Exploring the relationship between postoperative psychological resilience and symptom burden in esophageal cancer patients

PurposePsychological resilience (PR) is an important internal resource for alleviating symptom burden in esophageal cancer surgery patients. However, the relationship between PR and symptom burden has not been explored to date. The primary purpose of this study is to determine the relationship between these two *** multi-center cross-sectional study analyzed data from 486 esophageal cancer surgery patients (2022-2024), assessing symptom burden and PR using the Convalescence Symptom Assessment Scale for Esophagectomy Patients and the Connor-Davidson Resilience Scale. Symptom burden was visualized with radar charts, and network analysis identified the relationship between PR and symptom *** cancer patients experienced a significant symptom burden in the early postoperative period. Network analysis revealed that helplessness (Bridge Strength = 0.23), close and secure relationships (Bridge Strength = 0.21), disturbed sleep (Bridge Strength = 0.11), and pride in your achievements (Bridge Strength = 0.08) were bridge nodes connecting the combination network. Among these, helplessness was identified as a critical factor in initiating and sustaining the relationship between PR and symptom *** plays a crucial role in alleviating symptom burden. This study identifies the bridge nodes in the combination network of PR and symptom burden in esophageal cancer patients. Implementing interventions targeting these factors may enhance the precision and effectiveness of symptom management.

Exploring early postoperative core symptoms in Chinese patients with primary liver cancer: a cross-sectional and longitudinal network analysis

BackgroundPatients with primary liver cancer (PLC) experience a range of symptoms in the early postoperative period. Symptoms include cancer-related symptoms and adverse effects of treatment. Exploring the core symptoms and their dynamics in the early post-hepatectomy patients may help provide better symptom management *** purpose of this study was to identify the core symptoms in early post-hepatectomy patients and to explore the trajectory of their *** the period from March 2021 to September 2022, a total of 281 patients diagnosed with PLC and undergoing radical curative surgery were recruited from the hepatobiliary surgery departments of two hospitals in Eastern China, among whom 249 individuals (88.6%) agreed to participate in the study. A comprehensive symptom assessment was administered to the patients 1-2 days after surgery (T1) and 1-2 days before discharge (T2). Network analysis was used to identify core symptoms in early post-hepatectomy patients based on symptom *** network analysis identified the core symptoms of dry mouth, pain, abdominal distension, the lack of appetite, and nausea at T1. In contrast, at T2, the lack of appetite, pain, shortness of breath, and disturbed sleep were revealed. At the two early time points, while the results of the network structure test showed no statistically significant difference in the overall symptom network structure, there was a significant difference in the results for the core *** study demonstrates the need to identify core symptoms in the early post-hepatectomy to improve symptom management in PLC patients. It is crucial to apply network analysis as an essential component of cancer care.

Materials space-tectonics methodology of conductive mesoporous carbon nanohorns embedded graphene aerogels anchoring Pd nanocrystalline for an efficient ethanol oxidation reaction

Materials space-tectonics (MST) methodology is a powerful strategy to design catalytic materials synthesis to overcome the limitation of monotonous pore geometries and develop functional structures with surface-specific functions. Based on the MST, herein, carbon nanohorns (CNH) as "spacers" inserted into three-dimensional (3D) nitrogen-doped graphene aerogel (NG) anchoring Pd nanocrystalline (Pd/CNH@NG) was constructed through a facile hydrothermal approach and wet chemical reduction method without surfactant and employed as electrocatalyst for ethanol oxidation reaction (EOR). The introduction of CNH can provide important mesopore and macropore characters and generate more active sites to anchor Pd nanocrystalline on the basis of unique internal and interstitial nanopore structures, high surface area and conductive graphitic structure, then enhancing the electrocatalytic efficiency, stability and poison tolerance of catalysts. Meanwhile, the role of CNH doped NG also includes accelerating the reaction of the intermediate (CH3COads) manufactured on the Pd-based aerogels surface with adsorbed hydroxyl (OHads) due to coordination effect from Pyridinic-nitrogen and pyrrolic-nitrogen species. The electrochemical results reveal that the specific and mass activities of the obtained Pd/CNH@NG-2 aerogel (the mass ratio of graphene oxide and CNH is 1:3) are 1.38 and 2.86 times higher than those of the commercial Pd/C for EOR. Simultaneously, the current density of Pd/CNH@NG-2 still maintains 70 % after 500 cycles of complete CV, but commercial Pd/C only maintains 22.6 % of the initial activity. The synthesis of Pd/CNH@NG not only offers a porous three-dimensional aerogel for the EOR but also optimizes a facile strategy to fabricate superior supports for promoting the commercialization of DEFCs.

Electrosynthesis has become an increasingly popular platform in modern organic chemistry, possessing distinctive features and reaction parameters like applied current/potential, electrodes, electrolyte systems, and cell design. While these unique features give chemists more opportunities to control reactivity and selectivity, they also increase the dimensionalities of a reaction and complicate the interactions between variables, making the optimization more challenging. Herein, we present a machine learning (ML) workflow that leverages physical organic descriptor-based yield prediction and orthogonal experimental design to strike a delicate balance between the need for sampling diversity and the pursuit of yield improvements, thereby efficiently identifying ideal conditions for enantioselective palladaelectro-catalyzed annulation from extensive synthetic space. This work shows the potential of synergizing organic electrochemistry and a data-driven approach to tackle multidimensional chemical optimization problems.

Anchoring Bi(OH)3 species modified Pd nanocrystalline on nitrogen-doped graphene enables a robust oxidation of ethanol

Combining Pd with oxophilic species and appropriate catalyst supports is regarded as an effective approach to strengthen the intrinsic electrocatalytic performance of ethanol oxidation reaction (EOR). In this work, palladium nanocrystals (NCs) modified with Bi (OH)3 were successfully immobilized on the three-dimensional nitrogen-doped graphene (Pd-Bi(OH)3/NG) via a simple one-pot route. The resulting Pd-Bi(OH)3/NG demonstrates exceptional electrocatalytic performance for EOR under alkaline conditions, exhibiting enhanced mass and specific activity, kinetics, durability and long-term stability, significantly surpassing that of commercial Pd/C. Notably, the electrocatalytic performance of Pd-Bi(OH)3/NG-2 (with an atomic ratio of Pd/Bi at 7:1) is the best among all the tested electrocatalysts. The results reveal that the improved electrocatalytic activity of Pd-Bi (OH)3/NG may stem from the significant contributions of the Bi(OH)3 species decorating of the Pd NCs and their incorporation into the nitrogen-doped graphene.

Symptom Experiences before Medical Help-Seeking and Psychosocial Responses of Patients with Esophageal Cancer: A Qualitative Study

Objective. The purpose of this study was to investigate patients with esophageal cancer symptom experiences before seeking medical help and psychosocial responses. Methods. Within one month of diagnosis, semistructured interviews were held with 14 adult patients with esophageal cancer. To fully comprehend the symptom experiences that patients with esophageal cancer had prior to seeking medical help and the psychosocial responses they had to these experiences, thematic analysis and interpretative phenomenological analysis were used. Results. Patients with esophageal cancer recounted their symptom experiences, and psychosocial responses were categorized into three main themes: physical, psychological, and social. Three subthemes were included in the physiological aspect: early symptoms (warning signs), specific symptoms (the catalyst for seeking medical help), and concurrent symptoms (masked side effects). Four subthemes were associated with psychological aspects: unfairness, regret, worry, and acceptance. Two subthemes were related to the social element, namely, dragging down one's family and societal self-isolation. Conclusion. Due to the self-remitting nature of early symptoms, the normalization of their interpretation, and concealment by other illnesses, patients with esophageal cancer may delay medical help-seeking. The findings could serve as a reference for healthcare professionals to implement tailored psychosocial interventions and help patients identify symptoms early in order to seek medical help.

Economic evaluation of hepatocellular carcinoma surveillance in chronic hepatitis B patients with virological remission

BackgroundSubsequent risk of hepatocellular carcinoma (HCC) persists in chronic hepatitis B (CHB) patients with virological remission. We aimed to assess the cost-effectiveness of HCC surveillance in those patients and determine appropriate age to commence or discontinue *** developed an individual-based state transition model, simulating the advancement of HCC in CHB patients with virological remission. We used this model to compare the incremental cost-effectiveness ratio (ICER) and long-term health outcomes of biannual or annual HCC surveillance for varying durations with no *** compensated cirrhosis patients with CHB, biannual surveillance was not cost-effective for all age groups, while annual surveillance was cost-effective for patients aged 55 to 70 (ICER USD 28,076 / quality-adjusted life years [QALY] gained), which detected 176 additional early HCC cases in a 100,000-person cohort compared to no surveillance. In CHB patients with advanced fibrosis, annual surveillance for patients aged 40 to 75 was the most cost-effective strategy (ICER USD 4,984/QALY gained), which detected 289 additional early HCC per 100,000 *** surveillance for patients with compensated cirrhosis or advanced fibrosis was a more cost-effective option that demonstrated substantial economic benefits, being slightly less effective than biannual surveillance at a significantly lower cost, providing insights for professionals in evaluating HCC surveillance among high-risk patients in China. The most cost-effective age group for surveillance of hepatocellular carcinoma (HCC) varied depending on the stage of the disease. Regular annual surveillance for patients with compensated cirrhosis or advanced fibrosis was more cost-effective option, showing great economic and clinical benefits with slightly less effective than biannual surveillance but significantly lower cost.

Trajectory of change in symptom patterns among patients undergoing surgery for oesophageal cancer: a prospective longitudinal study using latent transition analysis

Purpose Patients with oesophageal cancer may experience a heavy burden of postoperative symptoms. There is insufficient evidence to fully explain the role of symptom clusters in the trajectory of cancer care for patients with oesophagectomy. The aim of this study was to identify the change trajectory of individual symptoms after surgery. Methods A prospective longitudinal study was conducted to investigate patients' experience of symptoms at four time points after surgery. Latent profile analysis was used to identify patients' unique postoperative symptom experiences, and latent transition analysis was used to identify changes in patient symptom patterns across time points. Results Among the 398 patients, there were three symptom groups: the "persistent fatigue group", "fluctuating physical symptoms group", and "high symptoms group". Over time, the "persistent fatigue group" had a greater than 70% probability of remaining in this group, while the "fluctuating physical symptoms group" and "high symptoms group" had a large change in transition. Anastomotic site, primary caregiver, and cancer stage were found to predict the pattern of transition between symptom groups. Conclusions Patients' experience of symptoms changes over time, and it is important to target interventions based on the trajectory of symptoms to improve symptom management in oesophageal cancer patients.

This study investigated tooth flank modification of a line contact spiral bevel gear to solve the issues of meshing impact and edge contact during the meshing process. To this end, the geometric interference and impact caused by errors and load deformations during the alternating meshing process of multiple tooth flank pairs were studied. First, the condition for controlling the transmission error of the line contact spiral bevel gear pair without errors as a constant value was derived. Then, considering the influence of load deformations and errors, the modification areas and amounts of the modified tooth flank were determined, and a modification method for the line contact spiral bevel gear was proposed. The contact analysis results of the example pairs before and after modification showed that the proposed modification method could effectively avoid the meshing impact and edge contact of the line contact tooth flank pair, while reducing the maximum contact stress. © 2024, The Author(s), under exclusive license to Springer Nature Switzerland AG.

Survival experience of patients undergoing oesophagectomy during the recovery period: A meta-synthesis of qualitative studies

Aims and ObjectiveTo systematically review and synthesise existing qualitative research evidence describing the survival experience of patients undergoing oesophagectomy during recovery. BackgroundPatients with oesophageal cancer undergoing surgical treatment have severe physical and psychological burdens during the recovery period. Qualitative studies on the survival experience of patients undergoing oesophagectomy are increasing annually, but there is no integration of qualitative evidence. DesignA systematic review and synthesis of qualitative studies were conducted following the ENTREQ. MethodsFive English (CINAHL, Embase, PubMed, Web of Science and Cochrane library) and three Chinese (Wanfang, CNKI and VIP) databases were searched for literature on the survival of patients undergoing oesophagectomy during the recovery period from its establishment in April 2022. The quality of the literature was evaluated by the 'Qualitative Research Quality Evaluation Criteria for the JBI Evidence-Based Health Care Centre in Australia', and the data were synthesised by the thematic synthesis method of Thomas and Harden. ResultsA total of 18 studies were included, and four themes were identified: physical and mental dual challenges, impaired social functioning, efforts to return to normal life, lack of knowledge and skills in post-discharge care, and thirst for external support. ConclusionsFuture research should focus on the problem of reduced social interaction during the recovery of patients with oesophageal cancer, formulating individualised exercise intervention programs and establishing a sound social support system. Relevance to Clinical PracticeThe results of this study provide evidence-based support for nurses to carry out targeted interventions and reference methods for patients with oesophageal cancer to rebuild their lives.
