The biosafety incident response competence scale for clinical nursing staff: a development and validation study

Aims This study was designed to develop a biosafety incident response competence scale and evaluate its validity and reliability among clinical nurses. Design This study employed a sequential approach, comprising four phases: (1) the establishment of a multidimensional conceptual model, (2) the preliminary selection of the items, (3) further exploration and psychometric testing of the items, (4) the application of the scale among clinical nurses. Methods The biosafety incident response competence conceptual model was developed through literature review and the Delphi method. A total of 1,712 clinical nurses participated in the preliminary items selection, while 1,027 clinical nurses were involved in the further psychometric testing from July 2023 to August 2023. The item analysis, exploratory factor analysis and confirmatory factor analysis were conducted to evaluate the construct validity. Reliability was measured using Cronbach's alpha, split-half reliability, and test-retest reliability, while validity analysis included content validity, structural validity, convergent validity, and discriminant validity. From September to November 2023, we conducted a survey using the established scale with a total of 4338 valid questionnaires collected. T-test and variance analysis was employed to determine potential variations in biosafety incident response competence based on participants characteristics. Results The final scale is composed of 4 factors and 29 items, including monitoring and warning abilities, nursing disposal abilities, biosafety knowledge preparedness, and infection protection abilities. The explanatory variance of the 4 factors was 75.100%. The Cronbach's alpha, split-half reliability and test-retest reliability were 0.974, 0.945 and 0.840 respectively. The Scale-level content validity index was 0.866. Since toys are potential sources of lead contamination for young children, we aim to explore potential correlations between blood-lead-levels (BLLs) of young children and toy-lead contamination using two concurrent investigations. In investigation-1, a total of 2249 children under 7 years old were studied using stratified-cluster-sampling in 6 districts of Shanghai, parents completed questionnaires and children's whole-blood was collected, and then BLLs were measured using tungsten-atomizer-absorption-spectrophotometer. In investigation-2, a total of 288 toys made of different materials (plastic, wooden, metals, and stuffed cloth) were collected in 6 great supermarkets/toy-stores and toy-lead-levels were measured by a portable X-ray-fluorescence instrument. The geometric-mean of BLLs was 2.02 & mu;g/dL. Adjusting for related confounders, playing with plastic toys was a significant risk factor (& beta;= 0.067, P < 0.05) for BLL-increment. Plastic toys accounted for the highest proportion (154/288, 53.47%) and had the highest rates exceeding toy-lead-standards (the over-standard rates of & GE;300/600 mg/kg, plastic toys: 3.90%/1.30%;other toys: 0.00%/0.00%). Children's BLLs in the three (Pudong-suburban, Xuhui, and Yangpu) districts and toy-lead over-standard rates in the neighboring stores were compared, the highest BLLs and over-standard rates were both found in Pudong-suburban district and similar BLLs and over-standard rates were observed between Yangpu and Xuhui districts (Pudong-suburban vs. Xuhui vs. Yangpu: 2.07 vs. 1.55 vs. 1.50 & mu;g/dL for the medians of BLLs, P < 0.001;1.67% vs. 0.00% vs. 0.00% for the over-standard rates of & GE;600 mg/kg). This study raised the possibility that lead contamination in toys (especially in low social-economic-status areas) might be associated with BLLs in young children. Lead contamination from plastic toys needs cautions for preventing children from lead exposure.

Melatonin partially rescues defects induced by tranexamic acid exposure during oocyte maturation in mice

Tranexamic acid (TXA) is widely used among young women because of its ability to whiten skin and treat menorrhagia. Nevertheless, its potential effects on oocyte maturation and quality have not yet been clearly clarified. Melatonin (MT) is an endogenous hormone released by the pineal gland and believed to protect cells from oxidative stress injury. In the present study, we used an in vitro maturation model to investigate the toxicity of TXA and the protective role of MT in mouse oocytes. Compared with the control group, the TXA-exposed group had significantly lower nuclear maturation (57.72% vs. 94.08%, P < 0.001) and early embryo cleavage rates (38.18% vs. 87.66%, P < 0.001). Further study showed that spindle organization (52.56% vs. 18.77%, P < 0.01) and chromosome alignment (33.23% vs. 16.66%, P < 0.01) were also disrupted after TXA treatment. Mechanistically, we have demonstrated that TXA induced early apoptosis of oocytes (P < 0.001) by raising the level of reactive oxygen species (P < 0.001), which was consistent with an increase in mitochondrial damage (P < 0.01). Fortunately, all these effects except the spindle defect were successfully rescued by an appropriate level of MT. Collectively, our findings indicate that MT could partially reverse TXA-induced oocyte quality deterioration in mice by effectively improving mitochondrial function and reducing oxidative stress-mediated apoptosis. NEW & NOTEWORTHY Tranexamic acid is increasingly used to whiten skin, reverse dermal damages, and treat heavy menstrual bleeding in young women. However, its potential toxicity in mammalian oocytes is still unclear. Our study revealed that tranexamic acid exposure impaired the mouse oocyte quality and subsequent embryo development. Meanwhile, melatonin has been found to exert beneficial effects in reducing tranexamic acid-induced mitochondrial dysfunction and oxidative stress.

Age-related cognitive decline presents a significant health challenge for older adults, with no specific medication available to counteract it. Early detection of cognitive decline is crucial for timely intervention and effective management. Sleep status plays a vital role in evaluating cognitive function, as disrupted sleep patterns are associated with cognitive decline and increased risk of neurodegenerative conditions. Existing methods for assessing sleep status, such as self-report questionnaires, actigraphy, and polysomnography, lack the ability to consistently monitor sleep status over a long period. In this paper we introduced SLEEPWISE, an AI-powered platform deployed in residential environments for continuous ambient sleep monitoring to assess the cognitive well-being of older adults. SLEEPWISE utilizes non-contact pads with highly sensitive piezoelectric sensors to monitor sleep parameters such as duration and efficiency, which are transmitted daily to the server. Moreover, we have also developed a gamified module for SLEEPWISE, allowing an objective assessment of daily cognitive functions. This module aids SLEEPWISE in establishing the correlation models between sleep and cognitive health. To evaluate the effectiveness of SLEEP-WISE, a pilot study was conducted with participants recruited from a local hospital. During the 30-days trials, SLEEPWISE identified a negative impact on congnitive performance associated with increased nocturnal body movements and getting out of bad. These findings emphasize the potential of SLEEPWISE as an early screening tool for evaluating daily cognitive health and well-being. © 2024 IEEE.

Tobacco Quitline Callers Who Use Cannabis and Their Likelihood of Quitting Cigarette Smoking

Introduction: Cigarette smoking continues to decline in the U.S., but cannabis use is increasing. Many people who smoke cigarettes also use cannabis. This study examines the characteristics of persons who co-use and those who do not co-use and the likelihood of quitting cigarettes for callers to Kick It California, a large state tobacco quitline. Methods: Data were examined from Kick It California callers from January 2020 through December 2023 (N=45,151), including those from a subgroup randomly sampled and reached for evaluation at 7 months after quitline enrollment (n=3,545). n =3,545). The rate of cigarette smoking cessation at 7 months after enrollment for people who co-use cannabis was compared with that for people who do not. Analyses started in 2023 and concluded in January 2024. Results: More than a quarter (27.2%) of Kick It California callers co-used cannabis. They were more likely to be male, to be younger, and to have a mental health condition than those who did not. Those who co-use cannabis and those who do not have similar rates of receiving quitline counseling or using Food and Drug Administration-approved - approved cessation aids. Controlled for effects of personal characteristics and use of smoking-cessation services, people who co-use cannabis were less likely to quit cigarette smoking 7 months after enrollment (23.2% vs 28.9%;p < 0.001). Among those who co-use, 42.9% intended to quit using cannabis in the next 30 days. Conclusions: A substantial percentage of tobacco quitline callers use cannabis. Those who do co- use quit cigarette smoking at a lower rate than those who do not. Over 40% of people who co-use reported intention to quit cannabis, making tobacco quitlines a rich environment to learn about people who co-use and develop strategies for intervention.

目的:研究经皮肝Ⅰ期胆道造瘘取石术(PTOBF)治疗有胆道手术史的肝内胆管结石患者的临床疗效。方法:2015年1月-2019年1月收治有胆道手术史的肝内胆管结石患者50例,随机分为两组。对照组给予传统经皮肝胆道镜治疗;试验组给予PTOBF治疗,比较两组治疗效果。结果:试验组取石次数、术后首次排气时间及排便时间均低于对照组,差异有统计学意义(P<0.05);试验组结石残留率及结石复发率均低于对照组,差异有统计学意义(P<0.05);试验组并发症发生率低于对照组,差异有统计学意义(P<0.05)。结论:PTOBF治疗有胆道手术史的肝内胆管结石患者,可减少术中取石次数,缩短首次排气时间及排便时间,降低结石残留率、术后结石复发率及并发症发生率。

New crystalline 1D/2D/3D indium selenides directed by piperidine and auxiliary solvents

The construction of cluster-based crystalline chalcogenide structures through the traditional solvothermal method relies on synergistic control of precursors, template cations and auxiliary solvents. Generally, the combination of metal precursors plays a crucial role in controlling the size of clusters, while organic templates and auxiliary solvents usually contribute to the type of clusters and architecture of the framework. Decades of synthetic efforts have been mainly devoted to expanding organic amine templates for constructing new structures. However, the important role of auxiliary solvents in enriching the chalcogenide family is usually disregarded. Reported here are several new crystalline In-Se compounds (ISP-1 to 4) with different dimensions, obtained by elaborately regulating auxiliary solvents under the direction of the same organic template, piperidine. Of these four structures, ISP-1 is constructed by irregular supertetrahedral clusters, giving a novel 2D structure with a corner-shared single Se atom and In2Se3 five-member ring as linkers;ISP-2 has a 1D structure composed by interlinked In2Se3 five-member rings;ISP-4 is constructed by supertetrahedral T2 clusters exhibiting an uncommon zeolite-like mog network.

Variability in competitive fitness among environmental and clinical azole-resistant Aspergillus fumigatus isolates

Azoles are the primary antifungal drugs used to treat infections caused by Aspergillus fumigatus. However, the emergence of azole resistance in A. fumigatus has become a global health concern despite the low proportion of resistant isolates in natural populations. In bacteria, antibiotic resistance incurs a fitness cost that renders strains less competitive in the absence of antibiotics. Consequently, fitness cost is a key determinant of the spread of resistant mutations. However, the cost of azole resistance and its underlying causes in A. fumigatus remain poorly understood. In this observation, we revealed that the 10 out of 15 screened azole-resistant isolates, which possessed the most common azole-targeted cyp51A mutations, particularly the presence of tandem repeats in the promoter region, exhibit fitness cost when competing with the susceptible isolates in azole-free environments. These results suggest that fitness cost may significantly influence the dynamics of azole resistance, which ultimately contributes to the low prevalence of azole-resistant A. fumigatus isolates in the environment and clinic. By constructing in situ cyp51A mutations in a parental azole-susceptible strain and reintroducing the wild-type cyp51A gene into the azole-resistant strains, we demonstrated that fitness cost is not directly dependent on cyp51A mutations but is instead associated with the evolution of variable mutations related to conidial germination or other unknown development-related processes. Importantly, our observations unexpectedly revealed that some azole-resistant isolates showed no detectable fitness cost, and some even exhibited significantly increased competitive fitness in azole-free environments, highlighting the potential risk associated with the prevalence of these isolates.
