Cavoatrial thrombectomy in hepatocellular carcinoma with tumor thrombus in the vena cava and atrium without the use of cardiopulmonary bypass

Background Hepatocellular carcinoma (HCC) with tumor thrombus (TT) in hepatic vein, inferior vena cava (IVC), and right atrium (RA) portends a poor prognosis because of intravascular extension leading to rapid distal metastases. En bloc resection of cavoatrial TT without the use of cardiopulmonary bypass (CPB) is challenging. We describe a new method of vascular occlusion for thrombus entering into the RA without the need for CPB as shown in echocardiography. Methods and Results Retrospective analysis was carried out in 1 HCC patient, who had undergone hepatectomy with TT extension into IVC and RA. The infrahepatic IVC was occluded with vascular tape and the right atrial appendage was controlled with a Satinsky clamp proximal to the TT. The IVC wall was incised under direct vision from the infrahepatic IVC cranially. The tumor and thrombus were then removed en bloc under direct vision. Thus, cavoatrial thrombectomy was performed under total hepatic vascular exclusion without the use of CPB. He survived for 6 months and died of tumor recurrence. Conclusions Using this technique, advanced HCC with cavoatrial TT can be resected safely without CPB and thereby avoid dislodgement of TT and air embolism. © 2014 Elsevier Inc. All rights reserved.

Designing for psychological change: Individuals' reward and cost valuations in weight management

Knowledge of the psychological constructs that underlie behavior offers valuable design opportunities for persuasive systems. We use the decision theory, which describes how behavior is underpinned by reward-cost valuations, as a framework for investigating such psychological constructs to deliver design objectives for weight management technologies. Objective: We applied a decision theory-based analysis in the domain of weight management to understand the rewards and costs that surround individuals' weight management behaviors, with the aim of uncovering design opportunities for weight management technologies. Methods: We conducted qualitative interviews with 15 participants who were or had been trying to lose weight. Thematic analysis was used to extract themes that covered the rewards and costs surrounding weight management behaviors. We supplemented our qualitative study with a quantitative survey of 100 respondents investigating the extent to which they agreed with statements reflecting themes from the qualitative study. Results: The primary obstacles to weight management were the rewards associated with unhealthy choices, such as the pleasures of unhealthy foods and unrestricted consumption in social situations, and the significant efforts required to change habits, plan, and exercise. Psychological constructs that supported positive weight management included feeling good after making healthy choices, being good to oneself, experiencing healthy yet still delicious foods, and receiving social support and encouraging messages (although opinions about encouraging messages was mixed). Conclusions: A rewards-costs driven enquiry revealed a wide range of psychological constructs that contribute to discouraging and supporting weight management. The constructs extracted from our qualitative study were verified by our quantitative survey, in which the majority of respondents also reported similar thoughts and feelings. UACAP: A Unified Auxiliary Channel Authentication Protocol

Authenticating spontaneous interactions between devices and users is challenging for several reasons: the wireless (and therefore invisible) nature of device communication, the heterogeneous nature of devices, and lack of appropriate user interfaces in mobile devices, and the requirement for unobtrusive user interaction. The most promising approach that has been proposed in literature involves the exploitation of the so-called auxiliary channels for authentication to bridge the gap between usability and security. This concept has spawned the independent development of various authentication methods and research prototypes, that, unfortunately, remain hard to compare and interchange and are rarely available to potential application developers. We present a novel, unified cryptographic authentication protocol framework (UACAP) to unify these approaches on using auxiliary channels and analyze its security properties. This protocol and a selection of auxiliary channels aimed at authentication of mobile devices has been implemented and released in an open-source ubiquitous authentication toolkit (OpenUAT). We also present an initial user study evaluating four of these channels.

Measuring Privacy and Utility in Privacy-Preserving Visualization

In previous work, we proposed a technique for preserving the privacy of quasi-identifiers in sensitive data when visualized using parallel coordinates. This paper builds on that work by introducing a number of metrics that can be used to assess both the level of privacy and the amount of utility that can be gained from the resulting visualizations. We also generalize our approach beyond parallel coordinates to scatter plots and other visualization techniques. Privacy preservation generally entails a trade-off between privacy and utility: the more the data are protected, the less useful the visualization. Using a visually-oriented approach, we can provide a higher amount of utility than directly applying data anonymization techniques used in data mining. To demonstrate this, we use the visual uncertainty framework for systematically defining metrics based on cluster artifacts and information theoretic principles. In a case study, we demonstrate the effectiveness of our technique as compared to standard data-based clustering in the context of privacy-preserving visualization.

Positive Affective Interactions: The Role of Repeated Exposure and Copresence

We describe and evaluate a new interface to induce positive emotions in users: a digital, interactive adaptive mirror. We study whether the induced affect is repeatable after a fixed interval (Study 1) and how copresence influences the emotion induction (Study 2). Results show that participants systematically feel more positive after an affective mirror session, that this effect is repeatable, and stronger when a friend is copresent.

Loose ontological coupling and the Social Semantic Web

Best practices for the publication of Semantic Web data currently place an unacceptably high burden on the end-user, who is supposed to locate and embrace third-party, ontological structures prior to publishing any information. This paper argues for a different publication paradigm where end-users are encouraged to publish potentially incomplete or conflicting information according to their own local context, and where heterogeneous data is consolidated a posteriori through bottom-up, decentralized processes. This approach has two key advantages: first, it greatly simplifies the publication of semantic information by allowing users to contribute purely local data, without any consideration for global schemas or third-party structures. Second, it takes advantage of expressive, semantic links to relate both schemas and instances, creating complex webs of knowledge where cultural or social differences can flourish and be confronted one another.

Low cyclin F expression in hepatocellular carcinoma associates with poor differentiation and unfavorable prognosis

Cyclin F, capable of forming Skp1-Cul1-F-box protein ubiquitin ligase complex, is implicated in controlling centrosome duplication and preventing genome instability. Cyclin F oscillates during cell cycle with a similar pattern to cyclin A. However, its expression and significance in cancer remain obscure. In this study, we showed that cyclin F was noticeably decreased in 16 pairs of tissue samples of hepatocellular carcinoma (HCC) compared to paracarcinoma tissues, at both mRNA and protein levels. Immunohistochemical staining data revealed that in 71.8% (176/245) of HCC cases, cyclin F expression in tumor tissue was much lower than that in nontumorous tissue. Low cyclin F expression, defined by receiver operating characteristic curve analysis, was present in 69.0% of HCC patients. Low expression of cyclin F was significantly correlated with tumor size, clinical stage, serum alpha-fetoprotein level and tumor multiplicity. Further study showed that cyclin F expression was reversely associated with tumor differentiation in HCC. KaplanMeier analysis indicated that low cyclin F expression was related to poor overall survival and recurrence-free survival. The prognostic impact of cyclin F was further confirmed by stratified survival analysis. Importantly, multivariate analysis revealed that low cyclin F expression was an independent poor prognostic marker for overall survival. We conclude that cyclin F is downregulated in HCC and is a promising prognostic marker for patients suffering from this deadly disease. (Cancer Sci 2013;104: 508-515)

Multivariate Analysis of Prognostic Factors for Survival following Doxorubicin-eluting Bead Transarterial Chemoembolization for Hepatocellular Carcinoma

Purpose: To identify prognostic factors for survival in patients with hepatocellular carcinoma (HCC) treated with transarterial chemoembolization with doxorubicin-eluting beads (DEBs). Materials and Methods: In a retrospective;single-center analysis, tumor- and patient-related factors were recorded for univariate and multivariate analyses via Kaplan-Meier and Cox regression. Infiltrative HCC phenotype and portal vein. invasion (PVI) were correlated, and patients with either or both were classified as having radiographically advanced (RAdv) HCC. The primary endpoint was overall survival, which was calculated from the time of first DEB Chemoembolization procedure. Results: A total of 168 patients underwent 248: procedures, of which 215 (86.7%) were outpatient procedures. Mean length of stay was 0.33 days, and 25 patients (10.1%) were readmitted within 30 days. A total of 33 patients underwent liver transplantation and were excluded from survival analyses. A total of 150 had cirrhosis;62, 50, and 18 had Child class A, B, and C disease, respectively. Forty-one patients had infiltrative HCC phenotype, 28 of whom also had PVI. Multivariate analysis of survival in all patients showed, alpha-fetoprotein (AFP), performance status (PS) RAdv HCC, Child classification, albumin level, and ascites to predict survival: In patients without RAdv HCC AFP, PS, Child classification, albumin level, and International :Normalized Ratio were independent predictors. Increased bilirubin level was not an-independent risk factor for death. Conclusions: Independent prognostic factors in patients with HCC undergoing DEB chemoembolization have been: identified. Increased bilirubin level Was not an independent risk factor. These data can he used in HCC patient selection and counseling for DEB chemoembolization.

Presurgery serum osteopontin (OPN) level has been demonstrated to correlate to tumor recurrence and survival of hepatocellular carcinoma (HCC) patients. This study investigated the postoperative dynamic changes of serum OPN level and its clinical significance in HCC patients. Presurgery serum OPN levels were measured by enzyme-linked immunosorbent assay in cohort A of 179 HCC patients and were compared with the multiple controls including different kinds of liver diseases and healthy individuals. In cohort B of 110 patients with resectable HCCs, besides preoperative assays, serum OPN was monitored at 1 week, 1, and a parts per thousand yen2 months after operation. The baseline presurgery serum OPN of HCC patients was significantly higher than that of the patients with the other kinds of liver diseases (p < 0.0001). The prognostic values of presurgery serum OPN level in HCC patients were further confirmed. The postsurgery OPN levels were significantly elevated within 1 week after HCC resection, then decreased at 1 month and reached the nadir later than 2 months after operations. It increased again at the time of tumor recurrence, then declined after the second removal of recurrent HCCs. Moreover, postoperative OPN in alpha-fetoprotein-negative and -positive HCC patients had the same changing pattern;it only correlated to liver function and C-reactive protein level. After a transient fluctuation, serum OPN levels significantly decrease after curative resection of HCCs. Postoperative serum OPN could serve as a surrogate serologic biomarker for monitoring treatment response and tumor recurrence after HCC resection, including alpha-fetoprotein-negative ones.
