Use and Benefit of Neoadjuvant Versus Adjuvant Chemotherapy in Node-Negative, T1 Triple Negative Breast Cancer

PURPOSE:There are scarce data about the use and benefit of neoadjuvant chemotherapy (NACT) in small size, node-negative triple-negative breast cancer (TNBC). We examined pathological response and survival outcomes of patients with T1N0 TNBC who received NACT. METHODS:This is a retrospective cohort study using data from the Surveillance, Epidemiology, and End Results database of patients with T1N0 TNBC diagnosed between 2010 and 2020. Cox regression models were used to compare overall survival (OS) for adjuvant chemotherapy (ACT) and NACT cohorts. Cumulative incidence functions and breast cancer-specific hazard models were used to compare breast cancer specific survival (BCSS) between ACT and NACT cohorts. RESULTS:We found 8146 patients treated with ACT and 1263 patients treated with NACT. Age < 50 years, mastectomy and radiation therapy were associated with higher odds of receiving NACT. There was no difference in BCSS or OS for patients with T1b/T1cN0 TNBC who received NACT or ACT. However, patients with T1aN0 TNBC had worse BCSS and OS when treated with NACT as compared to those who received ACT. Pathological complete response (pCR) in T1N0 tumors was associated with improved OS (HR, 0.28) and BCSS (HR, 0.21). Furthermore, T1aN0 tumors had lower pCR rate than T1cN0 tumors (18% vs. 47%). CONCLUSION:Patients with node-negative, T1b and T1c tumors benefit equally from the use of ACT and NACT. The results of worse outcome in patients with node-negative, T1a tumors treated with NACT compared to ACT are intriguing and need further investigation.

Annexin A1 protects periodontal ligament cells against lipopolysaccharide-induced inflammatory response and cellular senescence: An implication in periodontitis

Periodontitis is an inflammatory condition that affects the tooth-supporting structures, triggered by the host's immune response toward the bacterial deposits around the teeth. Annexin A1 (AnxA1), a vital member of the annexin superfamily, is known for its diverse physiological functions, particularly its anti-inflammatory and anti-senescence properties. We hypothesized that AnxA1 has a protective effect against lipopolysaccharide (LPS)-induced inflammatory responses and cellular damage in periodontal ligament cells (PDLCs). In this study, we demonstrate that LPS stimulation significantly reduced telomerase activity in PDLCs, a decline that was dose-dependently reversed by AnxA1. Importantly, AnxA1 protected the cells from LPS-induced cellular senescence and the downregulation of human telomerase reverse transcriptase (hTERT) expression. In line with this, AnxA1 suppressed the LPS-induced expression of p21 and p16 at both the mRNA and protein levels. Furthermore, AnxA1 demonstrated potent anti-inflammatory effects by inhibiting the secretion of interleukin 6 (IL-6), interleukin 8 (IL-8), and monocyte chemoattractant protein-1 (MCP-1). It also mitigated LPS-induced oxidative stress by reducing the levels of phosphorylated Foxo3a (Ser253) and restored sirtuin 1 (SIRT1) expression. Notably, SIRT1 silencing abolished AnxA1's protective effects on Foxo3a phosphorylation and cellular senescence, suggesting that SIRT1 mediates AnxA1's actions. In conclusion, AnxA1 protected PDLCs against LPS-triggered inflammation and cell senescence by activating SIRT1 signal pathway. These findings indicate that AnxA1 could serve as a promising therapeutic strategy for the treatment of periodontitis.

VarGAN: Adversarial Learning of Variable Semantic Representations

Variable names are of critical importance in code representation learning. However, due to diverse naming conventions, variables often receive arbitrary names, leading to long-tail, out-of-vocabulary (OOV), and other well-known problems. While the Byte-Pair Encoding (BPE) tokenizer has addressed the surface-level recognition of low-frequency tokens, it has not noticed the inadequate training of low-frequency identifiers by code representation models, resulting in an imbalanced distribution of rare and common identifiers. Consequently, code representation models struggle to effectively capture the semantics of low-frequency variable names. In this paper, we propose VarGAN, a novel method for variable name representations. VarGAN strengthens the training of low-frequency variables through adversarial training. Specifically, we regard the code representation model as a generator responsible for producing vectors from source code. Additionally, we employ a discriminator that detects whether the code input to the generator contains low-frequency variables. This adversarial setup regularizes the distribution of rare variables, making them overlap with their corresponding high-frequency counterparts in the vector space. Experimental results demonstrate that VarGAN empowers CodeBERT to generate code vectors that exhibit more uniform distribution for both low- and high-frequency identifiers. There is an improvement of 8% in similarity and relatedness scores compared to VarCLR in the IdBench benchmark. VarGAN is also validated in downstream tasks, where it exhibits enhanced capabilities in capturing token- and code-level semantics.

Aims Research on the appropriate scale structure and spatial layout of ecological,urban,and agricultural spaces and the effective and methodical development and protection of urban space can provide a solid foundation for the sustainable development of the complex socio-economic urban environment. Methods Based on the perspective of a coupled social-ecological system,this paper constructed an assessment model to determine the ecological protection importance and development suitability of urban space,proposed the ecological-urban-agricultural priority for densely populated urban areas,and established the appropriate func-tional layout of urban space using a priority matrix-based mapping method. Results Through an empirical case study of Shenzhen,this paper determined the optimal scale structure of the types of functional spaces,identified areas of functional conflict,and proposed targeted optimization strategies for func-tional mismatch *** findings show that the appropriate scale ratio of Shenzhen's urban,ecological,and agricul-tural functions is 5∶4∶1,which is consistent with the actual structure ratio of Shenzhen's land use trends. Conclusions The proposed method can predict the appropriate scale structure of urban space development and ecological protection based on the comprehensive multi-perspective consideration of social and economic factors to provide a scientific reference for effective decision-making on urban space optimization and sustainable development.

A Novel Method for Small-Target Detection in Sea Clutter: Spectral Clustering Based on Neighborhood Density Similarity Measure

Small-target detection in sea clutter has always been difficult due to the low signal clutter ratio (SCR). Feature-based detection methods have emerged as a significant area of research in recent years, where pattern recognition methods involve the use of similarity measures. The Gaussian kernel function used in traditional spectral clustering algorithms performs well when handling nonlinear data, however, it may not effectively capture the relationships between sparsely distributed samples. Therefore, this article proposes a novel method for small-target detection in sea clutter: spectral clustering based on neighborhood density similarity measure (SC-ND) to address the aforementioned issues. It incorporates neighborhood density into the Gaussian kernel function and then uses label information to guide the construction of the similarity matrix, at which point a false alarm controllable is achieved. The similarity matrix is then spectrally decomposed to obtain a new representation of the samples, which are subsequently classified using an improved k-means algorithm. Validated on the IPIX 1993 radar dataset, the proposed method achieves a detection probability of 71.1% with an observation time of 1.024 s and a false alarm rate of 0.001. The results are better than fractal-based detector, tri-feature-based detector, and feature-based detector using three TF features.

Synergistic chemical traction and pre-intercalation strategies enable high-quality MnO2 composites for efficient ammonium capture

The delta-MnO2 exhibits an ideal capacitive deionization (CDI) electrode for the efficient removal of ammonium ions (NH4+), owing to its unique layered structure that facilitates effective insertion and extraction of ions. However, MnO2 is constrained by self-aggregation, inadequate electrical conductivity, and sluggish reaction kinetics in practical applications, leading to subpar ammonium removal performance. Here, the polyaniline (PANI) intercalated MnO2 and carbon nanotubes (CNTs) composites (P/M-C-X) were synthesized by a straightforward and swift one-step inorganic/organic interface reaction using the chemical traction effect of aniline. The synergistic effect of the increased layer spacing and the introduced oxygen vacancy after PANI intercalated and the 1D/2D conductive interconnect structure constructed by CNTs can effectively improve the electrical conductivity, promote ions/electrons diffusion, and enhance the charge reaction kinetics. As results, P/M-C-50 exhibits excellent NH4+ removal performance, including the maximum salt adsorption capacity (SAC) (160.0 mg g- 1 ), ultra-high salt adsorption rate (SAR) (1.07 mg g- 1 s- 1 ), and good cyclic stability. Furthermore, the comprehension of NH4+ storage mechanism has been enhanced through structural characterization, electrochemical analysis, and theoretical calculation. This study shows that P/M-C-50 has broad potential as a CDI ammonium removal electrode, and lays a solid foundation for the effective recovery and reuse of ammonium resources.

Monthly and Interannual Variations in Upwelling and Chlorophyll off the Vietnam Coast in Summer Based on Deep Learning

The deep learning technique is used to automatically identify upwelling regions off the Vietnam coast using Sea Surface Temperature (SST) data from satellite imagery. After delineating these upwelling areas, this study is pioneering in deriving the upwelling probability off the Vietnam coast, pinpointing two significant upwelling hotspots. To deepen the understanding of the individual impacts of wind and sea surface height anomalies on monthly and spatial variations in upwelling probability and chlorophyll distribution, the Empirical Orthogonal Function method was employed to analyze this data set. The upwelling probability demonstrates a close relationship with wind and sea surface height anomalies, while chlorophyll concentration correlates with the strength of the southwesterly wind. The southwesterly wind, Ekman pumping, eddy dipole, and northeastward jet have been identified as key drivers of the formation and spatial variability of upwelling in summer, albeit with varying contributions to different parts of the coastal region. The distribution of upwelling probability correlates with chlorophyll variation off the northern Vietnam coast. High chlorophyll concentration off the southern Vietnam coast is primarily influenced by the southwesterly wind that carries the Mekong River plume eastward. Furthermore, two abnormal scenarios in upwelling and chlorophyll concentration during 2010 and 2018 were analyzed, attributed to the abnormal southwesterly monsoon and coastal circulation influenced by an El Ni & ntilde;o event and a weak La Ni & ntilde;a event with a positive Indian Ocean Dipole, respectively. This study elucidates the dynamic processes underlying the intricate chlorophyll distribution over the continental shelf, which is influenced by both the river plume and upwelling.

Strength Degradation Mechanism of CFRP and Aluminium Alloy Hybrid Bonded-Riveted Joints Under Salt Spray Environment

Hybrid bonded-riveted joint has poor corrosion resistance under salt spray environment, especially for dissimilar materials. The study investigates the strength degradation mechanism of carbon fiber reinforced polymer and aluminium alloy (CFRP-Al) hybrid bonded-riveted joints under salt spray environment, and it proposes a method to improve corrosion resistance of CFRP-Al hybrid bonded-riveted joints. The study found that the strength degradation of CFRP-Al hybrid joints under salt spray environment was mainly attributed to the decrease in bonding strength at the aluminum/adhesive interface. Comparison results further showed that the diffusion of corrosive medium along the aluminum/adhesive bonding interface caused a 33.3% decrease in joint strength, while the accelerated electrochemical corrosion of the aluminum surface caused a 59.3% decrease in joint strength. Applying laser to modify the surface characteristics of aluminum resulted in a 46% increase in the maximum shear loads of CFRP-Al hybrid bonded-riveted joints before salt spray exposure and a 45% increase after salt spray exposure. It has been found that laser surface treatment produced rough microstructures on aluminum surface, which increases the bonding area between adhesive and aluminum alloy, effectively preventing the diffusion of corrosive medium along the bonding interface. Furthermore, a dense oxide layer was formed by laser surface treatment on aluminum surface, which contributed to slow down the galvanic corrosion between CFRP and aluminum. Consequently, an apparently improvement was observed in the bearing capacity and corrosion resistance of CFRP-Al hybrid bonded-riveted joints.

Economic evaluation of hepatocellular carcinoma surveillance in chronic hepatitis B patients with virological remission

BackgroundSubsequent risk of hepatocellular carcinoma (HCC) persists in chronic hepatitis B (CHB) patients with virological remission. We aimed to assess the cost-effectiveness of HCC surveillance in those patients and determine appropriate age to commence or discontinue *** developed an individual-based state transition model, simulating the advancement of HCC in CHB patients with virological remission. We used this model to compare the incremental cost-effectiveness ratio (ICER) and long-term health outcomes of biannual or annual HCC surveillance for varying durations with no *** compensated cirrhosis patients with CHB, biannual surveillance was not cost-effective for all age groups, while annual surveillance was cost-effective for patients aged 55 to 70 (ICER USD 28,076 / quality-adjusted life years [QALY] gained), which detected 176 additional early HCC cases in a 100,000-person cohort compared to no surveillance. In CHB patients with advanced fibrosis, annual surveillance for patients aged 40 to 75 was the most cost-effective strategy (ICER USD 4,984/QALY gained), which detected 289 additional early HCC per 100,000 *** surveillance for patients with compensated cirrhosis or advanced fibrosis was a more cost-effective option that demonstrated substantial economic benefits, being slightly less effective than biannual surveillance at a significantly lower cost, providing insights for professionals in evaluating HCC surveillance among high-risk patients in China. The most cost-effective age group for surveillance of hepatocellular carcinoma (HCC) varied depending on the stage of the disease. Regular annual surveillance for patients with compensated cirrhosis or advanced fibrosis was more cost-effective option, showing great economic and clinical benefits with slightly less effective than biannual surveillance but significantly lower cost.
