The recent policy statement of the Medical Library Association (MLA) takes the position that scientific evidence is the basis for improving the quality of library and information sciences now and in the future. Research activity is seen as the foundation of an evolving knowledge base for the profession--a knowledge base that will set health sciences librarians apart from others in an increasingly competitive world of information service providers. The statement represents the culmination of many years of activity by association members, during which the role of research in health information practice has been debated. Over a similar time period, the quality movement, with its increasing demand for the collection and use of data, has been growing. Developments such as total quality management (TQM) and continuous quality improvement (CQI) reinforce the centrality of research with its increasing demand for the collection and use of data, has been growing. Adherence to practice guidelines: The role of specialty society guidelines

Background: Physician adherence to guidelines is often poor, but the reasons have not been completely studied. We investigated whether physician adherence to guidelines for percutaneous transluminal coronary angioplasty (PTCA) and coronary artery bypass grafting (CABG) varied by source, development methods, or the extent of their evidence-base. Methods and Results: We assessed adherence to guidelines developed by the American College of Cardiology/American Heart Association (ACC/AHA) for PTCA (1988 and 1993) and for CABG (1990) and guidelines developed by RAND for PTCA and CABG in 1990. We randomly sampled patients on Medicare who were undergoing coronary angiography in 5 states in 1991 and 1992, extracting clinical and laboratory data from medical records and using computer programs to classify the appropriateness of each procedure. A total of 543 PTCA and 676 CABG procedures were studied. By use of the 1988 ACC/AHA guidelines, 30% of PTCAs were rated class III (inappropriate), whereas 24% were class III by use of the 1993 guidelines. Only 1.5% of CABG procedures were class III with ACC/AHA guidelines. By use of RAND guidelines, 12% of PTCA and 9% of CABG procedures were classified as inappropriate. Conclusions: Adherence to guidelines is higher when the recommendations are supported by evidence from randomized clinical trials (CABG). The credibility of the source and familiarity with the guidelines do not ensure compliance. When evidence is lacking, as with PTCA at the time of this study, guideline recommendations may lag behind appropriate changes in clinical practice. More frequent revisions coupled with on-line access have the potential to make guidelines more useful.

Performance indicators in NHS libraries.

Performance indicators (PIs), workload statistics and accreditation are different ways to assess the quality of a library information service (LIS), and can be used together to give an overall picture of how well a LIS is performing its functions. PIs point out how something has been done. They are comparative values, often expressed partly as ratios or percentages, which indicate the quality or level of services and can be used to compare similar services, or the same service across time. PIs, performance ratios or performance standards are derived from performance measures, which show how much of it has been done. This paper looks at some aspects of performance measurement and suggests other areas for statistics collection in order to make better use of existing data, and considers the usefulness and derivation of PIs in NHS LIS.

Physician views on practicing professionalism in the corporate age

Arnold Relman argues that medical education does not prepare students and residents to practice their profession in today's corporate health care system. Corporate health care administrators agree: Physicians enter the workforce unskilled in contract negotiation, evidence-based medicine, navigating bureaucratic systems, and so forth. What about practicing physicians? Do they agree as well? According to this study, they do. Feeling like decentered double agents and unprepared, physicians find themselves professionally lost, struggling to balance issues of cost and care and expressing lots of negativity toward the cultures of medicine and managed care. However, physicians are resilient. A group of physicians, who may be called proactive, are meeting the professional demands of corporate health care by becoming sophisticated about its bureaucratic organization and the ways in which their professional and personal commitments fit within the system. Following the lead of proactive physicians, the auf hors support Relman's thesis that education for both students and physicians requires a major overhaul.

Medical audit and libraries--an opportunity for progress.

Working for patients has necessitated the use of medical audit as a quality assurance tool. This paper presents an overview of the form and content of audit, and examines the supporting role of the library. It notes the failure of many Regional Medical Audit Implementation Plans to recognize the value of library services, and proposes a strategy to enable librarians to play an active part in the process, and by doing so to increase awareness of library services and improve decision making.

Throughout the history of the development and use of medical technologies (phar-maceuticals, laboratory tests, invasive and noninvasive procedures, and medical devices), physicians and their patients have been the primary assessors of a technology's worth. If a drug, test, or procedure produces consistent beneficial effects, with little or no risk, then such a technology is commonly added to the clinician's arsenal of weapons against illness and disease. Academic researchers, however, with their keen desires to find new and better ways to diagnose, treat, and cure, are relied upon to produce evidence documenting a technology's efficacy. Others engaged in technology assessment activities—from their own perspectives—include manufacturers, who produce data showing the value of the products they hope to sell; hospitals that wish to make cost-effective purchases; and government agencies, with mandates to protect the public and to spend public dollars wisely. Still, it is the practicing physician who ultimately judges a technology's value based upon its use in individual clinical situations. Until very recently, with the apparent adequacy of these evaluative methods, medical professional societies were not required to enter the technology assessment arena.

From intelligence to evidence-based healthcare: a purchasing odyssey.

Since the appearance of the NHS Executive's guidance document, Purchasing Intelligence (1991), efforts have been concentrated on the development of 'purchasing intelligence facilities' staffed, in the main, by health librarians. However, the concept of 'intelligence' has a wider dimension that impacts upon the whole culture of an organization. The authors examine purchasing organizations against popular theories of organizational development. The characteristics of Handy's Task culture, with Athena as its patron, may be observed in many authorities. Using the results of two surveys, conducted in March 1995, they identify some key information management issues involving structure, tasks, technology and people. Skills in information 'transformation' must be acquired by all staff in purchasing authorities. The intelligence officer must become a modern-day Odysseus if the intelligence facility is to 'survive and thrive'.
