People as a Service: A Mobile-centric Model for Providing Collective Sociological Profiles

Researchers from sociological disciplines could greatly benefit from collective information from the many people who use mobile devices to communicate via various social apps and services. However, processing that information is difficult because it's scattered among numerous social platforms. Furthermore, users are becoming increasingly concerned about how and by whom their information is being accessed. A new mobile-centric computing model allows sociological profiles of people to be generated, kept, and securely provided to third parties as a service. With this model, device owners can be fully aware and in control of how their information is accessed, while still contributing to collective sociological information. [ABSTRACT FROM PUBLISHER]

Stereotactic Body Radiation Therapy for Liver Metastases and Primary Hepatocellular Carcinoma: Normal Tissue Tolerances and Toxicity (vol 17, pg 111, 2010)

Background: There is growing interest in stereotactic body radiation therapy (SBRT) as a noninvasive means of treating inoperable hepatic metastases and primary intrahepatic hepatobiliary carcinomas. While initial outcomes are encouraging, the safety of delivering such large, ablative doses is still being studied. Methods: We compiled all dose-volume constraints from seven prospective trials of liver SBRT and linked them to reported toxicities. Dose thresholds were made isoeffective, and grade 3 or higher toxicities for liver and adjacent normal tissues were ***: Four cases of grade 3-5 radiation-induced liver disease (RILD) were identified, including 1 treatment-related death, from all patients treated for metastasis. Three of these 4 cases were linked to excessive radiation doses in a large volume of liver. In 56 patients treated for hepatocellular carcinoma (HCC), 1 case of grade 5 RILD and 2 cases of grade 2 hepatic toxicity were reported. Additionally, a prominent retrospective series reported 3 cases of grade 5 RILD in 9 patients treated for ***: SBRT appears to be safe for treatment of hepatic metastasis. The use of SBRT for HCC should be undertaken with caution or within the context of a clinical trial. Strict adherence to reported dose-volume constraints is advocated.

Long-term lamivudine therapy reduces the risk of long-term complications of chronic hepatitis B infection even in patients without advanced disease

Background: Long-term effects of lamivudine treatment on chronic hepatitis B patients without advanced disease remain unknown. Our aim was to investigate the effects of long-term lamivudine treatment and lamivudine-resistant virus (YMDD) on the development of cirrhosis and hepatocellular carcinoma (HCC) in asymptomatic patients without advanced disease. Methods: One hundred and forty-two hepatitis B e antigen (HBeAg)-positive patients (median age: 33.9 years) on long-term lamivudine (median treatment duration: 89.9 months) and 124 HBeAg-positive controls (median age: 33.4 years) were prospectively followed up. Patients were monitored for the development of cirrhosis and HCC, liver biochemistry, hepatitis B virus (HBV) DNA levels, HBeAg seroconversion and hepatitis flares. YMDD mutations (YMDD-MT) were determined annually. Results: Lamivudine-treated patients had a significantly lower cumulative rate of development of cirrhosis and/or HCC compared with controls (P=0.005). YMDD-MT occurred in 76.3% of patients after 8 years of lamivudine treatment. When compared with controls and patients with YMDD-MT, patients without YMDD-MT had the greatest reduction of HBV DNA and bilirubin levels, slowest decline of albumin level, highest rate of HBeAg seroconversion and lowest risk of hepatitis flare. Patients with YMDD-MT still had a lower risk for developing cirrhosis and/or HCC (P=0.024) and a greater HBV DNA reduction (P=0.001) in comparison with controls. Patients with YMDD-MT and controls had a similar chance of hepatitis flares and hepatic decompensation. Conclusions: Long-term lamivudine treatment was associated with a reduced chance of developing cirrhosis and HCC in patients without advanced disease. Although YMDD-MT reduced the benefits from lamivudine therapy, the outcome of these patients was still better than untreated patients.

Hepatocellular cancer (HCC) is most common primary liver malignancy in adults. Treatment for HCC is a multispecialty undertaking, with surgical, locoregional, and systemic options available. Choice of treatment depends upon patient and disease factors. Surgical therapy, including resection and transplantation, is the primary curative treatment and is best suited to patients with early disease. More advanced disease may be amenable to locoregional therapies to "bridge" to transplantation, downstage disease, or as destination therapy for unresectable cases. These include percutaneous ablation, transarterial therapy, external radiation, and radioembolization with yttrium-90 conjugated beads. Patients with more advanced disease may benefit most from systemic chemotherapeutic or small molecule inhibitor options available, many of which have only been recently FDA approved. Immunotherapy is the newest component of HCC treatment. The Y-90 consultant should be familiar with all modalities of HCC treatment and the interplay between them. (C) 2019 Elsevier Inc. Field cancerization currently described the theory of tumorigenesis and, until now, has been described in almost all organ systems except in liver. For this reason, we explore the presence of field cancerization in liver and its underlying clinical implication in hepatocellular carcinoma (HCC). In our study, methylation profile of HCC and surgically resected margin (SRM) were established by methylation-specific PCR. Liver cirrhosis (LC), chronic hepatitis and normal liver were treated in the same way as the background control. The correlation analysis among the methylation profile of HCC, SRM and clinicopathological data of HCC patients was made respectively. Our results showed that methylation abnormities related to HCC, but not background disease existed in histologically negative SRM. Monoclonal and polyclonal models may coexist in field cancerization in liver. Patients with RIZ1 methylation in SRM had a shorter disease free survival. The local recurrence trend of early and later recurrence in HCC is potentially related to a second field tumor. From these results, we can suggest that field cancerization exists in liver. The study of field cancerization in liver plays an important role in hepatocarcinogenesis. Second field tumor derived form field cancerization may have important implications in HCC prognosis assessment that is worthy of further study. (Cancer Sci 2009;100: 996-1004).

Emotional labor refers to the process in which workers are required to express emotions regardless of their actual feelings by the organization. In workplaces where such display rules exist, workers experience an emotional workload. Continued exposure to emotional workload can lead to severe mental and psychological issues. Nevertheless, research on assessing emotional workload remains understudied. In this study, we propose a machine learning model to automatically evaluate workers' emotional workload in emotional labor situations through multimodal sensing. The data collection study was designed based on a call center scenario. Within the study, we manipulated customer behaviors as confederates and assessed the worker's emotional workload. As a result, this study provides a benchmark using well-known features and standard machine learning methods. We achieved an accuracy of up to 87% for binary and three-class classification cases. Finally, we discuss the significance of assessing emotional workload and considerations for its practical application in the workplace.

Akt is frequently hyperactivated in human cancers and is targeted for cancer therapy. However, the physiological consequences of systemic Akt isoform inhibition were not fully explored. We showed that while combined Akt1 and Akt3 deletion in adult mice is tolerated, combined Akt1 and Akt2 deletion induced rapid mortality. Akt2(-/-) mice survived hepatic Akt1 deletion but all developed spontaneous hepatocellular carcinoma (HCC), which is associated with FoxO-dependent liver injury and inflammation. The gene expression signature of HCC-bearing livers is similar to aggressive human HCC. Consistently, neither Akt1(-/-) nor Akt2(-/-) mice are resistant to diethylnitrosamine-induced hepatocarcinogenesis, and Akt2(-/-) mice display a high incidence of lung metastasis. Thus, in contrast to other cancers, hepatic Akt inhibition induces liver injury that could promote HCC.

Lymphoblastoid alpha-interferon in the prevention of hepatocellular carcinoma (HCC) in high-risk HbsAg-positive resected cirrhotic HCC cases: A 14-year follow-up

Hepatocellular carcinoma (HCC) is a leading cause of death worldwide. It is also a common long-term complication of chronic hepatitis B or C infections, with cirrhosis as a risk factor for premalignant development. Natural lymphoblastoid alpha-interferon (alphaN1-IFN) has been widely used in treating chronic HBV and HCV carriers. To investigate its long-term beneficial effect in reducing the development of HCC, 20 hepatitis B surface antigen (HBsAg) positive resected cirrhotic HCC patients who have undergone chemotherapy followed by long-term treatment of 3MU alphaN1-IFN daily for 10 days every 3 months were analyzed in a 14 year follow-up. Results indicated that these patients survived and remained free of HCC. In contrast, 10 other patients receiving chemotherapy alone after resection eventually had HCC recur. Similar recurrence of HCC was seen in patients who had either reduced dosage of alphaN1-IFN or stopped interferon therapy or received 30MU dose at 6-month intervals. Our findings indicate the therapeutic potential and long-term safety of alphaN1-IFN in suppressing the development of HCC in high-risk patients.
