Alpha-adrenoceptor blockade prevents exercise-induced vasoconstriction of stenotic coronary arteries

OBJECTIVES The study aimed to evaluate the role of alpha-adrenergic mechanisms during dynamic exercise in both normal and stenotic coronary arteries. BACKGROUND Paradoxical vasoconstriction of stenotic coronary arteries has been reported during dynamic exercise and may be due to several factors such as alpha-adrenergic drive, a decreased release of nitric oxide, platelet aggregation with release of serotonin, or a passive collapse of the vessel wall. METHODS Twenty-six patients were studied at rest, during two levels of supine bicycle exercise and after 1.6 mg sublingual nitroglycerin The alpha-blocker phentolamine was given to 16 patients before exercise, five of whom had also taken a beta-adrenergic-blocker the same morning. Ten patients served as controls. The cross-sectional areas of a normal and a stenotic coronary vessel were determined by biplane quantitative coronary arteriography. RESULTS In the normal vessel segments, coronary cross-sectional area did not change after phentolamine injection, but increased in all patient groups similarly during exercise. Although coronary vasoconstriction existed in stenotic vessel segments in control patients, phentolamine-treated patients showed exercise-induced vasodilation without difference in Twenty-six patients were patients with and without chronic beta-blockade. CONCLUSIONS Exercise-induced vasoconstriction of stenotic coronary arteries is prevented by intracoronary administration of phentolamine There was no difference in coronary vasomotion between patients receiving phentolamine alone;md patients receiving phentolamine in addition to a beta-blocker. This finding suggests that exercise-induced vasoconstriction is mediated not only by endothelial dysfunction but also by alpha-adrenergic mechanisms. (C) 1999 by the American College of Cardiology.

Reduced thrombus burden with abciximab delivered locally before percutaneous intervention saphenous vein grafts

Background Existing thrombus can complicate percutaneous saphenous vein graft (SVG) intervention. Local delivery of thrombolytics has been used to reduce the thrombus burden often associated with these interventions. We sought to determine whether local delivery of a platelet glycoprotein IIb/IIIa inhibitor is feasible and can reduce thrombus burden before percutaneous SVG intervention. Methods We performed a multicenter pilot study of abciximab (0.25 mg/kg) given by local delivery catheter before percutaneous intervention for de novo SVG stenoses followed by intravenous infusion. All patients (n = 58) had greater than or equal to 60% stenosis and Thrombolysis In Myocardial Infarction (TIMI) grade >0 flow in an SVG of 3 to 4 mm in diameter. Percent diameter stenosis, TIMI thrombus grade, and TIMI flow grade were measured before and after delivery of abciximab and after intervention. Results Median percent diameter stenosis improved from 69% to 45% (P = .0001) after local delivery, and TIMI thrombus grade greater than or equal to 1 incidence reduced from 68% to 34% (P = .0001). TIMI flow grade was not significantly affected (P = .12). All patients had a successful intervention (less than or equal to 50% residual stenosis). Conclusions Local abciximab delivery before percutaneous SVG intervention is associated with significantly reduced thrombus burden, significantly improved percent diameter stenosis, and excellent acute procedural results. Further studies of this approach are warranted to define its clinical utility.

The influence of abciximab use on clinical outcome after aortocoronary vein graft interventions

OBJECTIVES The purpose of this study was to evaluate the effect of abciximab use on clinical outcome in aortocoronary vein graft interventions. BACKGROUND Although large randomized trials have demonstrated a significant benefit of abciximab use in the setting of percutaneous coronary interventions, there is relatively little data with respect to the use of this agent in percutaneous vein graft interventions. METHODS Three hundred and forty-three patients were identified;210 undergoing vein graft intervention without abciximab and 133 patients with abciximab. RESULTS There were differences in baseline clinical and angiographic characteristics between the two groups;advanced age, unstable angina, older vein grafts and thrombus containing lesions were relatively common in both groups. Angiographic and procedural success rates were similar with or without the use of abciximab (89% vs. 92%, p = 0.15, and 85% vs. 91%, p = 0.12, respectively). The in-hospital composite end point of death/Q-wave myocardial infarction (QWMI)/repeat revascularization was similar between the two groups. Utilizing statistical modeling to adjust for baseline differences between the groups, abciximab use did not influence the cumulative long-term composite end point of death/MI/repeat revascularization. CONCLUSIONS This study demonstrates that in this relatively high-risk population undergoing aortocoronary vein graft interventions, the administration of abciximab periprocedurally does not appear to reduce major adverse clinical events. Effect of sex, hemodynamics, body size, and other clinical variables on the corrected Thrombolysis In Myocardial Infarction frame count used as an assessment of coronary blood flow

Background Compared with the conventional Thrombolysis in Myocardial Infarction (TIMI) flow grade system, the corrected TIMI frame count (CTFC) quantifies coronary blood flow in a more reproducible Fashion. The purpose of this study was to determine if the CTFC is affected by sex, body size, hemodynamics, or other selected clinical variables. Methods and Results CTFC was measured in 534 coronary arteries from 200 consecutive patients referred for coronary angiography. CTFC in each artery was related to patient variables (sex, age, race, and body surface area), clinical variables (cardiac rhythm, medication use, diabetes, hypertension, hypercholesterolemia, smoking, and left ventricular hypertrophy), angiographic variables (wall motion abnormality in each coronary artery distribution, left ventricular ejection fraction, percent stenosis in the artery, and presence of collaterals), and hemodynamic variables (aortic systolic and diastolic blood pressure and left ventricular end-diastolic pressure). By multivariate analysis, CTFC in all arteries was significantly associated with aortic systolic and diastolic pressures and body surface area. In addition, there were significant associations between CTFC and age and sex in some but not all arteries. Although significant the absolute change in CTFC associated with these variables was small. Conclusions CTFC provides a quantitative assessment of coronary blood flow that varies only a small amount in association with body size, systemic arterial pressure, age, and sex.

Relationship between corrected TIMI frame counts at three weeks and late survival after myocardial infarction

OBJECTIVES To evaluate the corrected Thrombolysis in Myocardial Infarction (TIMI) frame count (CTFC) as a predictor of late survival after myocardial infarction. BACKGROUND Thrombolysis in Myocardial Infarction flow grades predict late survival after myocardial infarction. The CTFC provides a more reproducible measurement of infarct-related artery blood flow than the TIMI how grade, and has been linked to 30-day outcomes, but it has not vet been established how the CTFC correlates with late survival. METHODS Of 1,001 patients with acute myocardial infarction presenting within 4 h of symptom onset, 882 underwent angiography at approximately three weeks. Infarct artery flow was assessed, blinded to clinical outcomes, according to the CTFC and TIMI flow grade. Late cardiac mortality and survival were determined in 97.5% of patients. RESULTS The mean CTFC was 40 +/- 29 in 644 patent infarct arteries (median, 34 [interquartile range, 24 to 47]). The CTFC, assessed as a continuous univariate variable, was found to be a predictor of five-year survival, as was the TIMI flow grade (both p < 0.001). On multivariate analysis, factors associated with five-year survival included the ejection fraction or end-systolic volume index (both p < 0.001), exercise duration (p = 0.005), age(p = 0.008), diabetes (p = 0.02) and CTFC (p = 0.02) or TIMI flow (p = 0.02). The same factors, except for the CTFC and TIMI flow grade, were predictors of 10-year survival. CONCLUSIONS The CTFC three weeks after myocardial infarction was an independent predictor of five-year survival, but not 10-year survival. Although the CTFC provided additional prognostic information within TlMI flow grades, its superiority was not demonstrated. (C) 2000 by the American College of Cardiology.

Effect of theophylline on the warm-up phenomenon

This study examined whether the adenosine receptor antagonist theophylline prevents the warm-up phenomenon in patients with stable angina undergoing serial exercise tests. Our findings offer evidence that adenosine does not play a role in the warm-up phenomenon, and indirectly suggest that the warm-up phenomenon does not represent ischemic preconditioning in humans.

Relation between infarct artery patency at late angiography after acute myocardial infarction and signal-averaged electrocardiography

Patency of the infarct-related artery (IRA) after acute myocardial infarction has a strong, independent association with a lower mortality rate at 1 year.(1-3) A patent IRA is associated with improved left ventricular remodeling(4) and reduced rates of sudden cardiac death.(1) The signal-averaged electrocardiogram (SAECG) is a noninvasive, computerized surface electrocardiographic recording that can detect delayed low amplitude cardiac signals ("late potentials") in the terminal portion of the QRS complex or within the ST segment.(5) The presence of late potentials after myocardial infarction is a sensitive and powerful variable that is predictive of sustained ventricular tachyarrhythmias.(6,7)

Usefulness of intravenous enoxaparin for percutaneous coronary intervention in stable angina pectoris

This pilot study was designed to determine whether the low molecular weight heparin, enoxaparin, could be used for elective percutaneous coronary intervention (PCI) to provide antithrombotic effects without the full systemic anticoagulation that occurs with the use of unfractionated heparin. Sixty patients were randomized to receive intravenous enoxaparin (1 mg/kg bolus dose) or unfractionated heparin at the time of coronary intervention. Laboratory testing was performed at baseline, 5 minutes, and 4 hours after study drug to test if a single bolus dose of intravenous enoxaparin can consistently achieve therapeutic antithrombotic effect, thus eliminating the need for multiple doses of heparin and closely monitoring levels of anticoagulation during PCI. Thirty percent of patients who received unfractionated heparin required a second bolus of intravenous heparin to achieve the target-activated clotting time of 300 seconds before PCI. Enoxaparin showed antithrombotic properties comparable to that of unfractionated heparin as measured by anti-Xa levels, with less inhibition of thrombin (factor IIa) at the time points measured (p <0.0001). Angioplasty success rates, in-hospital ischemia, bleeding, and vascular complications were similar in both groups. Thus, intravenous enoxaparin has predictable and effective antithrombotic effects during elective PCI. Although the level of anticoagulation attained with enoxaparin is significantly lower than that after unfractionated heparin, no increase in ischemic complications were noted. The use of a single bolus of intravenous enoxaparin, without the need for measuring the activated clotting time or titrating heparin anticoagulation, has the potential for simplifying the performance and perhaps enhancing the safety of PCI.
