Benchmarking and networking through collaborative groups.

The search for better ways to deliver healthcare services at lower cost led a group of hospitals in the Carolinas, 2 years ago, to pioneer a new concept, the collaborative group. A collaborative group typically is composed of five or six similar (but not competing) hospitals that work together closely to help each other make the changes necessary to survive in an increasingly competitive marketplace. Hospital staff members at all levels are able to work together to discover improvement opportunities through benchmarking teams and networking efforts. This article describes their experiences, achievements, and lessons learned.

Developing a hospital policy for clinical pathways.

Clinical pathways are used for planning, coordinating and facilitating the continuum of patient care from before admission to after discharge. They rapidly are changing the provision of healthcare services through improved patient care and cost containment, but they also have raised questions about possible legal ramifications. If sound policies and procedures for clinical pathways are developed and consistently adhered to by healthcare facilities, the potential for liability may be minimized while patient care is improved. This article describes a multidisciplinary task force's work to develop such policies and procedures.

Comparative data analysis using collaborative skilled nursing/long-term care indicator assessment.

This article describes a process developed by Eastern Mercy Health System (EMHS), headquartered in Radnor, PA, to identify opportunities for quality improvement using defined outcome-based indicators in skilled nursing and long-term care (SN/LTC). The model is built on collaboration among the system's freestanding and hospital-based facilities;it includes a cyclical approach to the exchange of information. The experiences of three of the system's members reflect how each has realized measurable benefits by applying to their unique settings the lessons learned through the collaborative process.

Teaching total quality management: developing and deploying education throughout a healthcare system.

Healthcare organizations are faced with the responsibility of teaching their entire staff the founding principles, concepts, and practices of total quality management (TQM), to improve organizational effectiveness. Purchasing training products from outside consultants can be costly, but healthcare leaders can design and develop their own TQM training products, integrating each product with their organization's mission and values. This article describes how a Catholic healthcare system successfully developed and deployed a compendium of TQM educational products throughout the health-care system.

With the recent formation of the Queen Elizabeth II Health Sciences Centre in Halifax, Nova Scotia, a number of challenges have arisen that must be addressed. One involves the porter service, which faces increased demands since every department has new and often expanded duties for porters. This paper identifies obstacles facing the porter service and develops recommendations, which may also be of interest to other healthcare facilities facing similar challenges.

A 10‐Point Strategic Checklist for Rural Health Care Systems

Abstract: A checklist format is used to provide a framework for rural hospital executives and community members for gauging the health and stability of rural hospitals and rural hospital systems. Benchmarks are provided for financial and operational performance and emphasis is placed on medical staff size and physician recruitment. Physician/hospital organizations and regional partnerships are used as examples of strategies available to rural providers. The importance of market knowledge and regional strategic alliances also is stressed. In an era of dwindling resources and tight reimbursement, rural providers are encouraged to consider cooperative clinical programming and technology consolidation. Copyright © 1995, Wiley Blackwell. Quality Improvement in Ambulatory Care (QIAC), a national demonstration project undertaken in northern Minnesota, recently was honored by the American College of Physician Executives at its 1992 national meeting in San Francisco. The Merck, Sharp & Dohme Award for Advances in Quality, an award recognizing significant advances in improving the quality of healthcare delivery, was awarded for the first time ever to the QIAC project. Impartial reviewers, using objective criteria, selected this project as this year's most significant advance in improving healthcare quality because of its magnitude and its innovation.

Establishing a rural hospital cooperative: a case study.

Rural hospitals have traditionally been the providers of health services in rural areas. In recent years, however, rural hospitals have come under increasing economic and regulatory stress. Rural hospital cooperatives represent a new level of shared management that addresses mutual problems using a horizontal organizational model. In 1987, the Western New York Rural Health Care Cooperative was established by building on fragments of associations that had existed previously. Triggered by an opportunity for grant funding, a major regional cooperative evolved which has demonstrated effective responses to a variety of problems inherent in providing rural health care. Particularly successful programs have such features as a quality assurance plan that uses cooperative-wide peer review, nursing and allied health education, physician recruitment, and the active involvement of a university medical school. This paper is a case study of the development and success of the Western New York Rural Health Care Cooperative.
