Virtual reality (VR) is cited as offering the ultimate empathy machine [31]. This theory makes sense intuitively since VR enables a user to step in to another's shoes and experience the world as they do. We define this specific class of mental state as 'emotional empathy' [49]. The ability of VR to evoke emotional empathy is widely lauded as a good thing [18, 35, 43]. In this paper we invite labels such as 'Luddites' and 'technophobes' as we question the soundness of such claims. We instead offer warnings regarding employing VR is this manner and urge caution. Rather than dismiss the usefulness of VR in this realm we offer alternative implementation techniques in order to evoke more positive results in users. VR offers much utility for psychologists, psychiatrists, and neuroscientists due to the ability it affords to alter cognition. While promoting the medium in general, we offer warnings regarding potential short and long term neurological impacts. We encourage increased research focus on the underlying neural mechanisms that underpin VR's successful multisensory hijack.

Towards an Architecture to Guarantee Both Data Privacy and Utility in the First Phases of Digital Clinical Trials

In the era of the Internet of Things (IoT), drug developers can potentially access a wealth of real-world, participant-generated data that enable better insights and streamlined clinical trial processes. Protection of confidential data is of primary interest when it comes to health data, as medical condition influences daily, professional, and social life. Current approaches in digital trials entail that private user data are provisioned to the trial investigator that is considered a trusted party. The aim of this paper is to present the technical requirements and the research challenges to secure the flow and control of personal data and to protect the interests of all the involved parties during the first phases of a clinical trial, namely the characterization of the potential patients and their possible recruitment. The proposed architecture will let the individuals keep their data private during these phases while providing a useful sketch of their data to the investigator. Proof-of-concept implementations are evaluated in terms of performances achieved in real-world environments.

Smart Device-Based Notifications to Promote Healthy Behavior Related to Childhood Obesity and Overweight

Obesity is one of the most serious public health challenges of the 21st century and it is a threat to the life of people according to World Health Organization. In this scenario, family environment is important to establish healthy habits which help to reduce levels of obesity and control overweight in children. However, little efforts have been focused on helping parents to promote and have healthy lifestyles. In this paper, we present two smart device-based notification prototypes to promote healthy behavior with the aim of avoiding childhood overweight and obesity. The first prototype helps parents to follow a healthy snack routine, based on a nutritionist suggestion. Using a fridge magnet, parents receive graphical reminders of which snacks they and their children should consume. The second prototype provides a graphical reminder that prevents parents from forgetting the required equipment to practice sports. Prototypes were evaluated by nine nutritionists from three countries (Costa Rica, Mexico and Spain). Evaluations were based on anticipation of use and the ergonomics of human-system interaction according to the ISO 9241-210. Results show that the system is considered useful. Even though they might not be willing to use the system, they would recommend it to their patients. Based on the ISO 9241-210 the best ranked features were the system's comprehensibility, the perceived effectiveness and clarity. The worst ranked features were the system's suitability for learning and its discriminability.

Efficacy of personalized models in discriminating high cognitive demand conditions using text-based interactions

Although high cognitive demand conditions can impair psychological, physical, and behavioral processes without appropriate management, current measurement methods are too cumbersome for continuous monitoring of cognitive demand, and do not account for individual differences. This research uses keystroke and linguistic markers of typed text to construct individualized models of cognitive demand response to discriminate high and low cognitive demand conditions, the results of which can have implications for design of cognitive demand monitoring systems for personalized health management. We constructed within-subject models of cognitive demand response for nine participants and one between-subjects model based on 20 participants. The AUCs for personalized models ranged from 0.679 to 0.953 (Mean=0.826, SD=0.085), significantly higher than chance (p < 0.0001) and the 0.714 AUC for the generic model (p=0.002). Although the features in each model were different, the most common features across models are rate of negative emotion, lexical diversity, rate of words over six letters, and word count. These results confirm significant individual differences in cognitive demand response and suggest that those developing measurement methods used in a monitoring system should consider adaptation to individual characteristics. Our research operationalizes the effects of cognitive demand on HCI and contributes a unique combination of text and keystroke features used to detect high cognitive demand situations.

Tutorials in eLearning-How Presentation Affects Outcomes

The presentation of learning materials affects how we learn. In this paper, we use eye tracking to investigate how different sequences of text and test questions affect performance outcomes, eye movements, and reading behavior for first (L1) English language and second (L2) English language readers. We show that different presentation sequences induce different performance outcomes, eye movements, and reading behavior. The sequence can affect how a participant reads the text as well as their perceptions of how well they understood what they read. For instance, if questions and text are not shown together, this improves participants' ability to accurately perceive their comprehension and promotes thorough reading. Alternatively, showing questions before the text promotes skimming behavior. Importantly, the presentation sequence affects both L1 and L2 readers in the same way. We observe that L2 reader take longer to read text but have the same comprehension levels as L1 readers;this difference comes primarily from longer fixation durations. The results from this paper can be used to design learning materials in eLearning environments to influence how students interact with the learning environment as well as how they learn. The purpose of this investigation is to make informative decisions about designing adaptive eLearning environments.

Hepatocellular carcinoma (HCC) is a common worldwide cancer that is rising rapidly in incidence. MRI is a powerful noninvasive imaging modality for HCC detection, but lack of specific contrast agents limits visualization of small tumors. EGFR is frequently overexpressed in HCC and is a promising target. Peptides have fast binding kinetics, short circulatory half-life, low imaging background, high vascular permeability, and enhanced tissue diffusion for deep tumor penetration. We demonstrate a peptide specific for EGFR labeled with an ultrasmall paramagnetic iron oxide (UPIO) nanoparticle with 3.5 nm dimensions to target HCC using T-1-weighted MRI. We modified the hydrophobic core with oleic acid and capped with PEGylated phospholipids DSPE-PEG and DSPE-PEG-Mal. The EGFR peptide is attached via thioether-mediated conjugation of a GGGSC linker to the maleimide-terminated phospholipids. On in-vivo MR images of HCC xenograft tumors, we observed peak nanoprobe uptake at 2 h post-injection followed by a rapid return to baseline by similar to 24 h. We measured significantly greater MR signal in tumor with the targeted nanoprobe versus scrambled peptide, blocked peptide, and Gadoteridol. Segmented regions on MR images support rapid renal clearance. No significant difference in animal weight, necropsy, hematology, and chemistry was found between treatment and control groups at one month post-injection. Our nanoprobe based on an EGFR specific peptide labeled with UPIO designed for high stability and biocompatibility showed rapid tumor uptake and systemic clearance to demonstrate safety and promise for clinical translation to detect early HCC.

The Importance of Worker Reputation Information in Microtask-Based Crowd Work Systems

This paper presents the first systematic investigation of the potential performance gains for crowd work systems, deriving from available information at the requester about individual worker reputation. In particular, we first formalize the optimal task assignment problem when workers' reputation estimates are available, as the maximization of a monotone (submodular) function subject to Matroid constraints. Then, being the optimal problem NP-hard, we propose a simple but efficient greedy heuristic task allocation algorithm. We also propose a simple "maximum a-posteriori" decision rule and a decision algorithm based on message passing. Finally, we test and compare different solutions, showing that system performance can greatly benefit from information about workers' reputation. Our main findings are that: i) even largely inaccurate estimates of workers' reputation can be effectively exploited in the task assignment to greatly improve system performance;ii) the performance of the maximuma-posteriori decision rule quickly degrades as worker reputation estimates become inaccurate;iii) when workers' reputation estimates are significantly inaccurate, the best performance can be obtained by combining our proposed task assignment algorithm with the message-passing decision algorithm.

Diagnostic accuracy of osteopontin plus alpha-fetoprotein in the hepatocellular carcinoma: A meta-analysis

Objective: Osteopontin (OPN) has been reported as a potential biomarker for diagnosis of hepatocellular carcinoma (HCC) in many inconsistent results. This study demonstrates a systematic meta-analysis for the evaluation on diagnostic accuracy of serum or plasma OPN and alpha-fetoprotein (AFP) alone and combined assays for HCC.& para;& para;Methods: Relevant literatures were searched in PubMed up to August 2016. The quality of each study was evaluated by QUADAS-2 (quality assessment for studies of diagnostic accuracy). Statistical analysis was performed by Meta-Disc 1.4 and Stata 12.0. The random-effect models were used to estimate pooled sensitivity, specificity and other diagnostic indicators of OPN and/or AFP in HCC.& para;& para;Results: A total of 14 case-control literatures (15 studies) met the inclusion criteria in this meta-analysis. The respective pooled diagnostic sensitivity and specificity were 0.71 (95% Cl: 0.69-0.74) and 0.80 (95% Cl: 0.78-0.82) for OPN;0.61 (95% Cl: 0.58-0.63) and 0.92 (95% Cl: 0.91-0.94) for AFP;0.82 (95% Cl: 0.79-0.84) and 0.77 (95% Cl: 0.74-0.80) for OPN plus AFP. Their area under the curve (AUC) values were 0.8786, 0.8718 and 0.9005, respectively.& para;& para;Conclusion: Combination of OPN and AFP was better than OPN or AFP alone in diagnosis of HCC. MicroRNA-30b Suppresses Epithelial-Mesenchymal Transition and Metastasis of Hepatoma Cells

Epithelial-mesenchymal transition (EMT) is critical for induction of invasiveness and metastasis in HCC. Growing evidence indicates that upregulation of Snail, the major EMT inducer, significantly correlates with the metastasis and poor prognosis of HCC. Here, we investigate the underlying mechanism of miR-30b in suppressing metastasis of hepatoma cells by targeting Snail. In this study, we found that miR-30b was significantly downregulated and negatively associated with Snail production in HCC cell lines with higher metastatic potentials. Gain- and loss-of-function studies revealed that miR-30b could dramatically inhibit in vitro HCC cell migration and invasion. In vivo orthotopic liver xenograft model further demonstrated that stable over-expression of miR-30b significantly repressed the local invasion and lung metastasis of hepatoma cells. Meanwhile, the restoration of miR-30b expression suppressed the distant colonization of hepatoma cells. Both gain- and loss-of-function studies showed that miR-30b suppressed the EMT of hepatoma cells as indicated by the morphology changes and deregulation of epithelial and mesenchymal markers. Using RNAi, we further investigated the role of Snail in HCC cell EMT and demonstrated that knockdown of Snail significantly inhibited the EMT and cancer cell metastasis. Additionally, miR-30b exhibited inhibitory effects on HCC cell proliferation in vitro and in vivo. In conclusion, our findings highlight the significance of miR-30b downregulation in HCC tumor metastasis and invasiveness, and implicate a new potential therapeutic target for HCC metastasis. J. Anatomical versus non-anatomical liver resection for hepatocellular carcinoma exceeding Milan criteria

Background: Liver resection is effective for hepatocellular carcinoma (HCC) exceeding the Milan criteria in selected patients. However, the benefit of anatomical resection (AR) versus non-anatomical resection (NAR) has not been clarified in this patient subgroup. This study aimed to compare outcomes between AR and NAR for HCC exceeding the Milan criteria. Methods: Data on consecutive patients with HCC exceeding the Milan criteria who underwent liver resection with curative intent over a recent 6-year interval were extracted from a prospective single-centre HCC database and examined retrospectively. The postoperative outcomes of patients were compared before and after propensity score matching. Results: Some 546 patients were included: 264 in the AR and 282 in the NAR group. In the original cohort, the AR group contained more patients with larger tumours, multiple tumours, macroscopic portal vein tumour thrombi, incomplete tumour capsules and microscopic vascular invasion. After propensity score matching, 177 pairs of patients were selected. The baseline data, including liver function and tumour burden, were similar in the matched groups. The 3-year recurrence-free survival rate was comparable between the matched NAR and AR groups (36.5 versus 28.5 per cent;P = 0.448). Similar results were observed for 3-year overall survival (57.5 versus 50.3 per cent;P = 0.385), recurrence patterns and early recurrence rates (57.6 per cent versus 59.9 per cent;P = 0.712). Conclusion: AR and NAR achieved favourable and similar outcomes for HCC exceeding the Milan criteria in selected patients.
