This article seeks to define the principles that are at the heart of the concept of children's hospice care and to indicate the wide range of activities to which that concept is relevant. The discussion is organized around five main points: 1) an analysis of that which is distinctive of the hospice philosophy or hospice-like approaches to care;2) a sketch of situations involving children to which approaches guided by such a philosophy are or might be relevant;3) a description of the principal contexts within which these approaches might be implemented;4) some suggestions concerning the application of ideal models to practical realities;and 5) some concluding remarks concerning issues for practice and research. References to relevant literature indicate the present status of work in this field.

The transprofessional model: Blending intents in terminal care of AIDS

Current terminal care services present dying patients and their families with a dichotomy in service delivery and the intentcare between curative treatments and palliative treatments. This arbitrary dichotomy reduces patients’ quality of life in many cases androbs patients and families of benefiting from the psychosocial aspects of treatment until the last few weeks of life. This article presents a blended model of care, the Transprofessional Model, inwhich patients receive both curative and palliative service throughout their care process. The blended intent model differs from traditional home care in that services are provided by a care coordination team composed of nurses and social workers;the traditional model of care is often case managed by a single, registered nurse. The combination of the multi-disciplinary approach to care coordination andtraining in both curative and palliative services in the Transprofessional Model demonstrates that this blended model of care producesa bio-psychosocial focus to terminal care as compared to a primaryfocus on curative services present in the traditional model of homecare. © 1998, Taylor & Francis Group, LLC. New initiatives transforming hospice care.

Hospice care has been successful in serving a large segment of the terminally ill population in the United States. This article addresses a number of significant trends that may impact the future of hospice care. It is proposed that as many as one-third of those who die will not be in a position to make use of any end-stage program of care. Of the remaining, some will have difficulty being served by hospices due to uncertain prognosis and continued efforts at curative treatment. New models of caring for chronically terminally ill persons are being developed and are reviewed. A clearer definition of who ought to be served by hospice programs is encouraged.

Empowerment through information: supporting rural families of oncology patients in palliative care.

A research project examining the support needs of families caring for a relative in palliative care was conducted in New South Wales in 1997. Data were collected from 19 families and 10 specialist palliative care nurses from eight centres throughout New South Wales using audio-taped interviews. The findings show that information was one of the most important support needs for families to enable them to effectively care for their sick relative. The information provided needed to meet the individual and varying needs of families at different stages of their palliative care journey. Often the families described not knowing what their information needs were until a crisis occurred. Lack of information on a range of areas, from practical resources to providing physical care and managing medications, was problematic. Health professionals need to take responsibility in determining if families are aware of the information available, identify the most appropriate medium for this material and provide it at the pace and time wanted by the individual family.

Issues influencing the provision of palliative care services to remote aboriginal communities in the Northern Territory.

Palliative care is a service available to many Australians subject to location and varying criteria. This paper seeks to identify some of the issues that hamper the provision of this sophisticated service to Aborigines living in remote parts of the Northern Territory. The paucity of literary sources of information demonstrates the need for research to be carried out in this field.
