目的探讨异位妊娠的发病因素,从重视其发病原因着手,降低异位妊娠的发病率。方法将安阳市第二人民医院2006-01-2009-12收治的异位妊娠130例,按经产妇与未产妇进行分组,分析其发病因素。结果 130例异位妊娠中有人工流产史73例(56.4%),盆腔炎、附件炎32例(24.42%),带宫内节育器31例(23.65%),绝育手术30例(22.9%)。结论异位妊娠的发生与人工流产、盆腔炎、附件炎、带宫内节育器及绝育手术有着密切关系。

Ectopic pregnancy

Ectopic pregnancy occurs in approximately 2% of all pregnancies in the United States, and is the nation's leading cause of first trimester maternal death. Its incidence has increased sixfold in the past 25 years, despite significant improvements in techniques for early diagnosis and management. This article reviews the epidemiology, risk factors, and common clinical presentations of ectopic pregnancy. Both traditional and newly developed strategies for diagnosis and management are described. The primary care physician is in an excellent position to screen for and diagnose ectopic pregnancy, and to counsel patients regarding treatment options and future risks. With the increasing trend toward outpatient nonsurgical management of ectopics, it is expected that the roll of the primary care physician in managing patients with ectopic pregnancy will continue to increase.

Intracytoplasmic sperm injection - Survey of world results

The widespread application of intracytoplasmic sperm injection (ICSI) has raised concern about the efficacy and safety of this novel technique. The European Society of Human Reproduction and Embryology (ESHRE) has established an ICSI Task Force to collect annually the clinical results, the outcome of pregnancy, and the follow-up of children after ICSI using ejaculated, epididymal, and testicular sperm in order to address these important issues in a relatively short time. Over a 3 year span (1993-1995), the number of centers for ICSI increased from 35 to 101, and the total number of ICSI cycles per year rose from 3,157 to 23,932. The incidence of oocytes damaged by the procedure remained low (<10%), whereas the fertilization rates obtained with ejaculated, epididymal, and testicular spermatozoa for 1995 were 64% 62%, and 52%, respectively. Thus, 86-90% of the couples had embryo transfer, and the viable pregnancy rate was 21% for ejaculated, 22% for epididymal, and 19% for testicular sperm, while the incidence of multiple gestations was 29%, 30%, and 38%, respectively. It is noteworthy that no difference was found in ICSI results concerning the etiology of azoospermia, for example, obstructive (congenital or acquired) or nonobstructive. Furthermore, 3,149 transfers of frozen-thawed embryos after ICSI with ejaculated, epididymal, or testicular sperm were performed, and in 11%, 9%, and 7% of them, respectively, a viable pregnancy was achieved. The ICSI results were similar during this 3-year period, irrespective of the origin of the sperm. The perinatal outcome of children born after ICSI was not different from that after in vitro fertilization or natural conception and was only affected by multiplicity. Moreover, the incidence of major or minor malformations was not increased, but the chromosomal, especially the sex chromosomal, aberration rate was slightly elevated (similar to 2%). Worldwide, tubal sterilization is the most commonly chosen form of contraception by women who have completed their desired childbearing. In the United States, it is chosen by more married or formerly married women than any other method. Sterilization has enjoyed increasing popularity since the 1950s, primarily owing to the technologic advances and increasing social acceptance. Improvements in technique and technology have rendered it a safe, effective, and cost-efficient outpatient procedure. This makes tubal sterilization a particularly attractive choice in the managed care setting and in developing countries, where access to medical care occurs most frequently in rural clinic settings.

MANAGEMENT AND OUTCOME OF PREGNANCIES ASSOCIATED WITH COPPER-T INTRAUTERINE CONTRACEPTIVE DEVICE

The clinical outcome of 918 pregnancies in women who conceived while using one or another of four models of the Copper T intrauterine contraceptive device (IUD) is presented. Elective abortion was chosen by 58 per cent of the women. The clinical management and course of 275 women who elected to allow their unplanned pregnancy to continue are described. The Copper T was either expelled or extracted during the pregnancy in 118 of the women, while the IUD remained in situ for the duration of the pregnancy in the remaining 157 patients. The incidence of spontaneous abortion more than doubled when the IUD remained in utero. The chance of a live birth being premature was four times greater when the IUD remained in situ than when it was removed. There were 27 ectopic pregnancies in this entire series, giving an incidence of 3.4 per cent. In addition, the data suggest, but do not prove, that there is an increased risk of having an ectopic pregnancy if the woman has worn the Copper T for more than two years. A benign fibroma of the vocal cord was the only congenital anomaly detected. These observations suggest that the incidence of developmental defects is not adversely affected by the presence of intrauterine copper.

REPEAT ECTOPIC PREGNANCY - STUDY OF 123 CONSECUTIVE CASES

In a study of 1,330 ectopic pregnancies, 123 (9 per cent) patients experienced repeat tubal pregnancies. Of these 123 patients with repeat ectopics, 96 (78 per cent) were surgically sterilized with their second operative procedure. The remaining 27 potentially fertile patients have had nine pregnancies, but only three living children. Cornual wedge resection of the uterine tube predisposed to interstitial pregnancy and to rupture of the uterus in four cases. Four patients had three ectopic pregnancies each. Repair of the involved tube is usually technically possible following linear salpingotomy and should be done if future fertility is desired. If the patient does not desire future pregnancy, she should be sterilized to avoid the risk of repeat ectopic pregnancy.
