSO')) and machine learning.(extreme g.adient boosting.('Xg.oost')). Final model selection was based on prog.ostic accuracy and clinical utility. Results: There were 14 019 patients (surg.cal-site infections = 12.3%) for derivation and 8464 patients (surg.cal-site infections = 11.4%) for external validation. The LASSO model was selected due to similar discrimination to extreme g.adient boosting.(AUC 0.738 (95% c.i. 0.725 to 0.750) versus 0.737 (95% c.i. 0.709 to 0.765)), but g.eater explainability. The final score included six variables: country income, ASA g.ade, diabetes, and operative contamination, approach, and duration. Model performance remained g.od on external validation (AUC 0.730 (95% c.i. 0.715 to 0.744);calibration intercept -0.098 and slope 1.008) and demonstrated superior performance to the external validation of all previous models. Conclusion: The 'g.obal Surg.cal-Site Infection' score allows accurate prediction of the risk of surg.cal-site infections with six simple

Backg.ound Surg.cal site infection (SSI) is one of the most common infections associated with health care, but its importance as a g.obal health priority is not fully understood. We quantified the burden of SSI after g.strointestinal surg.ry in countries in all parts of the world. Methods This international, prospective, multicentre cohort study included consecutive patients underg.ing.elective or emerg.ncy g.strointestinal resection within 2-week time periods at any health-care facility in any country. Countries with participating.centres were stratified into hig.-income, middle-income, and low-income g.oups according.to the UN's Human Development Index (HDI). Data variables from the g.obalSurg.1 study and other studies that have been found to affect the likelihood of SSI were entered into risk adjustment models. The primary outcome measure was the 30-day SSI incidence (defined by US Centers for Disease Control and Prevention criteria for superficial and deep incisional SSI). Relationships with explanatory variables were examined using.Bayesian multilevel log.stic reg.ession models. This trial is reg.stered with ClinicalTrials. g.v, number NCT02662231. Finding. Between Jan 4, 2016, and July 31, 2016, 13 265 records were submitted for analysis. 12 539 patients from 343 hospitals in 66 countries were included. 7339 (58.5%) patient were from hig.-HDI countries (193 hospitals in 30 countries), 3918 (31.2%) patients were from middle-HDI countries (82 hospitals in 18 countries), and 1282 (1.2%) patients were from low-HDI countries (68 hospitals in 18 countries). In total, 1538 (12.3%) patients had SSI within 30 days of surg.ry. The incidence of SSI varied between countries with hig. (691 [9.4%] of 7339 patients), middle (549 [14.0%] of 3918 patients), and low (298 [23.2%] of 1282) HDI (p<0.001). The hig.est SSI incidence in each HDI g.oup was after dirty surg.ry (102 [17.8%] of 574 patients in hig.-HDI countries;74 [31.4%] of 236 patients in middle-HDI countries;72 [39.8

Backg.ound: This study assessed the potential cost-effectiveness of hig. (80–100%) vs low (21–35%) fraction of inspired oxyg.n (FiO₂) at preventing.surg.cal site infections (SSIs) after abdominal surg.ry in Nig.ria, India, and South Africa. Methods: Decision-analytic models were constructed using.best available evidence sourced from unbundled data of an ong.ing.pilot trial assessing.the effectiveness of hig. FiO₂, published literature, and a cost survey in Nig.ria, India, and South Africa. Effectiveness was measured as percentag. of SSIs at 30 days after surg.ry, a healthcare perspective was adopted, and costs were reported in US dollars ($). Results: Hig. FiO₂ may be cost-effective (cheaper and effective). In Nig.ria, the averag. cost for hig. FiO₂ was $216 compared with $222 for low FiO₂ leading.to a −$6 (95% confidence interval [CI]: −$13 to −$1) difference in costs. In India, the averag. cost for hig. FiO₂ was $184 compared with $195 for low FiO₂ leading.to a −$11 (95% CI: −$15 to −$6) difference in costs. In South Africa, the averag. cost for hig. FiO₂ was $1164 compared with $1257 for low FiO₂ leading.to a −$93 (95% CI: −$132 to −$65) difference in costs. The hig. FiO₂ arm had few SSIs, 7.33% compared with 8.38% for low FiO₂, leading.to a −1.05 (95% CI: −1.14 to −0.90) percentag. point reduction in SSIs. Conclusion: Hig. FiO₂ could be cost-effective at preventing.SSIs in the three countries but further data from larg. clinical trials are required to confirm this. © 2023 The Authors
