The terrorist attacks of Sept 11, 2001, we a horrifying wake-up call for the United States and the rest of the world. The attacks led to the deployment of the disaster medical assistance team (DMAT) from Massachusetts General Hospital in Boston. In this article, members of the team outline what they did during the days after Sept 11 and the lessons they brought back to better prepare their DMAT for the next disaster.

Our Community Hospital: The Evolution of a Rural Primary Care Hospital

She also is a research associate of the North Carolina Rural Health Research Program.2 Bernstein is director of the North Carolina Office of Rural Health and Resource Development in Raleigh NC. He also is codirector of the North Carolina Rural Health Research Program.3 Dihoff is a rural health consultant with the North Carolina Office of Rural Health and is project officer for its Our Community Hospital transition program.

In the next few years, Our Community Hospital, located in the small town of Scotland Neck, NC, will undergo a conversion through which it may serve as an appropriate model for similar small hospitals in distressed rural communities. With technical and grant assistance from the Office of Rural Health and Resource Development of the North Carolina Department of Human Resources, the hospital has begun to phase out almost all acute care services and will expand and strengthen its focus on primary care, emergency medical services, and services for elderly persons This paper addresses four issues of greatest concern to hospital administrators, rural health professionals, academics, and rural residents interested in hospital conversions: (1) community involvement during the planning process;(2) the evolution of the program's structure;(3) financing for the project;and (4) the development of cooperation between state and federal governments, foundations, and private groups. This case study describes one possible course in addressing an acute health care problem facing rural America–the viability of rural hospitals. Copyright © 1992, Wiley Blackwell. All rights reserved

For the past six years he has been involved in studies of occupational stress in the health professions. His current research is concerned with the measurement of quality of life and the cost4Ectiveness of medical technology. THEODORE W. WHITLEY Ph.D. is Director of the Division of Research in the Department of Emergency Medicine at the East Carolina University School of Medicine. Dr. Whitley is currently involved in surveying several groups of emergency health care providers to idenufy sources of Occupational satisfaction and stress. E. JACKSON ALLISON JR M.D. M.P.H. is Professor and Chairman of the Department of Emergency Medicine at the East Carolina University School of Medicine. Dr. Allison is a member of the Board of Directors of the American College of Emergency Physicians and is Chairman-Elect cf the Residency Review Committe for Emergency Medicine. He has been actively involved in the improvement of prehospital care in eastern North Carolina for the past seven years. SAMLRA S. LANDIS R.N. MICN is Coordinator of the Division of Emergency Medical Sewices in the Department of Emergency Medicine at the East Cam lina University School of Medicine. Ms. Iandis has been instrumental in developing advanced life support training programs in eastern North Carolina during the past eight years. She is currently responsible for conducting initial training and continuing education programs for prehospital care providers at advanced levels.

A model of organizational characteristics, occupational stress and mental health in emergency medical technicians (EMTs) is developed and tested. Supervisor behavior and work group support are used as predictors of negative role perception. Negative role perception is specified as intervening between the organizational variables and occupational stress. Occupational stress is hypothesized to directly influence depression. Data collected from 250 EMTs is used in a structural equation analysis to estimate model parameters. The sample is mostly male (74%), white (94%), and married (68%). Results suggest that there is a direct relationship between perceived occupational stress and increased depression. Role perception is a critical intervening variable between supervisor behavior, work group support, and occupational stress. The findings suggest that EMTs are more satisfied when supervisory practices result in an environment which encourages open expression and group problem solving. This work environment leads to more supportive relationships among squad members, reduced role ambiguity, and decreased occupational stress and depression. Copyright © 1988, Wiley Blackwell. Developing an emergency department team for treatment of stroke with recombinant tissue plasminogen activator.

The EMS, ED, neurology, radiology, nursing, laboratory, and pharmacy departments are integral parts of the stroke team system. These are the key departments responsible for the support of the stroke team, or nucleus, which emanates from the departments of neurology and emergency medicine in the author's medical center. In the same way that cardiac victims are treated for "heart attack," so stroke victims should be treated for "brain attack." The emphasis on public awareness to community outreach and senior citizens groups through the aid of the public affairs department of the institution, the American Heart Association, and the National Stroke Association (NSA) has been a tremendous lift in this, the decade of the brain. The Public Broadcasting System and other television channels have helped enormously in publicizing stroke signs and symptoms, new treatment of stroke, and concurrent risk factors. New clinical research for thrombolytics and neuro-protective agents are now in progress to determine the best treatment for the damaged brain. The emphasis is changing: Time is brain. We hope to change the expenditure attributable to rehabilitation and managed care after stroke to the more hopeful prospects of prevention, early treatment, and fast recovery. We can then see that our friends and family members, as victims of stroke, may proceed to independence rather than long-term care facilities or nursing homes. This will be possible only if hospitals and medical centers nationwide adequately gear themselves for the treatment of ischemic stroke through the creation of a core stroke team, finely-honed interdepartmental cooperation, and the development of an efficient ED team that is fully immersed in the ethics and protocol of "brain attack."

A GOAL PROGRAMMING-MODEL FOR THE SITING OF MULTILEVEL EMS SYSTEMS

Facility siting models known as location covering techniques have proven to be useful particularly for emergency medical services (EMS) planning, given the importance of ambulances responding to demand within some maximum time constraint. These models represent a set of methods which focus the health planner's attention on the access of people to health care, since they attempt to “cover” people in need of service within some specified time standard. This research develops a technique for the locational planning of sophisticated EMS systems, characterized by multiple levels of emergency health services. Specifically, a two-tiered system with “basic life support” and “advanced life support” capabilities is modeled as a goal program. By applying location covering techniques within a goal programming framework, this study develops a method for the siting of multilevel EMS systems so that (1) each service level maximizes coverage of its own demand population, and (2) “back-up” coordination between levels is assured. The usefulness of this goal program as a health planning tool is evidenced in the model's explicit articulation of EMS policy objectives and its ability to link system levels in terms of “goal-directed behavior”. The working of this multilevel covering model is demonstrated by reference to EMS planning scenarios and related numerical examples.

The existence of stress, coping with stress, and the effects of stress in Emergency Medical Services (EMS) are highly current and hotly debated subjects. This article describes in part the results of a national survey of 25 EMS systems in 24 large metropolitan areas. The portion of the survey reported here focuses on sources of stress and programs available to promote more effective coping with stress. Results are discussed generally and in terms of the services' organizational affiliates. © 1983 by the haworth press, inc.
