It is estimated that in 1999, psychological counselors will work in over half of all general practices (primary care physician partnerships) in the United Kingdom. The history of the growth of counseling in general practice is, in many ways, typical of the British way of doing things. Ad hoc arrangements at a personal level, in which individual general practices let a counselor, of ten a social worker, colleague, or personal friend, see those patients who had a variety of problems the primary care physician felt unskilled to handle, characterized many of the early reports of General Practice Counseling.(2,3,26) From provisional data available from the first ever National Survey of general practice counselors,(29) it seems that most counselors are women (87%), with an average age of 48 years;all but a tiny handful have recognizable counselling or psychotherapy qualifications. Fifty percent usually possess a British Association for Counseling Recognized Certificate and Diploma, 25% possess additional counselling and psychotherapy qualifications, and 13% possess qualifications at a Masters level. The counselors surveyed come from many backgrounds, including social work, teaching, and nursing, and they work between 5 to 30 hours a week in general practice. As more research studies are published in the United Kingdom, their work, along with other members of the primary care teams (doctors, nurse practitioners, health visitors, psychotherapists, social workers, and others), is becoming increasingly valued by their fellow primary care team members, and by the patients they see who are referred to them by their primary care colleagues.(4,21,24)

There is evidence that mentally ill offenders (MIOs) in prisons commit more infractions, serve longer sentences, and are more likely to be victimized than inmates who are not mentally ill. Humanistic and prison management interests are served if intervention programs minimize symptoms and promote coping and other functional skills. A collaborative agreement was established between Washington State Department of Corrections and a consortium of University of Washington faculty to mutually develop a prison-based program of clinical management and psychoeducation for MIOs. The resulting program is described, along with rationale, planning processes, implementation, and initial evaluation. Most aspects of the planned program are in place. Clinical and behavioral progress by inmates following program participation has been documented. Issues concerning treatment program implementation in prisons are discussed. This paper is the second of a series on the role of the nurse teacher and is based on a review of the literature and the personal experience of the authors. Identifying the most appropriate way to support students in both the academic and clinical setting has been a major concern for nurse teachers. The move of nurse education into the higher education arena has increased the diverse nature of the nurse teachers role and raised questions about the nature and function of the personal tutor in relation to students undertaking nursing courses. This paper discusses the concept of the personal tutor, with particular focus on the academic aspects of the role. The time implication of adopting an all encompassing personal tutor role, within the current climate of higher education, is considered. The paper also highlights that the roles tutors adopt in practice are diverse and usually as a result of convenience rather than identified outcomes. (C) 2000 Harcourt Publishers Ltd.

A true experimental, posttest only, control group design was used to pilot test the effectiveness of a telephone nursing intervention in increasing community survival, and decreasing the number and length of rehospitalizations for persons with schizophrenia. The vulnerability model was used as a;framework for the study. The intervention was hypothesized to function as a moderating factor to reduce the effects of stress on vulnerable persons with schizophrenia. Experimental subjects received telephone nursing intervention weekly for 3 months;control subjects received informational calls at weeks 6 and 12. Readmission rates were 13% (n = 2) for experimental subjects and 23% (n = 5) for controls. Compared with controls, experimental subjects experienced a 4% increase in community survival and: a 27% reduction in length of stay if readmission occurred. These preliminary data suggest that telephone intervention can increase community survival in persons with schizophrenia. Future studies should focus on increasing sample size and refining the intervention. Copyright (C) 2001 by W.B. Saunders Company.
