Indigent health care plan for Tennessee.

The author describes the development of an Indigent Health Care Program for the State of Tennessee from the campaign pledge of the Governor, through the development of an Indigent Care Cabinet Council, to the development and enactment of appropriate legislation required to implement this program. The program deals with availability of health care services, accessibility to health care services and funding mechanisms. The backbone of the program is the involvement of the local community and the coordinating and networking of the existing private, community and state health care providers.

State legislation and policy affecting rural hospital conversion and closure

Many rural hospitals have closed or converted to organizations that provide health services other than general, acute inpatient care. However, little is known about why rural hospitals convert rather than close. This study evaluates the relationship between state policy and rates gf rural hospital conversion relative to rates of rural hospital closure. The expectation was that the relationship among state policies and rates of conversion and rates of closure differs as a function of whether a state policy facilitates rural hospital transition to alternative models of care or supports them in their existing form. National data mere analyzed for all rural hospitals from 1984 to 1991. Results indicate that state policies have done little to facilitate widespread adoption of conversion among rural hospitals. Despite these findings, results also indicate that some state policies have resulted in an increase in the rate of rural hospitals conversions as an alternative to closure.

What types of hospital patients wait for alternative placement? Findings from an exploratory case study and policy implications

Concern about delayed hospital discharges has accentuated since the advent of the Prospective Payment System (PPS) and resultant hospital undeireimbursement for the care of those patients awaiting alternative placement. This article reports on ongoing research on the delayed discharge problem. Analyses of the determinants of the likelihood of delay and the number of days of delay by patients awaiting alternative placement in 76 North Carolina acute care general hospitals (about two thirds of all such hospitals) during May 1991 indicate that, although demographic attributes such as age, race, and sex are important correlates of delay, which averaged nearly 11 days, a patient’s requirement for heavy care is the most significant policy-relevant variable in explaining the number of delay days. © 1993 by The Haworth Press, Inc. Background. This article examines teledentistry and some fits current legal issues, Topics include licensure, malpractice, technology and ethics General recommendations for the dental practitioner are included. The literature review includes state and federal laws, the Telemedicine Report ta Congress and numerous articles (both printed and electronic) associated with the topic;Sources were selected for timeliness and relevance to legal issues and implications of telemedicine/teledentistry for the dental practitioner. Conclusions, Numerous issues require resolution before telemedicine and teledentistry will truly realize their enormous potential to increase access to health care while decreasing health care costs. These issues include interstate licensure, jurisdiction and mal-practice, as well as technological, security and ethical questions, Practice Implications. Telemedicine teledentistry are relatively new to the dental field. Many of the legal issues reviewed have yet to be resolved by the legislature or the courts. Furthermore, technology has not yet progressed to the point where the practitioner can be certain that no technological failure will occur during a teledental consultation. In spite of these problems, the potential of telemedicine and teledentistry is tremendous. Improvement in accessibility of health care and lowered health care costs are only two of the many advantages that will emerge as telemedicine and teledentistry become integrated with, and fundamentally change, the practice of medicine and dentistry.

Cancer care extends from diagnosis through the late stages Of advanced illness as patients confront dying and their families cope with caregiving and grief Palliative care is a rapidly developing area of clinical focus that offers valuable services to patients in terms of symptom management and adjustment to illness, including issues of life completion and life closure. It is often appropriate to offer certain elements of palliative care early in the course of illness. As disease progresses, physical comfort and enhancing quality of life increasingly become primary goals of cancer care. Specialized palliative care programs, epitomized by hospice, are invaluable resources for patients with far-advanced illness and their families. Current regulations and prevailing payment structures limit access to and the scope of hospice services and highlight the need for innovative models of delivering and financing palliative care.
