The terrorist attacks of Sept 11, 2001, we a horrifying wake-up call for the United States and the rest of the world. The attacks led to the deployment of the disaster medical assistance team (DMAT) from Massachusetts General Hospital in Boston. In this article, members of the team outline what they did during the days after Sept 11 and the lessons they brought back to better prepare their DMAT for the next disaster.

Evolution of the perioperative clinical nurse specialist role.

Perioperative nursing roles continue to rapidly change as we enter the twenty-first century. The need for strong leadership skills, expert clinical skills, creative management, ongoing continuing education, and research continues to grow in every department of surgical services. The clinical nurse specialist plays an important role in addressing each of these needs. Great opportunities exist within the field of nursing for perioperative nurses to expand their practice using their creativity, ideas, and skills. Using the clinical nurse specialist in the perioperative setting can foster creativity, stimulate development of new methods based on research, and maximize the delivery of high quality care by the entire OR staff.

A collaborative program for cardiovascular patient follow-up.

Transitions from one health care system to another are common, but they can affect coordination of care and measurement of patient outcomes. In this project, a community hospital and a tertiary center collaborated to obtain follow-up information on patients 30 days after they underwent cardiac catheterization and revascularization. Results show that patients experienced similar physical and emotional concerns regardless of diagnosis or procedure. Although the intent was to gather information, nurses also provided education and arranged services for patients. This project has implications for the development of collaborative interventions to improve care for patients after undergoing coronary procedures.

No one is more capable of identifying problems related to health care than health care practitioners. Identifying a problem is the first step to every new product or invention. This article provides step-by-step information to help health care practitioners identify problems and possible solutions, look for a creative company to bring the solution to the marketplace, and present the product idea to potential industry partners. The author also discusses the legal aspects of product development and the advantages and disadvantages of licensing a new product or starting a business to develop a product.

The RN first assistant's role in a laparoscopic cholecystectomy.

Laparoscopic cholecystectomy is one of the most frequently performed laparoscopic procedures today and involves the same surgical principles as an open cholecystectomy, except that access to the abdomen is gained transabdominally using video laparoscopic techniques and instrumentation. Registered nurse first assistants (RNFAs) provide retraction and tissue exposure for the surgeon during this procedure and should be aware of safe laparoscopic surgical techniques and potential injuries that can occur from manipulation of surrounding structures. The experience and expertise of the RNFA when assisting in a laparoscopic cholecystectomy are key to the successful completion of the surgery.

Clinical nurse specialist practice--spheres of influence.

Many nurses are uncertain of the differences in the educational backgrounds and practice domains of clinical nurse specialists (CNSs) and nurse practitioners (NPs). This uncertainty may make it difficult for perioperative nurses to select the advanced practice role that is most congruent with their career goals and interests. The National Association of Clinical Nurse Specialists developed the Statement on Clinical Nurse Specialist Practice and Education, which provides an organizing framework for the CNS practice role. This article offers distinctions between the educational experiences of CNSs and NPs and suggestions for perioperative CNS practice.

Optimizing second shift OR staffing.

In surgical suites when ORs sometimes run late, nurse anesthetists or perioperative nurses may be scheduled to work a second shift to cover procedures. Nurse anesthetists' OR workload in the afternoons can differ from that of perioperative nurses. At the end of long procedures, times to transport and stabilize patients can be considerable. This article shows that optimal second-shift OR staffing is the same for nurse anesthetists and perioperative nurses when assessed using anesthesia billing data and OR information systems data respectively. Managers do not need hospital information systems staff members to provide data from both anesthesia billing and OR information systems to make second-shift staffing decisions. One or the other is adequate.

Managing the new generation.

As the Silent Generation quickly approaches retirement, and the Baby Boomer generation moves into management and supervisory roles, a new generation is entering the workforce. Generation Xers were born between 1965 and 1975, at a time when the country experienced high prices, wage inflation, stagnant consumer demands, and increased unemployment. Perhaps as a result, newcomers to today's workforce often have different characteristics, behavioral traits, and belief systems from their predecessors. To effectively supervise and motivate individuals from this unique generation, health care managers must begin by understanding the evolutions that have shaped each generation and influenced the workforce. In doing so, managers can create a thriving multigenerational work environment and embrace supervisory concepts that attract and motivate the new generation.

Promoting patient safety through preoperative patient verification.

At Spectrum Health, Grand Rapids, Mich, a process to verify patient identification, procedure, and procedure side and site was developed and implemented to improve the current process. Focusing on risk-reduction strategies, an interdisciplinary task force revised the facility's surgical site identification policy and improved the surgical scheduling form to highlight information for sided procedures. Task force members also designed a verification checklist to require the comparison of any procedure data to the physician's order. This article describes these improvements and other methods that were established to ensure patient safety and reduce risk. Ongoing data collection verifies that the process remains in place and that team members are compliant. The improved process maximizes patient safety because it allows for early intervention when any discrepancies are identified.

An RN first assistant's guide to private practice.

In 1984, AORN developed its first official statement on RN first assistant (RNFA) practice. This article is a guide for RNFAs seeking self-employment. The dynamics of change, with reference to the role of the RNFA, in perioperative nursing and entrepreneurship will be discussed. Nurses' responsibilities in this expanded role, methods of becoming a change agent, and methods of obtaining privileges to practice as a self-employed RNFA (ie, functioning under the supervision of a surgeon) in health care organizations will be identified. Current trends and issues influencing RNFA practice, including certification, reimbursement, practice setting, education, competition, and changes in health care, also will be discussed.
