目的:探讨宫、腹腔镜联合三维超声宫腔造影对子宫纵隔的诊断和治疗的作用。方法:分析57例子宫纵隔患者应用不同的诊断方法以及对通过阴道三维超声宫腔造影,直到宫腔镜下子宫纵隔电切术的效果进行分析。结果:术前诊断符合率:宫腔镜检查96.2%,B超78.7%,三维超声宫腔造影100%。术中情况:行宫腔镜下子宫纵隔电切术患者57例,其中完全纵隔12例,合并阴道纵隔10例,不完全纵隔35例。平均手术时间(37.5±11.35)min,出血(15.21±6.02)ml。手术均一次完成,无子宫穿孔、脏器损伤等并发症发生;术后随访:术后随访55例患者,随访率96.5%,自然流产率由术前85.7%下降至术后2.9%,足月分娩率由术前3.6%上升至术后48.6%,差异有统计学意义。结论:三维超声宫腔造影检查可明显提高子宫纵隔的诊断符合率;经阴道三维超声宫腔造影,指导宫腔镜下子宫纵隔电切术一种安全、有效、理想的治疗方法。

The biological roles of estrogen-responsive finger protein (efp) in vivo were evaluated in mice carrying a loss-of-function mutation in efp by gene-targeted mutagenesis. Although efp homozygous mice were viable and fertile in both sexes, the uterus that expressed abundant estrogen receptor alpha exhibited significant underdevelopment. When the ovariectomized homozygotes were subjected to 17 beta-estradiol treatment, they showed remarkably attenuated responses to estrogen, as exemplified by decreased interstitial water imbibition and retarded endometrial cell increase, at least, attributable to the lower ratio of G1 to S-phase progression in epithelial cells. These results suggest that efp is essential for the normal estrogen-induced cell proliferation and uterine swelling as one of the direct targets of estrogen receptor alpha.

Hysteroscopic endometrial ablation is an effective alternative to hysterectomy in women with menorrhagia and large uteri

Study Objectives, To examine the feasibility: safety, and outcome of hysteroscopic endometrial ablation, and to determine the volume of fluid absorbed during resection versus rollerball coagulation in women with menorrhagia and large uteri. Design. Retrospective review (Canadian Task Force classification IL-2). Setting, University-affiliated teaching hospital. Patients. Forty-two consecutive patients (mean +/- SD age 45.6 +/- 6 yrs) with uterine size greater than 12 weeks (cavity >72 cm). Intervention. Endometrial ablation;26 (62%) women were pretreated to thin the endometrium. Measurements and Main Results, Resection was performed in 27 patients (65%) and rollerball coagulation in ij (35%). Ablation was successfully performed in all patients in a da) surgery setting. Multiple regression analysis examined the relationship of uterine size, pretreatment, procedure, and duration of surgery to amount of glycine absorbed. Glycine absorption was higher with resection than with coagulation (p = 0.04). Fluid absorption cor related with type of procedure (r = 0.32, p = 0.04) but not with duration of the procedure, uterine size, or pretreatment. One patient with uterine fibroids and one with endometrial adenocarcinoma had hysterectomy. With follow-up? of 39 (95%) of 41 women (excluding the one with adenocarcinoma) For 14 +/- 2 months, 38 (93%) were very satisfied. Thirty (73%) had amenorrhea, six (15%) had hypomenorrhea (<3 pads/day), and three (7%) had eumenorrhea (<10 pads/day). Conclusion, Hysteroscopic endometrial ablation may be a Feasible, safe, and effective alternative to hysterectomy in women with menorrhagia and large uteri.

IRON MOBILIZATION IN PREGNANT RAT

Fe exchange in the pregnant rat was quantitated by repeated determinations of plasma Fe turnover (PIT), transferrin Fe distribution and measurements of storage Fe and food Fe utilization employing selective radio-iron probes. Despite a 6-fold increase in PIT, intestinal absorption of Fe accounted for 40% of the PIT throughout pregnancy, with variations not exceeding .+-. 5%. Increased fetal requirements were efficiently compensated for by the mobilization of Fe from maternal tissue stores and by increased absorption, and there was no subsequent reduction in Fe supply to maternal tissues. Enhancement of Fe absorption occurred in the absence of a reduction in serum Fe levels or the size of Fe stores. In view of the close correlation between PIT and rates of absorption it is postulated that Fe absorption in the pregnant rat is regulated by PIT, which in turn is determined by the rate of plasma Fe clearance by the placenta and the maternal erythroid marrow.

Distribution and activation of uterine mononuclear phagocytes in peripartum endometrium and myometrium of the mouse

The present study tested the hypothesis that macrophage distribution and activation are enhanced in the uterus before term. Mid-uterine horn tissue strips from mice on Days 15 and 18 of pregnancy, the day of birth (= Day 19), and one day postpartum were paraffin-embedded and then sectioned, stained with a monoclonal pan-macrophage marker (BM8), and processed for visualization and quantification of resident macrophages per nuclear area. Macrophages were dispersed throughout the endometrium and subluminal epithelium;cell numbers declined on the day before term, then increased postpartum. Within myometrium, macrophages congregated in stroma surrounding muscle bundles, and staining was enhanced near term. Macrophage numbers were similar in pregnant and postpartum uteri, enhanced more than 2-fold over those in nonpregnant controls. Uterine sections were also analyzed by laser-scanning cytometry to enumerate activated macrophages (i.e., those that express the intercellular adhesion molecule marker CD54+) and to determine cell cycle (propidium iodide fluorescence). Activated macrophages were directly proportional to cell numbers and, by cell cycle analysis, were not terminally differentiated. Highest cell numbers occurred on Day 15: 4-fold greater than those in nonpregnant controls and 2-fold higher than those at Day 18 or in postpartum groups. These findings indicate a decline in endometrial macrophage numbers at least one day before the onset of parturition and raise the possibility that trafficking of this immune cell may contribute to onset of labor.

IDENTIFICATION OF FETAL GROWTH-RETARDATION BY ULTRASONOGRAPHIC ESTIMATION OF TOTAL INTRA-UTERINE VOLUME

Total intrauterine volume (TIUV) estimation may be useful for distinguishing normal from growth-retarded fetuses. Measurements of TIUV (179) were obtained from the ultrasonograms taken between 21-40 wk of gestation in 140 patients who delivered average birth wt babies. A mean curve for TIUV with upper and lower 2.5 and 10% tolerance limits was constructed and the TIUV''s of 16 patients with growth-retarded fetuses evaluated. Of these fetuses, 9 had birth wt below the 5th percentile for gestational age, and in each of these pregnancies the TIUV ultimately fell below the lower 2.5% tolerance limit.

Estradiol and pesticide methoxychlor do not exhibit additivity or synergism in the reproductive tract of adult ovariectomized mice

The pesticide methoxychlor (MXC) is a DDT substitute that exhibits estrogenic activities in different animals. To determine whether there is synergism between purified MXC and a natural estrogen, ovariectomized adult mice received 3 daily intraperitoneal doses of either 2.5 or 25 ng estradiol-17 beta or 0.125, 0.25, or 0.5 mg MXC administered separately or in combination. The mice were sacrificed on d 4. Reproductive tracts were excised, weighed, and one uterine horn was flushed with phosphate-buffered saline, with the fluid electrophoresed on a one-dimensional sodium dodecyl sulfate polyacrylamide gel to determine albumin content. The remaining uterine horn and vagina were prepared for histology and epithelial height measurements. Estradiol significantly increased reproductive-tract weights. Although both the vaginal and uterine epithelial heights increased in mice treated with combined chemicals when compared to controls, the organ histology did not show increased stimulation. Albumin content was significantly elevated only in the estradiol group. The present data do not suggest that either synergism or additive effects occurred between an estrogen and MXC in the reproductive tracts of ovariectomized adult mice. in fact, combining MXC with estradiol suggests inhibitory effects.

Histomorphological changes in the oviduct epithelium of the rabbit during early pregnancy

In this study, we determined the histomorphological changes in different regions of the oviduct (fimbriae, infundibulum, ampullae and isthmus) during early pregnancy (days 1-4) of the rabbit. It was observed that the number of secretory cells (PAS-positive cells) diminished in the fimbriae during the first 4 days of pregnancy with the corresponding increase in the number of non-secretory cells (PAS-negative cells). In contrast, there was an increase in the number of PAS-positive cells in the isthmus during early pregnancy. In both the infundibulum and the ampullae the number of PAS-positive cells was increased on day 3. A significant decrease in the height of the epithelium was observed in the fimbriae during early pregnancy. On the contrary, a significant increase in this parameter was observed in the isthmus during the first 3 days of pregnancy. A slight decrease in the height of the epithelium was observed in the infundibulum on day 4 of pregnancy, while in the ampullae it was significantly increased on days 1 and 3. The overall results indicate that during early pregnancy there is a selective modification in the histomorphological characteristics of the different regions of the rabbit oviduct.

UTERINE SIZE MEASURED BY ULTRASOUND DURING MENSTRUAL-CYCLE

In the following study changes in the size of non-pregnant uterus were measured by B-scan ultrasonography. Uterine size in different stages of the menstrual cycle was measured ultrasonically in 16 women, whose periods were confirmed to be ovulatory, both by basal body temperature (BBT) and by the radioimmunological measurement of plasma estradiol and progesterone. It was established that the size of the uterus grows significantly towards the end of the menstrual cycle. In addition to the above mentioned, the examination was performed on a woman of child-bearing age with disturbances in her menstrual cycle. Hormone measurements indicated that her period was anovulatory. No typical uterine growth characteristic of ovulatory cycles could be observed.
