[目的]了解本医院住院患者鲍曼不动杆菌的耐药性变迁,为临床合理使用抗菌药物提供依据.[方法]对2007～2010年本院住院患者各类标本分离的鲍曼不动杆菌进行回顾性统计分析.[结果]近4年来,鲍曼不动杆菌感染占临床分离革兰阴性杆菌的构成比逐年增加（P〈0.01）,对抗菌药物的耐药性显著提高,但亚胺培南、多粘菌素对鲍曼氏不动杆菌仍有较好的抗菌作用（耐受率〈30%）.[结论]鲍曼不动杆菌是临床病原菌的重要组成之一,其对临床常用抗菌药物的耐药性呈逐年上升趋势,多重耐药菌比例明显增加.应重视合理使用抗生素。

Isolation of an Acinetobacter sp ST-550 which produces a high level of indigo in a water-organic solvent two-phase system containing high levels of indole

Acinetobacter sp. strain ST-550 was isolated from fumus soil as an efficient indigo producer in the presence of organic solvent. The minimum inhibitory concentration of indole was 0.4 mg/ml for ST-550. ST-550 produced only a small amount of indigo (less than 0.01 mug/ml) when grown in the presence of indole at concentrations of 0.05 to 0.3 mg/ml without any organic solvent. However, ST-550 produced indigo effectively when grown in the presence of a large: volume of diphenylmethane and a high level of indole;optimized conditions were 3 ml of a medium containing 0.3 ml diphenylmethane and 2.7 mg indole. Under these conditions, ST-550 produced 0.88 mg indigo (292 mug/ml medium).

Nosocomial bloodstream infections caused by Acinetobacter species in United States hospitals:: Clinical features, molecular epidemiology, and antimicrobial susceptibility

We examined the clinical and epidemiological features of nosocomial bloodstream infections (BSIs) caused by Acinetobacter species and observed from 1 March 1995 through 28 February 1998 at 49 United States hospitals (SCOPE National Surveillance Program). Acinetobacter species were found in 24 hospitals (49%) and accounted for 1.5% of all nosocomial BSIs reported. One hundred twenty-nine isolates were identified either as A. baumannii (n = 111) or other Acinetobacter species (n = 18). Patients with A. baumannii BSI, compared with patients with nosocomial BSI caused by other gram-negative pathogens, were more frequently observed in the intensive care unit (69% vs. 47%, respectively;P < .001;odds ratio [OR] 2.4;95% confidence interval [CI] 1.6-3.7) and were more frequently receiving mechanical ventilation (58% vs. 30%, respectively;P < .001;OR 3.2;95% CI 2.1-4.8). Crude mortality in patients with A. baumannii BSI was 32%. Molecular relatedness of strains was studied by use of polymerase chain reaction-based fingerprinting. Clonal spread of a single strain occurred in 5 hospitals, Interhospital spread of epidemic A. baumannii strains was not observed, The most active antimicrobial agents against A. baumannii (90% minimum inhibitory concentration values) were imipenem (1 mg/L;100% of isolates susceptible), amikacin (8 mg/L;96%), tobramycin (4 mg/L;92%), and doxycycline (4 mg/L;91%), Thirty percent of isolates were resistant to greater than or equal to4 classes of antimicrobials and were considered to be multidrug resistant.

Bacteria associated with butterfly stain of Chilean tepa

Butterfly stain, common in Chilean tepa (Laureliopsis philippiana [Looser] Schodde) trees, appears in cross-section of the stem as a series of partially overlapping orange-brown arcs that in early stages resembles a butterfly. Bacterial isolates from stained tepa wood samples cultured under aerobic and anaerobic conditions were identified using several commercially available identification systems. Pseudomonas sp. were most common in the cultures handled aerobically, and Clostridium sp. were found in the cultures handled anaerobically. A pectinolytic Bacillus sp. and a Gram-negative rod-shaped bacterium that emitted a strong naphthalene-like odor similar to that of stained wood were also isolated. Not all isolates could be identified. This is the first report of Clostridium from tepa.

Phenotypic characterization and antibiotic resistance of Acinetobacter spp. isolated from aquatic sources

A total of 99 Acinetobacter isolates from sewage, freshwater aquaculture habitats, trout intestinal contents and frozen shrimps was characterized phenotypically and antibiotic susceptibility patterns determined. One group of genomic species, including Ac. johnsonii, Ac. lwoffi and spp. 15TU, was detected in all sample types and represented the majority of the isolates (n = 54). Isolates belonging to the Acb complex (Ac. calcoaceticus, Ac. baumannii and genomic species 3) were detected in sewage (n = 6) and frozen shrimps (n = 1), Ac. haemolyticus in frozen shrimps (n = 6) and trout intestinal contents (n = 2) and genomic species 11 in freshwater aquaculture habitats (n = 6) and trout intestinal contents (n = 1). Acinetobacter junii (n = 5), genomic species 10 (n = 2), 14BJ (n = 8) and 16BJ (n = 4) were only isolated from sewage. Acinetobacter isolates from sewage were generally more biochemically reactive and resistant to antimicrobial agents compared with isolates from other sample types. Different strains, often belonging to different genomic species, were isolated from sites situated upstream and downstream of the discharge point of a pharmaceutical plant. This finding supported the hypothesis that the waste effluent from the pharmaceutical plant was likely to cause a change in the distribution of Acinetobacter spp. by selecting and/or introducing antibiotic-resistant strains into the recipient sewers.

VITRECTOMY AND INTRAOCULAR GENTAMICIN MANAGEMENT OF HERELLEA ENDOPHTHALMITIS AFTER INCOMPLETE PHACOEMULSIFICATION

A patient with Herellea endophthalmitis after incomplete phacoemulsification was treated with vitrectomy and intravitreal infusion of 8 μg/ml of gentamicin and 400 μg of dexamethasone. The vitrectomies were done through a pars plana incision wth the vitrophage. Additionally, appropriate systemic antibiotics and prednisone were given.

ACINETOBACTER ANITRATUS INFECTIONS IN MAN

During a period of 17 months, 142 strains of Acinetobacter anitratus were isolated from 140 patients. They were examined for biochemical characteristics, antibiotic susceptibilities to 15 chemotherapeutic drugs and clinical and epidemiologic features. Biochemical studies were necessary for positive identification of this gram negative rod. Many isolates were susceptible to kanamycin, gentamicin, polymixin B, cotrimoxazole and nalidixic acid. Most isolates were hospital acquired and had been cultured from a number of anatomic sites in the presence of a variety of clinical situations. Acinetobacter anitratus was the primary infecting organism in two cases of septicaemia (one fatal), two pneumonias, two wound infections and six urinary tract infections. Because of its potential pathogenicity this organism should not be dismissed as a harmless commensal by laboratory staff and clinicians.
