PRELIMINARY-OBSERVATIONS ON THE DISTRIBUTION OF IVERMECTIN IN NIGERIA FOR CONTROL OF RIVER-BLINDNESS

A total of 118 925 individuals in four Nigerian states was treated for onchocerciasis between February and December, 1991, using centralized and house-to-house distribution of ivermectin. Pre-treatment prevalences of the disease ranged between 28% and 90%. Only 0·7% of those treated reported adverse reactions within three days of treatment: 230 individuals (0·19%) had headache, 210 (0·17%) general body pains, 150 (0·12%) pruritis, 120 (0·10%) oedema, 80 (0·06%) fever, 20 (0·02%) dizziness, 15 (0·01%) vomiting, 10 (0·01%) diarrhoea, and 25 individuals (0·02%) noticed that they passed intestinal *** in the endemic communities continues. The results show that mass treatment of onchocerciasis with ivermectin is quite safe and the drug's acceptability increases its potential as the drug of choice for control of onchocerciasis in Nigeria.

Community-directed, ivermectin-treatment programmes for onchocerciasis control in Uganda: the selection and validation of indicators for monitoring sustainability at the district level

The selection and validation of indicators for predicting and monitoring sustainment in community-directed, ivermectin-treatment programmes (CDITP) at the community level in the Kabale, Kisoro and Rukungiri districts of Uganda have already been reported. The aim of the present study was to select and validate similar indicators at the district level, in the same districts over the same 4-year period. Three dependent-variable scales of programme sustainability (PS), PS1, PS2 and PS3, were compared by district. As at the community level, Rukungiri district clearly performed better than Kabale or Kisoro. Cost variables compiled at the district level and the ratios of numbers of community-based distributors (CBD) to community members were used as input variables in regression and correlation models, with PS1, PS2, and PS3 as outcome variables. In the regression model, cost of training CBD was found to be statistically significant (P = 0.0186). This variable also scored 100% on a scale for programme-indicator sensitivity and hence was selected as a helpful indicator. In the correlation model, cost of health education of community members had a weak relationship with PS1 (P = 0.0662). Cost of training CBD had a significant negative correlation with PS2 (P = 0.0186), indicating that reducing the cost of training would facilitate sustainability. PS3 showed weak negative correlations with cost of health education of community members (P = 0.0586) and cost/person treated in the district (P = 0.0584). Sustainment of CDITP might be better, therefore, if the costs per person could be reduced. As correlation relationships may not be linear, however, they were not considered particularly useful in the selection of helpful indicators.

Community financing of local ivermectin distribution in Nigeria: potential payment and cost-recovery outlook

The preferred payment mechanism in a community financing scheme for local ivermectin distribution was elicited from randomly selected household heads from three communities in Nigeria using interviewer-administered structured questionnaires. The majority of the respondents in the three communities were prepared to pay for local ivermectin distribution. Additionally, the average amounts the respondents were prepared to pay per person treated ($0.28, $0.30 and $0.38 in Nike, Achi and Toro, respectively) were all more than the $0.20 ceiling recommended by the partners of the African Programme on Onchocerciasis Control (APOC). Thus, the cost-recovery outlook is bright in these communities. However, the preferred payment modality varied. Fee-for-service was the predominant payment modality in the Achi and Nike communities, while the Toro community preferred pre-payment. This study demonstrates that many communities have different payment preferences for endemic disease control efforts. This knowledge will help in developing acceptable and sustainable schemes. The imposition of unacceptable payment mechanisms will lead to an unwillingness to pay.

Strategic control of gastrointestinal nematode and lungworm infections with eprinomectin at turnout and eight weeks later

Two groups of 10 first-year grazing cattle were either left untreated as controls (group 1) or treated with eprinomectin as a pour-on application at turnout and eight weeks later (group 2). The control group developed a mild infection with gastrointestinal nematodes and lungworms during the season, wheares the treated animals remained healthy. The interval between the treatments allowed the establishment of adult worms, but the egg counts remained negligible. The total number of eggs shed by the treated cattle during the grazing season was significantly smaller than by the controls.

Effects of the multidrug-resistance-reversing agents verapamil and CL 347,099 on the efficacy of ivermectin or moxidectin against unselected and drug-selected strains of Haemonchus contortus in jirds (Meriones unguiculatus)

The development of anthelmintic resistance is making parasite control in small ruminants problematic. Following the discovery that the drug transporter P-glycoprotein may be involved in macrocyclic lactone resistance in Haemonchus contortus, we determined the effect of two multidrug-resistance modulators, verapamil and CL347,099, on the efficacy of ivermectin and moxidectin against unselected and drug-selected strains of H. contortus. CL347,099 is an analog of verapamil that has multidrug-resistance properties but weaker calcium-channel-blocking activity than the parent drug. The combinations of verapamil with either ivermectin or moxidectin significantly reduced worm counts of the selected strains as compared with the untreated controls, whereas ivermectin or moxidectin alone did not significantly reduce worm counts as compared with the untreated controls. The CL347,099 plus moxidectin combination was significantly more efficacious than moxidectin alone against the ivermectin-selected strain. The drug-combination regimes were without adverse effect on the jirds. However, higher levels of verapamil (greater than or equal to 40 mg/kg) produced some toxicity.

This paper reviews the issues relating to compliance and participation among the men and women of three countries within the remit of the African Programme for the Control of Onchocerciasis (APOC): Cameroon, Nigeria and Tanzania. Project-monitoring data from 109 focus-group discussions, 6069 household-survey respondents and 89 interviews with ivermectin distributors were analysed to gain an insight into the attitudes and behaviours of men and women in relation to ivermectin treatment and their participation in the programme. Although there are no statistically significant gender differences in coverages for ivermectin treatment, culturally prescribed gender relationships influence the ways in which men and women express and experience treatment-related behaviours. Gender roles also affect participation in the programme. Decision-making in communities on the selection of distributors tends to follow socio-cultural hierarchies based upon patriarchy and gerontocracy. Relatively few ivermectin distributors (21%) are women. Although they receive less support than their male counterparts, the female distributors are just as willing to continue ivermectin distribution in the community, and they perform as well or better than men in this regard. The terms 'community-directed', 'community participation, and even 'compliance' obfuscate important gender differences that are inherent in the implementation of onchocerciasis control. Development of strategies that recognize these gender differences will have important implications for long-term adherence to treatment and for the overall quality and sustainability, of the programme.

Ivermectin distribution using community volunteers in Kabarole district, Uganda

Ivermectin mass distribution for the control of onchocerciasis in Uganda began in 1991. This report describes a community based ivermectin distribution programme covering two foci in the Kabarole district which have an estimated 32 000 persons infected and another 110 000 at risk. Through nodule palpation in adult males, 143 villages were identified where nodule prevalence exceeded 20%. Skin snips were also taken from a sample of the population to measure changes in community microfilarial load (CMFL) with treatment. The delivery programme was integrated into the district health management structure, and used community volunteers supervised by medical assistants from adjacent heath facilities for annual ivermectin distribution campaigns. After initial efforts by the community to support distributors in-kind proved inadequate, ivermectin distributors earned money retailing condoms as part of the social marketing component of a district STD/AIDS programme. Reduction in the CMFL ranged from 40-62 % twelve months after the second ivermectin treatment in three villages, and from 69-84% six months after the fourth round of treatment in two villages. After four years of treatment, 85% of eligible persons were receiving ivermectin from community volunteers in each treatment cycle. Drop out rates among volunteers did not exceed 20% over the four years reported here. The direct cost of treatment was US$0.29 per person. Among the reasons for low per-person treatment costs were the strong supervisory structure, the presence of health centres in the foci and a well developed and capable district Primary Health Care management team.
