Coping with illness after brain diseases - a comparison between patients with malignant brain tumors, stroke, Parkinson's disease and traumatic brain injury

Purpose : This study investigates coping styles in patients suffering from different brain disorders (malignant brain tumors, stroke, Parkinson's disease and traumatic brain injury). Method: In a combined analysis of four prospective studies we investigated 21 patients with malignant glioma at the end radiochemotherapy, 30 patients one year after ischemic stroke, 54 patients suffering from various stages of Parkinson's disease, and 58 patients 6 to 8.5 months after traumatic brain injury. The assessment of coping with illness was based on the Freiburg Questionnaire on Coping with illness.(1) Results : With few exceptions, coping styles did not differ across the various brain pathologies. Differences occurred with respect to 'active, problem-oriented coping' (decreased in stroke patients), and coping by search for religious relief or quest for sense (increased in patients with Parkinson's disease). Conclusion : Differences in coping styles could be mainly related to age and social factors. Individual coping strategies seem only to be little related to the type of brain pathology.

Religious and other predictors of psychosocial adjustment in cancer patients

The authors tested the hypothesis that religious variables, such as a person's belief that his/her illness was God's will, would predict psychosocial adjustment in 50 patients who were predominantly Catholic Hispanic women attending a medical oncology clinic (42 women, 8 men). The patients were free of an Axis I mental disorder cognitive impairment, and severe pain and were riot undergoing intensive chemotherapy By using the Psychosocial Adjustment to Illness Scale as the outcome measure, the authors found few associations with religious variables, but many to clinical variables.

Spiritual issues associated with traumatic-onset disability

Purpose : The present study used a qualitative approach to discover the concepts used by people with a sudden-onset disability to express issues of spirituality. Using a cross-sectional time frame, the study sought to understand how spiritual issues were experienced at the onset of a disability from the perspective of the disabled person him or herself. The study also compared the perceptions of brain injury respondents to spinal cord injury respondents. Method: The qualitative approach was chosen to capture a detailed understanding of perceptions about spirituality, and the relationship of the disability to the disabled person's spiritual beliefs. The sample consisted of 16 individuals, all of whom had experienced a spinal cord injury or a brain injury. Results : The main findings of the study were as follows: (1) Participants recovering from a disability described spiritual issues relating to five themes which arose directly from the data (awareness, closeness, trust, purpose, vulnerability) and relating to three relationships found throughout the literature on spirituality (intrapersonal, interpersonal and transpersonal). These eight dimensions can be combined to form a matrix, which serves as a framework for considering spiritual issues associated with disability;and (2) There were differences in the spiritual concepts described by the participants with brain injury compared to those with spinal cord injury. In particular, those with brain injuries seemed to place greater emphasis on the importance of their families, and to be more aware of the need for trust in view of their memory deficits. Conclusions : These results represent one of the earliest attempts to take an empirical approach to the development of theory in the area of spirituality and disability. Religion, death of a loved one, and hypertension among older adults in Japan

Objective. To see whether three dimensions of religion (private religious practices, religious coping, and belief in the hypertension over time. afterlife) buffer the effect of the death of a significant other on change in self-reported Methods. Interviews were conducted with a nationally representative sample of people aged 60 and older in Japan at two points in time. 1996 and 1999. Complete data were available on 1,723 older Japanese, Respondents were asked a series of questions about their religious beliefs and practices, A bother a family member or close friend had died in the past year, and whether they had hypertension. Results. The data suggest that older adults in Japan who experienced the death of a loved one but who believed in a good afterlife were less likely to report they had hypertension at the follow-up interview than elderly people in Japan who lost a close other but did not believe in a good afterlife. Discussion. The results suggest how one overlooked dimension of religion (i.e.. religious beliefs) may bolster the health of older people in the face of adversity.

POSSESSION TRANCE IN A SEMI-URBAN COMMUNITY IN SRI LANKA

A survey of possession trance states in a semi-urban population of 7653 identified 37 subjects. The manifestations of the trance, and its course and outcome are outlined. The characteristics of the subjects, including presence of psychiatric disorder, are compared with that of the total population, and significant differences are noted. Four psychodynamic syndromes observed are described with illustrative case vignettes. The functions subserved by possession behaviour are reviewed, and comparisons are drawn between personal possession, ritual possession, and altered states of consciousness in Western society.

PERSONALITY-CORRELATES OF PLACEBO RESPONSIVITY AND RELIGIOSITY

Performances on measures of field dependence, locus of control, tolerance for ambiguity, and autonomic perception were compared for 11 members of the Hare Krishna movement, 15 Divine Light Mission members, 22 high and low marijuana placebo responders, and 16 university students. The members of the religious groups were less tolerant of ambiguity than the other groups. An hypothesis that the religious subjects would score like the high placebo responders, reflecting a similar reliance on external cues to define internal state, was rejected. Members of the religious movement, however, did have drug-related experiences similar to those of the volunteers for the drug study and unlike those of the college students.

Functional limitations and religious service attendance in later life: Barrier and/or benefit mechanism?

Objectives. Why do some studies report a negative relationship between religious service attendance and functional limitations? Two possible mechanisms. the health benefit mechanism and the functional barrier mechanism. were Systematically examined. Methods. With 2 waves of a national probability sample of adults aged 60 years and older. this research used structural equation models to estimate the influence of these 2 mechanisms. Results. Results indicated that functional limitations were associated with less frequent religious service attendance at the same wave, largely because of the barrier mechanism;no support was found for the benefit mechanism. Neither mechanism was significant over time. Discussion. Findings suggest that there is a temporal and salient decline in social activities such as religious service attendance when lower body functional limitations are highest. However, long-term engagement in religious service attendance is not predicted by baseline functional limitations. indicating that there are not long-term declines in attendance because of higher levels of functional limitations.

Patterns of religious practice and belief in the last year of life

Objectives. Although it is frequently assumed that issues of religious faith become more salient at the end of life. there is little or no population-based empirical evidence testing this assumption. Methods. Using data from the New Haven site of the Established Populations for Epidemiologic Studies of the Elderly (N = 2.812). the authors examined self-reports of attendance at services. self-ratings of religiousness, and strength and comfort felt from religion for respondents who did and did not die within 12 months following an interview. Religiousness was assessed at baseline (1982) and in follow-up interviews in 1985, 1988, and 1994. Cross-sectional comparisons of levels of religiousness were made among persons in their last 6 months of life, persons in their last 12 months of life, and persons who survived 12 months, and longitudinal comparisons were made with religiousness at the previous wave. Results. After adjusting for age, sex, education. marital status, religious affiliation, and a set of health status measures. the authors found that although attendance at religious services declined among the near-deceased, this group showed either stability or a small increase in feelings of religiousness and strength/comfort received from religion. Overall levels of attendance and religious feelings were high for this religiously diverse sample. Discussion. Community studies of respondents in their last year of life are rare. In this sample, religious involvement appears to continue throughout the last months of life.

Context . This study adds to the existing research on religion and health by focusing on the specific practice of prayer and its relationship to health outcomes. Objectives . The purpose of this study is to examine the relationship of frequency of prayer to 8 categories of physical and mental health. Design . The Presbyterian Church, USA, performed data collection as part of an ongoing research program. Members of the Presbyterian Church were randomly selected from the national population and surveyed by mail on their frequency of prayer and their health status, as measured by the Medical Outcomes Study Short-form 36 Health Survey.

Background Jerusalem's psychiatrists expect to encounter. as the millennium approaches. an ever-increasing number of tourists who, upon arriving in Jerusalem, may suffer psychotic decompensation. Aims To describe the Jerusalem syndrome as a unique acute psychotic state. Method This analysis is based on accumulated clinical experience and phenomenological data consisting of cultural and religious perspectives. Results Three main categories of the syndrome are identified and described. with special focus on the category pertaining to spontaneous manifestations. unconfounded by previous psychotic history or psychopathology. Conclusions The discrete form of the Jerusalem syndrome is related to religious excitement induced by proximity to the holy places of Jerusalem, and is indicated by seven characteristic sequential stages.
