The North Norwegian Health Net is a comprehensive scheme to connect all health-care institutions in the area to a national computer network. Services available include telemedicine, email and Web access. A general practitioner has responsibility for ensuring that the clinical information is correct. Medical departments are responsible for the content of their own Web pages. All institutions require authorization before connection to ensure data protection and security. Changes in communication between primary- and secondary-care sectors are being monitored. To date the implementation of the network programme has gone smoothly.

Lead toxicity, a man-made disease of young children, has only recently come to the focus of professional and public awareness. Lead has become increasingly bioavailable to humans as a direct result of industrial processing and manufacturing. While lead-based paint in older dwellings is the primary cause of lead toxicity among young children, airborne lead from gasoline fumes and factory emissions, plus dirt and dust into which high concentrations of lead have settled, are also significant sources of undue exposure. A variety of consumer products, including kitchen utensils, newsprint, and cosmetics are likewise potentially hazardous because of their high lead content. Exposure to excessive amounts of lead is especially harmful for young children: they are biologically and developmentally more vulnerable to its toxic effects. Even at levels of absorption that produce no medical symptoms, lead may impede children"s overall developmental progress by interfering with their performance in several crucial areas. Although the United States has been reluctant to assume an aggressive regulatory position toward lead, the federal government currently supports comprehensive lead poisoning control programs in 60 American cities. The future of these programs is jeopardized by the shift from categorical to block grants. Unless continued funding is made available for lead management programs and a strong regulatory stance is assumed toward lead, young children will continue to become afflicted with this debilitating, yet preventable, illness.

The mission of the Health Resources and Services Administration (HRSA) in the U.S. Department of Health and Human Services is to improve the health of the nation by assuring quality health care to underserved and vulnerable populations and by promoting a primary care and public health workforce. This article provides an overview of HRSA's prior work in performance measurement and highlights a current initiative designed to help the agency improve its performance measurement efforts in the context of the Government performance and Results Act. This initiative began with an agencywide assessment of capacity to measure performance at program, bureau, and agency levels. Based on the findings, HRSA is moving forward both with technical assistance to individual bureaus in developing performance indicators and data sources and with activities to coordinate performance measurement for the agency as a whole.

The aim of this essay is to provide an overview of initiatives designed to increase physical activity among different populations and in different settings, and to set the context for the major challenges that lie ahead. The decline in habitual physical activity with modernization, and the causal link between physical activity and health are briefly reviewed. The need to understand physical activity as a health behavior and examples of behavior change theories that have been applied to the promotion of physical activity are discussed. Diverse projects and campaigns in three countries, i.e. South Africa, Australia and the US, are highlighted. Common themes found in these physical activity initiatives include the development of a theory-driven research base, inclusion of behavioral and social scientists on multidisciplinary teams, and advocacy for environmental changes that promote physical activity. Within the next decade, research projects and national campaigns such as those described herein will yield important new findings on how to increase physical activity levels among targeted sectors of the population. The research agenda for the future includes development of both basic and applied research on physical activity, and the integration of theory across social, behavioral and biomedical disciplines.
