Two years ago, President Clinton led a charge in Washington to reform health care. One year ago, the big news was the collapse of all efforts to pass "comprehensive health care reform" in the 103rd Congress. Today, expanding access to health care is the farthest thing from our legislators' collective mind. The new agenda is how to limit the supposedly unsupportable costs of our current guarantees of care to the elderly and the poor. Some may think the switch from guaranteeing care to cutting programs was inevitable; in fact, it is inexcusable. Every other industrialized nation guarantees a high standard of care to virtually every citizen, at much lower cost than that of the U.S. system. Although there are differences in detail, the basic principles of these nations' systems are remarkably similar, so much so that they constitute an international standard.

The most important issue in the fight against HIV/AIDS is how to scale up existing programs that are only reaching small numbers of people to the national Level. Although nurse researchers have contributed in significant ways to the body of literature on health disparities, a broad framework and approach to health disparities is needed if we are to understand the complex interplay of variables that contribute to health disparities. This article reports on the work of the Center for the Advancement of Health Disparities Research and its development of a conceptual framework and method for approaching health disparities research and on a sample of nursing research consistent with this framework.

The Emergency Medical Treatment and Labor Act (EMTALA) was enacted by Congress because of its concern with an increasing number of reports that hospital emergency rooms were refusing to accept or treat individuals with emergency conditions if the individuals did not have insurance. With increasingly crowded emergency departments and a decreasing number of emergency departments, a periodic review of the effect that EMTALA has on the emergency medical services will prevent unintended consequences of this well-intentioned act.

The Department of Defense (DoD) undertook a major systems specification, acquisition, and implementation project of multivendor picture archiving and communications system (PACS) and teleradiology systems during 1997 with deployment of the first systems in 1998. These systems differ from their DoD predecessor system in being multivendor in origin, specifying adherence to the developing Digital Imaging and Communications in Medicine (DICOM) 3.0 standard and all of its service classes, emphasizing open architecture, using personal computer (PC) and web-based image viewing access, having radiologic telepresence over large geographic areas as a primary focus of implementation, and requiring bidirectional interfacing with the DoD hospital information system (HIS). The benefits and advantages to the military healthcare system accrue through the enabling of a seamless implementation of a virtual radiology operational environment throughout this vast healthcare organization providing efficient general and subspecialty radiologic interpretive and consultative services for our medical beneficiaries to any healthcare provider, anywhere and at any time of the night or day.
