Predictors and Cumulative Frequency of Hepatocellular Carcinoma in High and Intermediate LI-RADS Lesions: A Cohort Study from a Canadian Academic Institution

BackgroundThe frequency and predictors of hepatocellular carcinoma (HCC) within each liver imaging reporting and data system (LI-RADS) category remains unclear. We sought to estimate the cumulative frequency of HCC in LI-RADS observations of high/intermediate category and identify clinical/radiographic features associated with *** diagnostic imaging database was searched for computed tomography/magnetic resonance imaging reports of patients with evidence of cirrhosis and liver observations. LI-RADS categories were determined by imaging review, while demographic and clinical outcomes were assigned by chart review. A composite outcome of clinical/radiographic confirmation of HCC was used. We used multivariable analysis to identify features associated with HCC, and competing risks regression to estimate the cumulative frequency of HCC in each *** search returned 95 patients with 137 observations (LR2=4, LR3=53, LR4=37, and LR5=43). On multivariable analysis, increasing age (hazard ratio [HR] 1.76 per 10years, p=0.049), washout (HR 5.34, p<0.002), and increasing size (size<10mm reference, 10-20mm, HR 3.93, p=0.014;size>20mm, HR 21.69, p<0.001) were associated with HCC. Median time to diagnosis was 6.13months (interquartile range [IQR] 4.6-13.1), 4.7months (IQR 2.5-14.5), and 3.6months (IQR 1.9-6.6) for LR3, 4, and 5 category observations, respectively. The cumulative frequency of HCC was 59.8% in LR3, 84.62% in LR4, and 99.84% in LR5, at last *** frequency of HCC within each LI-RADS category reflects the intended purpose, intermediate probability for LR3, probable HCC for LR4, and definite HCC for LR5.

Short-term outcomes after major hepatic resection in patients with cirrhosis: a 75-case unicentric western experience

Background: The benefit of performing major hepatic resection (MHR) for hepatocellular carcinoma (HCC) in patients with cirrhosis remains controversial because of its high risk of posthepatectomy liver failure (PHLF). This study was conducted to assess the risk of MHR for HCC in patients with cirrhosis. Methods: Patients with Child-Pugh A or B cirrhosis and HCC who underwent MHR from January 2000 to June 2014 were retrospectively identified. Risk factors for postoperative morbidity and mortality using univariate and multivariate analyses were evaluated. Results: Seventy patients with Child-Pugh A (93%) and 5 (7%) with Child-Pugh B cirrhosis underwent MHR for HCC. Thirteen (17%) had Barcelona Clinic Liver Cancer (BCLC) stage A, 39 (50%) had BCLC B, and 23 (32%) had BCLC C disease. A perioperative blood transfusion was performed in 18 patients (24%). Ninety-day postoperative mortality was 9% (n=7). Major complications occurred in 16 patients (21%), including PHLF in 9 patients (12%). A multivariate analysis showed that perioperative blood transfusion was the main independent factor associated with mortality (OR=6.5) and major morbidity (OR=10). Conclusion: In selected patients with HCC and cirrhosis, MHR is feasible and has acceptable mortality, but careful perioperative management and limiting blood loss are required.

A Social Crowdsourcing Community Case Study: Interaction Patterns, Evolution, and Factors That Affect Them

A social crowdsourcing community (SCC) is a moderated online community where members participate in activities designed to elicit their opinions concerning topics related to products or services. This emerging paradigm of human computation systems is distinct in which it combines the powers of crowdsourcing and social networking to allow for systematic querying of crowds and synthesizing response data (i.e., contributions) into coherent reports for decision-makers. Since the SCC paradigm is relatively new, we apply methods from social network analysis and related fields to help answer important questions relevant to SCCs. We demonstrate the applicability of these methods by analyzing five years of interactions in a private SCC starting from its inception. This analysis reveals the patterns in members' interactions within the SCC and key factors that affect them. In addition, the analysis reveals the patterns in the evolution of these interactions.

Multi-modal recognition of worker activity for human-centered intelligent manufacturing

This study aims at sensing and understanding the worker's activity in a human-centered intelligent manufacturing system. We propose a novel multi-modal approach for worker activity recognition by leveraging information from different sensors and in different modalities. Specifically, a smart armband and a visual camera are applied to capture Inertial Measurement Unit (IMU) signals and videos, respectively. For the IMU signals, we design two novel feature transform mechanisms, in both frequency and spatial domains, to assemble the captured IMU signals as images, which allow using convolutional neural networks to learn the most discriminative features. Along with the above two modalities, we propose two other modalities for the video data, i.e., at the video frame and video clip levels. Each of the four modalities returns a probability distribution on activity prediction. Then, these probability distributions are fused to output the worker activity classification result. A worker activity dataset is established, which at present contains 6 common activities in assembly tasks, i.e., grab a tool/part, hammer a nail, use a power-screwdriver, rest arms, turn a screwdriver, and use a wrench. The developed multi-modal approach is evaluated on this dataset and achieves recognition accuracies as high as 97% and 100% in the leave-one-out and half-half experiments, respectively.

In one of the earlier essays in this department, we discussed a number of acronyms all having to do with system design, and all having the form \"x-centered design.\" The purpose of that essay was to demonstrate a broad framework within which to understand human-centered computing (HCC), and also to show the various convergences and divergences of the communities of practice that have introduced their own x-centered-design designations. Among them are learner-centered design, client-centered design, designer-centered design, decision-centered design, and work-oriented design.

Hematochezia From Metastasis of Hepatocellular Carcinoma to Colon in a Patient Who Underwent Liver Transplantation

A 50-year-old woman was admitted to the hospital with a 3-day history of hematochezia. She had been diagnosed with cryptogenic cirrhosis and hepato-cellular carcinoma (HCC] 4 years before presentation and underwent an orthotopic liver transplant. A 3-cm mass was discovered in the left lateral segment of the explant. Pathology showed poorly differentiated HCC with invasion of small vascular spaces and the liver capsule (Figure A). Immunohistochemistry demonstrated positivity for cytokeratin 7 and a canalicular staining pattern with CEA. CD 10 and CD 19 stains were negative.

Arterial Patency after Repeated Hepatic Artery Bland Particle Embolization

PURPOSE: To evaluate hepatic arterial patency after serial bland particle embolization procedures in patients with hepatocellular carcinoma (HCC). MATERIALS AND METHODS: All patients with HCC who underwent five or more hepatic artery bland embolization procedures with permanent particulate and spherical embolic agents between September 1996 and December 2007 were retrospectively reviewed. Data analyzed included patient demographics, vessels embolized, embolic agent used, and duration of arterial patency. RESULTS: Forty-three patients were identified who underwent five or more bland embolization procedures in the same arterial distribution. Of the 43 patients examined, 83% (n = 36) showed no change in the hepatic arterial tree after repeated bland embolization (mean treatment period of 48 months +/- 23). Six patients (13%) exhibited occlusion of a fifth-order or more distal vessel after an average of 5 embolizations +/- 2 over a period of 34 months +/- 27. A single case of vascular occlusion of a fourth-order vessel was observed after five embolizations over a period of 52 months. CONCLUSIONS: Repeated bland embolization of the hepatic arteries in HCC preserves patency of the hepatic arterial vasculature despite the fact that embolization is carried out to complete stasis.

Nano-carriers for delivery and targeting of active ingredients of Chinese medicine for hepatocellular carcinoma therapy

Active ingredients from Chinese medicine (AIFCM) are emerging as a class of anticancer agents for treating hepatocellular carcinoma (HCC). AIFCM have unique advantages over traditional chemotherapeutic drugs, such as their multi-target, multi-level, and coordinated intervention effects against HCC. Recent researches demonstrated their potentials in inhibiting HCC initiation, cell proliferation, angiogenesis, and metastasis. With the advantages of nano-carriers including better bioavailability, significant features of passive or active targeting and stimulus-responsive drug release. Formulation of AIFCM and nano-carriers shows great potential in promoting anti-HCC therapeutic development. Here, we critically assess nano-carriers for delivery and targeting of AIFCM for HCC therapy. First, we offer an overview of major pathogenic factors to induce HCC, including virus infection, alcoholic liver disease, mycotoxin, obesity, and type 2 diabetes. Second, the mechanisms of AIFCM for HCC therapy comprising gene regulation and transduction of different molecular pathways are discussed. Third, significant achievements of AIFCM-encapsulated nano-carriers and AIFCM-modified nano-carriers against HCC are presented. Finally, we provide concluding remarks and future perspectives of AIFCM for HCC therapy, hoping to offer some insight on how to improve AIFCM-based HCC therapy.

Simulating human-centered pervasive systems requires accurate assumptions on the behavior of human groups. Recent models consider this behavior as a combination of both social and spatial factors. Yet, establishing accurate traces of human groups is difficult: current techniques capture either positions, or contacts, with a limited accuracy. In this paper we introduce a new technique to capture such behaviors. The interest of this approach lies in the unprecedented accuracy at which both positions and orientations of humans, even gathered in a crowd, are captured. The open-source software pipeline we developed to exploit captured data allows extraction of several metrics on movement and social contacts, and permits study of their respective interrelationship. From the mobility to the topological connectivity, this framework offers a layered approach that can be tailored, allowing to compare and reason about models and traces. We demonstrate the accuracy and validity of our approach on social events and calibration runs in which we captured the motions of humans. In particular, we introduce an open-access trace of 50 individuals and compare it against random waypoint models that have the same global characteristics. Our fine-grain analyses, that take into account social interactions between users, show that the random way point model does not provide accurate predictions for socially-induced motion;to model human kinetics, new group- and interaction-based models should be developed. From the computer science point of view, these models are required to fully exploit the power of human-centered mobile computing, crucial for ubiquitous computing, and referred to as Short Range Communication Systems, Mobile Opportunistic Networking, or Mobile Networking in Proximity. UbiPaPaGo: Context-aware path planning

The increased prevalence of digital devices with communication capability heralds the era of ubiquitous computing, as predicted by Mark Weiser. Ubiquitous computing aims to provide users with intelligent human-centric context-aware services at anytime anywhere. Optimal path planning in a ubiquitous network considers the needs of users and the surrounding context. This approach is very different from that applied by existing research on car navigation and mobile robots. This study proposes a context-aware path planning mechanism based on spatial conceptual map (SCM) and genetic algorithm (GA), referred to as UbiPaPaGo. The SCM model is adopted to represent the real map of the surrounding environment. The optimal path is planned using a GA, which is a robust metaheuristic algorithm. UbiPaPaGo attempts to automatically find the best path that satisfies the requirements of an individual user. A prototype of UbiPaPaGo is implemented to demonstrate its feasibility and scalability. Experimental results validate the effectiveness and the efficiency of UbiPaPaGo in finding the optimal path. (C) 2010 Elsevier Ltd. All rights reserved.
